Acute and chronic over-exertion: do depressed immune responses provide useful markers?

There are ethical objections to inducing cumulative muscle damage and associated decrements of performance deliberately in a healthy athlete. Available data on acute and chronic over-exertion thus include the changes of immune response observed following a single bout of exhausting exercise, sequential observations made on top-level competitors as they approach peak training periods, and longitudinal laboratory studies of heavy (but not necessarily damaging) bouts of training. In all three of these situations, subclinical muscle damage initiates an acute inflammatory response, with a resulting

The Impact of Overtraining on Anxiety-Like Behavior and Tight Junction Proteins of the Blood-Brain Barrier in Mice.

PURPOSE: Overtraining Syndrome (OTS) is a chronic maladaptive condition characterized by persistent performance decline due to excessive training and inadequate recovery. OTS is associated with mood disturbances, but the underlying neurobiological mechanisms remain unclear. This study investigated whether a 6-week overtraining (OT) protocol induces anxiety-like behavior and changes in hippocampal BBB-related tight junction proteins in mice. METHODS: Eight-week-old male C57BL/6J mice were assigned to sedentary (SED, n = 5), moderate exercise (ME, n = 5), or overtraining (OT; downhill treadmill

Intrafasciomembranal Fluid Pressure: A Novel Approach to the Etiology of Myalgias.

Fascia is a continuous membrane (fasciomembrane) that enables differentiation of fluid pressure on either side. Fascia membrane also enables an internal increased fluid pressure at all muscle levels (fibers, fiber bundles, skeletal muscles, compartments), and the author introduces a new unifying term for these pressures, regardless of the anatomical level - the intrafasciomembranal fluid pressure (IFMFP). Swelling, pain, and loss of tissue function are identified as common cardinal symptoms in trigger point (TrP), chronic exertional compartment syndrome (CECS), overtraining syndrome (OTS), and

The Effect of an Essential Amino Acid Supplement on Muscle Performance and Inflammation in an Animal Model of Overtraining.

BACKGROUND/OBJECTIVES: Overtraining (OT) syndrome occurs when an athlete's training load exceeds the athlete's ability to recover, leading to a performance decrement. The use of nutritional interventions to increase resiliency to OT is a topic of interest for maximizing performance adaptations. The purpose of this study was to determine whether an essential amino acid (EAA) supplementation administered intraperitoneally and orally can mitigate performance decrements and inflammatory responses in an animal model of resistance training-induced overtraining. METHODS: Thirty-two C57BL/6J mice wer

How to exhaust your bone marrow.

A 32-year-old man was admitted to the hospital because of oedema and 8 kg of gained weight. The oedema decreased spontaneously over weeks and there was no evidence for a nephrotic syndrome; however, the blood tests revealed a moderate pancytopenia. The patient practiced excessive physical activity at work and in his spare time, and kept a very thorough training and weight diary. Owing to a high intake of energy and protein drinks he tried to optimise his physical performance and kept a normal body mass index  at 23.7. A bone marrow biopsy showed gelatinous bone marrow transformation, normally

Chronic exertional compartment syndrome in a patient with restrictive cardiomyopathy and portal hypertension.

Chronic exertional compartment syndrome (CECS) is a condition that produces exercise-induced pain and swelling due to a transient increase in compartment pressures. It is thought to be due to muscle hypertrophy and is classically associated with young athletes under 30, overtraining, anabolic steroid use, and aberrant running biomechanics. We present a unique case of CECS in a patient without the traditional risk factors but rather diagnosed with cardiac cirrhosis and portal hypertension. This patient's exercise-induced bilateral leg pain met the CECS criteria for leg compartment pressure test

Behavioral addictions: a novel challenge for psychopharmacology.

Although addictive syndromes have been traditionally related to substance-use disorders, during the last few decades a novel addictive group, including the so-called "behavioral or no-drug addictions," has been recognized and has attracted increasing attention for its relevant social impact. This group includes pathological gambling, compulsive shopping, TV/Internet/social network/videogame addictions, workaholism, sex and relationship addictions, orthorexia, and overtraining syndrome. Substance and behavioral addictions show similar phenomenological features, such as craving, dependence, tole

Intrafasciomembranal Fluid Pressure: A Novel Approach to the Etiology of Myalgias, Part II.

The fascia forms a hierarchy of spaces (small and large compartments) that contain and enclose muscle fibers, fiber bundles, skeletal muscles, and compartments of several skeletal muscles. Solid fascia serves as a membrane that enables an increased volume and fluid pressure within such a fasciomembrane, an intrafasciomembrial fluid pressure (IFMFP). Increased IFMFP provides a theoretical model and a common explanation for the etiology of the myalgias: trigger point (TrP), chronic exertional compartment syndrome (CECS), overtraining syndrome (OTS), and delayed onset muscle soreness (DOMS). Tick-Borne Illnesses.

Tick-borne diseases are prevalent throughout the United States. These illnesses are caused by a variety of different pathogens that use ticks as vectors, including bacteria, viruses, rickettsia, and protozoa. Some of the most common illnesses caused by ticks are Lyme disease, Rocky Mountain spotted fever, babesiosis, ehrlichiosis, anaplasmosis, tularemia, Colorado tick fever, tick-borne relapsing fever, and Powassan disease. Unique skin changes, fever, and influenza-like symptoms may indicate tick-borne disease. Early diagnosis can be difficult as well as nonspecific and can resemble overtrain

Overreaching-induced oxidative stress, enhanced HSP72 expression, antioxidant and oxidative enzymes downregulation.

