Inner ear anomalies in children with isolated unilateral congenital aural atresia.

OBJECTIVES/HYPOTHESIS: We aim to define the frequencies of anomalies of the inner ear, oval window, and round window ipsilateral to isolated non-syndromic unilateral aural atresia. METHODS AND MATERIALS: Retrospective case series. We reviewed high resolution computed tomography scans of the temporal bones of 70 children with isolated non-syndromic unilateral congenital aural atresia. Scans were reviewed according to the Jahrsdoerfer criteria and further evaluated for anomalies of the vestibule, semi-circular canals, cochlea, internal auditory canal and vestibulocochlear nerve. RESULTS: Inner

CT Imaging Categorization and Biomarker Study of Anomalous Tympanic Segment of the Facial Nerves in Patients With Hearing Loss in the Absence of Microtia.

The purpose of this study is to categorize anomalous tympanic facial nerve (FN) on high-resolution computed tomography (HRCT) and to determinate the significance of associated temporal bone anomalies and congenital syndromes without microtia in patients with hearing loss. A retrospective analysis of HRCT findings in 30 temporal bones in 18 patients with anomalous FN was performed. Abnormalities of the tympanic FN were categorized as follows: category 1: FN medially positioned, but above the oval window; category 2: FN in the oval window niche; and category 3: FN below the oval window. Potentia

The windows of the inner ear.

The oval and round windows of the inner ear are important structures for the transmission of sound and may be affected by a variety of disease entities. The anatomy of this small area is one that often causes the radiology trainee some difficulty, but there are certain disease states that can be easily diagnosed when knowing where and how to look. As this area is very important to the otologist in a variety of preoperative settings, accurate assessment of the windows and recognition of important and potentially complex intra-operative anomalies, will greatly aid our surgical colleagues.

Transmission of change in the atmospheric pressure of the external ear to the perilymph.

In experiments using guinea pigs, the middle ear and perilymphatic pressures were simultaneously registered in response to pressure change in the external ear canal. In the first experiment, pressure was slowly loaded in the ear canal in the range of 200 mm H2O to -200 mm H2O. Pressure transmission to the perilymph was smaller when the bulla was open to the outside than when it was closed. It was significantly impaired by disruption of the ossicular chain and especially by closure of the round window. The data indicate that air volume in the middle ear cavity plays an important role in transmi

Two cases of Tullio phenomenon in which oval and round window reinforcement surgery was effective.

AIM: To report the effect of oval and round window reinforcement surgery performed in two patients with the Tullio phenomenon. Case 1: A male with bilateral superior canal dehiscence syndrome. Downbeat nystagmus with leftward horizontal eye movement was recorded in an electronystagmogram using a pure-tone sound of 110dB at 2000 or 4000Hz in the right ear. Case 2: A female who had undergone stapes surgery. Computed tomography revealed an ossicular prosthesis in the vestibule. An audiogram indicated mild to moderate hearing impairment in the right ear. Leftward horizontal nystagmus was recorded

Observations on bilateral symmetry of the stapedial footplate lesion and narrowing of the oval window niche in otosclerosis.

OBJECTIVES: We consider whether patients with bilateral otosclerosis who have surgery on their second ear show symmetry in the degree of severity of their footplate otosclerosis. METHODS: The severity of the stapedial otosclerotic lesion and the degree of narrowing of the oval window niche by exostoses were recorded for each ear of 269 patients who elected to undergo operation on the second ear. The severities and similarities of the otosclerotic lesion affecting the oval window niche were compared between the ears. RESULTS: The majority of oval window niches were normal in width in both ear

Perilymphatic Fistula Revisited: A Histopathologic Study of Inner Ear Dehiscence and Possible Third Window Syndrome.

HYPOTHESIS: Cases historically presumed to be perilymphatic fistula of the round and/or oval window may be cases of third window syndrome due to inner ear dehiscence. BACKGROUND: Perilymphatic fistula (PLF) is a condition where a pathologic external connection of the perilymphatic space is present, often with an associated otic capsule or stapes defect. Recently, it has become evident that otic capsule defects in locations that lack perilymphatic fluid leak can cause clinical symptoms. Such clinical entities are deemed "third window" syndromes-among which superior semicircular canal dehiscenc

Abnormal facial nerve course associated with stapes fixation or oval window absence: report of two cases.

Congenital minor ear malformations are very uncommon. Their etiology can be explained by the embryological development of the middle ear structures. Their classification, diagnosis and treatment pose certain problems. We report on one case of congenital stapes fixation and one case of oval window absence, both associated with an abnormal facial nerve course. Good long-term results show that performing a platinotomy or vestibulotomy in cases of congenital stapes fixation or oval window absence can be a relatively safe procedure, even in presence of deviated facial nerve course. The literature a

How do tympanic-membrane perforations affect human middle-ear sound transmission?

Although tympanic-membrane (TM) perforations are common sequelae of middle-ear disease, the hearing losses they cause have not been accurately determined, largely because additional pathological conditions occur in these ears. Our measurements of acoustic transmission before and after making controlled perforations in cadaver ears show that perforations cause frequency-dependent loss that: (1) is largest at low frequencies; (2) increases as perforation size increases; and (3) does not depend on perforation location. The dominant loss mechanism is the reduction in sound-pressure difference acro

The double-membrane break syndrome in sudden hearing loss.

