Lymphatic Drainage of the Abdominal Wall Within the Extended Nodal Dissection Template During Prostate Cancer Surgery-Mapping Overlapping Patterns.

BACKGROUND: The number of lymph nodes (LNs) excised during extended pelvic LN dissections (ePLND) in prostate cancer (PCa) is related to complications including lymphoceles (up to 15%) and lymphedema (up to 14%). Approaches that reduce the resection of LNs related to healthy anatomies may help reduce the complication rate. The aim of this study was to unravel the complexity of the pelvic lymphatics by researching to what extent the lymphatics draining the abdominal wall overlap with the ePLND template and the lymphatic metastatic spread of primary PCa lesions. METHODS: This prospective (NCT05

Resective Surgery for Double Epileptic Foci Overlapping Anterior and Posterior Language Areas: A Case of Epilepsy With Tuberous Sclerosis Complex.

Tuberous sclerosis complex is a genetic systematic disorder characterized by hamartomas in multiple organs. Cortical tubers, the hamartomas in the cerebrum, cause multifocal refractory seizures. In certain cases, epileptic foci potentially involve language areas, and hence, extra- and intraoperative cortical mapping can help identify anterior and posterior areas, thus avoiding postsurgical language impairment. We report on a 21-year-old female with tuberous sclerosis complex experiencing refractory partial seizures due to two epileptic foci in the left hemisphere overlapping anterior and poste

Case Report: Multisystem Autoimmune and Overlapping GAD65-Antibody-Associated Neurological Disorders With Beneficial Effect of Epilepsy Surgery and Rituximab Treatment.

Glutamic acid decarboxylase (GAD) antibodies are associated with disabling conditions such as stiff person syndrome, temporal lobe epilepsy (TLE), limbic encephalitis, cerebellar ataxia (CA), and ocular movement disorders, which are usually chronic and difficult to treat. GAD-related TLE has poor response to anti-seizure medications and immune therapies, and epilepsy surgery is rarely successful. We report on a 47-year-old female with history of migraine, autoimmune thyroid disease, ankylosing spondylitis, and drug-resistant TLE. A video electroencephalography recorded frequent seizures with t

The 30-day readmission rate of patients with an overlap of probable sarcopenia and malnutrition undergoing major oncological surgery.

BACKGROUND: Overlapping sarcopenia and malnutrition may increase the risk of readmission in surgical oncology. Overlapping probable sarcopenia/malnutrition was found in 4.6% of 238 patients and the 30-day unplanned readmission rate was 9.0%. In multivariate analysis, the overlap of probable sarcopenia and malnutrition was a significant predictor for the 30-day unplanned readmission (OR= 8.10, 95%CI= 1.20-0.55; p=0.032). BACKGROUND: ■ Probable sarcopenia plus malnutrition was significantly associated with unplanned readmission. BACKGROUND: ■ Overlap of probable sarcopenia and malnutrition was

Impact of Robotic Surgery on Esophagogastric Gap During Modified Side Overlap With Fundoplication by Yamashita Reconstruction After Proximal Gastrectomy.

INTRODUCTION: Modified side overlap with fundoplication by Yamashita (mSOFY) reconstruction is an antireflux procedure after proximal gastrectomy. Precise alignment between the esophageal and gastric entry holes is important for maintaining an appropriate anastomotic configuration. We evaluated the impact of robotic surgery on the esophagogastric gap (EGG), a novel intraoperative parameter reflecting anastomotic alignment. MATERIALS AND METHODS: We retrospectively analyzed 42 patients who underwent proximal gastrectomy with mSOFY reconstruction between June 2019 and August 2025. EGG was defin

Neoadjuvant therapy vs. surgery alone for esophageal squamous cell carcinoma undergoing thoracoscopic esophagectomy:An overlap weighting analysis.

BACKGROUND: The survival benefit of neoadjuvant therapy (NAT) in patients with esophageal squamous cell carcinoma (ESCC) undergoing thoracoscopic esophagectomy (TE) remains controversial. This study aims to evaluate the long-term survival impact of NAT followed by surgery versus surgery alone (S-alone) in this population. METHODS: This ambispective cohort study retrospectively collected baseline data and prospectively followed patients until July 2022. All enrolled patients had ESCC and underwent TE between January 2015 and December 2016. Patients were divided into the NAT group (n = 269) and

Intravenous to Oral Transition of Amiodarone (IOTA): Effect of Various Durations of Overlap on Atrial Fibrillation Recurrence After Cardiothoracic Surgery.

The use of amiodarone for postoperative atrial fibrillation (AF) is widespread; however, there is a paucity of data on the optimal duration of overlap when transitioning from intravenous (IV) to oral amiodarone. The objective of this study was to evaluate the safety and efficacy of varying durations of overlap when amiodarone IV infusion is transitioned to oral administration in cardiothoracic surgery patients. This retrospective, observational, single-center study included cardiothoracic surgery patients who were initiated on IV amiodarone for supraventricular arrhythmia and subsequently tran

Reduced Surgical Site Infection Risk With Overlapping Knotless Tailless Versus Buried Knot Techniques in Running Subcuticular Closure for Posterior Thoracic and Lumbar Spine Surgery.

BACKGROUND: Are small abscesses and suture reactions seen with buried knots (BK) or tails at the ends of a running subcuticular skin closure using non-barbed absorbable suture (RSAS) associated with surgical site infections (SSIs)? Would the elimination of BK and tails reduce the rate of SSI? The senior author (SK) has been using an overlapping knotless tailless (KT) RSAS method since 2013. As three spine surgeons in the same division at a tertiary referral center primarily used the BK method, we decided to compare deep SSI rates between the BK and KT methods in posterior thoracic and lumbar s

Proximal Femur Salvage in Revision Knee Arthroplasty Due to Oncologic Indications: Long-term Results of Onlay and Overlapping Allograft in Revision Surgeries.

BACKGROUND: Mechanical failures of tumor endoprosthesis in the distal femur usually require revision surgery. We investigated if the proximal femur host bone can be salvaged by onlay and overlapping allograft in revision surgeries due to aseptic loosening and stem fractures. METHODS: We retrospectively reviewed 18 patients (7 men and 11 women) with osteosarcoma around the knee. The entire cohort was classified into three subgroups (no bone graft: 6, onlay allograft: 7, and overlapping allograft: 5) according to our treatment strategy. RESULTS: The median interval from the initial surgery to

Short middle overlap anastomosis reinforced with Toupet-like fundoplication (SMART): a novel antireflux technique for lower mediastinal esophagogastrostomy after esophagogastric junction cancer surgery.

BACKGROUND: Lower mediastinal esophagogastrostomy after proximal gastrectomy with lower esophagectomy for esophagogastric junction (EGJ) cancer remains technically demanding owing to the high risk of anastomotic leakage and reflux. We developed a novel reconstruction technique, the short middle overlap anastomosis reinforced with Toupet-like fundoplication (SMART) method, to address these challenges. METHODS: From March 2017 to April 2025, 40 patients underwent radical surgery for EGJ cancer at our institution. Among them, 10 patients with ≤3-cm esophageal invasion received laparoscopic/robot