Overreaching (OVR) is defined as the initial phase of overtraining syndrome and is known as a metabolic imbalance leading to short-term fatigue. Exercise increases reactive oxygen species production, which can oxidize intracellular structures impairing cell function and thus leads to OVR process. The aim of this work is to study the behavior of oxidative stress markers in subjects submitted to an OVR protocol. Thirty rats were divided in exercise and control group, and submitted to an 8-week-endurance training (ET) and a 3-week-OVR protocol. Thiobarbituric acid reactive substances (TBARs), rea

Fast pitch softball injuries.

The popularity of fast pitch softball in the US and throughout the world is well documented. Along with this popularity, there has been a concomitant increase in the number of injuries. Nearly 52% of cases qualify as major disabling injuries requiring 3 weeks or more of treatment and 2% require surgery. Interestingly, 75% of injuries occur during away games and approximately 31% of traumas occur during nonpositional and conditioning drills. Injuries range from contusions and tendinitis to ligamentous disorders and fractures. Although head and neck traumas account for 4 to 12% of cases, upper e

Exercise and soft tissue injury.

Once the almost exclusive domain of the orthopaedic surgeon, sports injuries are now being seen with increasing frequency by other specialists, including rheumatologists. It is therefore important for rheumatologists to be able to diagnose and manage the various musculoskeletal conditions that are associated with physical activity. Soft tissue injuries are a very common cause of morbidity in both competitive and recreational athletes. Most of these conditions are provoked by muscle-tendon overload (or overuse) that is usually the result of excessive training or improper training techniques. Ho

Brain neurotransmitters in fatigue and overtraining.

Since the publication of the serotonin hypothesis, numerous theories involving the accumulation or depletion of different substances in the brain have been proposed to explain central fatigue. Although the theoretical rationale for the "serotonin-fatigue hypothesis" is clear, several seemingly well-conducted studies have failed to support a significant role for 5-hydroxytryptamine in the development of fatigue. As brain function appears to be dependent upon the interaction of a number of systems, it is unlikely that a single neurotransmitter system is responsible for central fatigue. Several o

[Exercise-induced bronchoconstriction].

Terms exercise-induced asthma (EIA) or exercise-induced bronchoconstriction (EIB) are used to describe transient bronchoconstriction occurring during or immediately after vigorous exercise in some subjects. For the diagnosis of EIB it is necessary to show at least 10% decrease in FEV1 from baseline following physical exercise. The prevalence of EIB has been reported to be 12-15% in general population, 10-20% in summer olympic athletes, affecting up to 50-70% of winter athletes (particularly ski runners and skaters). There are two key theories explaining EIB: thermal and osmotic. Differential d

Physiological Readiness and Resilience: Pillars of Military Preparedness.

Warfighters require a range of physical capabilities to meet the demands of the military profession, and physical training must address performance along an entire continuum, depending on individual needs and mission requirements. Strength and power capabilities are needed for optimal performance of anaerobic tasks such as heavy load carriage, sprinting under load, and maneuvering over uneven terrain. For optimal performance, soldiers must also be able to recover from mission demands and strenuous training. The demands placed on a soldier can result in a chronic stress, leading to decreased mi

A multidisciplinary approach to overreaching detection in endurance trained athletes.

In sport, high training load required to reach peak performance pushes human adaptation to their limits. In that process, athletes may experience general fatigue, impaired performance, and may be identified as overreached (OR). When this state lasts for several months, an overtraining syndrome is diagnosed (OT). Until now, no variable per se can detect OR, a requirement to prevent the transition from OR to OT. It encouraged us to further investigate OR using a multivariate approach, including physiological, biomechanical, cognitive, and perceptive monitoring. Twenty-four highly trained triathl

Influence of Regular Physical Activity on Sleep.

Good sleep and adequate physical activity are essential to health. Yet, large numbers of people are chronically deficient in sleep and physical activity. About 1 in 3 Americans get less than 7 h of sleep per night and only 1 of 4 adults regularly complete weekly physical activity in amounts recommended for good health. This chapter reviews research that has examined relationships between regular physical activity and sleep. The overall weight of evidence supports that regular physical activity is associated with better sleep quality among healthy adults, with epidemiological studies showing mo

Correlations between injury, training intensity, and physical and mental exhaustion among college athletes.

The primary purpose of this research was to obtain information concerning injury incidence and perceptions of training intensities and fatigue levels among college athletes via a survey study. A second purpose was to illuminate correlations between the collected data. This study employed an investigator-designed survey instrument administered to 411 NCAA Division II male and female athletes, with 149 completed responses. The survey included 3 themes: injury incidence, training intensity, and physical and mental exhaustion. Men and women spent 4.5 days per week training using moderate- and high

Polymorphonuclear leucocyte phagocytic function, γδ T-lymphocytes and testosterone as separate stress-responsive markers of prolonged, high-intensity training programs.

Excessive exercise with limited recovery may lead to detrimental states of overreaching or the overtraining syndrome. Chronic maladaptation in endocrine and immune mechanisms occur with the incidence of these states. Exercise-induced cortisol and testosterone responses have been proposed as biomarkers of overreaching, with blunted responses following intensified-training periods. Yet, limited information on the effects of overreaching in immunity is available. Healthy individuals completed a 30-min running protocol (the RPETP) before and after a 12-day intensified-training period. Blood and sa