Some patients with sudden hearing loss actually have two membrane breaks, one at the oval or round window and one further inside the cochlea. One may heal spontaneously and the other not, or both, or neither. It is very likely impossible to detect this early from the threshold audiogram, because the intracochlear breaks allows endolymph and perilymph mixing which spreads over much of the anatomically normal clchlea causing diffuse loss of function. This theory is illustrated by three case histories.

Whispers from vestigial nubbins: Arrested development provokes trait loss in toads.

Despite the use of acoustic communication, many species of toads (family Bufonidae) have lost parts of the tympanic middle ear, representing at least 12 independent evolutionary occurrences of trait loss. The comparative development of the tympanic middle ear in toads is poorly understood. Here, we compared middle ear development among two pairs of closely related toad species in the genera Atelopus and Rhinella that have (eared) or lack (earless) middle ear structures. We bred toads in Peru and Ecuador, preserved developmental series from tadpoles to juveniles, and examined ontogenetic timing

Imaging of congenital middle ear deafness.

INTRODUCTION: Conductive hearing loss, in the presence of a normal external ear, is usually due to an interference with the normal transmission of sound from the tympanic membrane across the ossicular chain to the oval window. If conductive hearing loss occurs in a young child, congenital middle ear pathology is the most likely cause. MATERIALS AND METHODS: High resolution computed tomography of the middle ear is the diagnostic modality of choice. Details of the imaging technique are outlined in this article. Eight cases of congenital middle ear deafness imaged in the radiological departments

Oval window atresia: a novel surgical approach and pathognomonic radiological finding.

OBJECTIVES: The facial nerve usually occupies the oval window area in patients with oval window atresia. During exploration, if the facial nerve is discovered to lie in the oval window area, this is usually regarded as a contraindication for further surgical intervention. The aim of the present paper is to demonstrate the preoperative pathognomonic radiological sign and describe a new surgical approach for this difficult situation. METHODS: 3 patients and 4 ears were operated due to conductive hearing loss by the same surgeon in a tertiary referral center. Their clinical presentation, radiolo

Computed tomographic image analysis of ears with otosclerosis.

To evaluate the usefulness of quantitative analysis of computed tomographic images in diagnosing stapedial otosclerosis, high-resolution temporal bone computed tomography (CT) of 32 ears with otosclerosis and 19 control ears was used. The images on the section containing the oval window were digitized using a scanner, and the ratio of the mean gray scale value of 5 areas in the optic capsule including the area anterior to the oval window to that of the cortical bone was calculated. The mean value of the ratio of the area anterior to the oval window was significantly lower in the otosclerosis g

Revision stapedectomy: a review of 258 cases.

We reviewed the records of 258 revision stapedectomy operations performed at the Otological Medical Group during an eight year period. Displacement of the prosthesis to the inferior edge of the window was the commonest cause of failure (41%) and occurred predominately in wire-Gelfoam pad cases. An oval window fistula, a short prosthesis or bony closure of the window were causes of failure in 9% each. Incus necrosis was the cause of failure in 5%. Less than 50% of the operations resulted in postoperative conductive deficit of 10 db or less. The results were better than this in incus bypass proc

Ear Cubes for local controlled drug delivery to the inner ear.

A new type of advanced drug delivery systems is proposed: Miniaturized implants, which can be placed into tiny holes drilled into (or close to) the oval window. They consist of two parts: 1) A cylinder, which is inserted into the hole crossing the oval window. The cylinder (being longer than the depth of the hole) is partly located within the inner ear and surrounded by perilymph. This provides direct access to the target site, and at the same time assures implant fixation. 2) A cuboid, which is located in the middle ear, serving as a drug reservoir. One side of the cuboid is in direct contact

Allograft stapes surgery for conductive hearing loss in patients with ossicular chain anomalies.

An allograft stapes was used during surgical intervention for conductive hearing loss due to ossicular malformations in 11 ears of seven patients. The external auditory canal and tympanic membrane were normal in all ears. The surgical findings for the ossicles were stapes fixation (7 ears), stapes fixation with discontinuity of the incudostapedial joint (in both ears of one patient), a deformed incus and stapes crura compressed by the facial nerve (one ear in which the stapes was not fixed) and an anomaly of the incus and stapes combined with a dermoid cyst (in one ear in which the stapes was

Imaging and clinical evaluation of isolated atresia of the oval window.

BACKGROUND AND PURPOSE: Congenital causes of hearing loss in children commonly are encountered, and imaging aids in diagnosis as well as presurgical evaluation. Atresia of the oval window not associated with atresia of the external auditory canal (EAC) is a rare cause of congenital hearing loss in children. We present the clinical and imaging findings in children with isolated oval-window atresia. METHODS: Atresia of the oval window was defined as the absence of the structure with the presence of a bony plate superimposed between the vestibule and middle ear. The bony plate is within the expe
