Trachoma in the Pacific Islands: evidence from Trachoma Rapid Assessment.

AIMS: To establish the presence or absence of trachoma in the Pacific Island region. METHODS: Trachoma Rapid Assessment methodology was used in Kiribati, Nauru, Vanuatu, Solomon Islands and Fiji. Advised by key informants, high-risk communities were chosen from each country. All available children aged 1-9 years and adults > or = 40 years were examined. RESULTS: A total of 903 adults > or = 40 years and 3102 children aged 1-9 years were screened at 67 sites. Rates of active trachoma in children were >15% in all sites in Kiribati and >20% in all sites in Nauru. However, there was a high varia

Oral Cancer Disparities in the Outer US-affiliated Pacific Islands.

OBJECTIVE/HYPOTHESIS: Oral cancers in the US-affiliated Pacific Islands are poorly described despite disproportionately higher incidences in certain jurisdictions. This study attempts to better characterize the incidence, staging, and management of oral cancers in this region. STUDY DESIGN: Retrospective Epidemiological Study. METHODS: A retrospective review was conducted across the US-affiliated Pacific Islands between 2007 and 2019. Patient data were obtained for individuals with primary head and neck cancers from the Pacific Regional Central Cancer Registry database. All cohorts were age-

Pacific Islands Cohort on Cardiometabolic Health Study: rationale and design.

BACKGROUND: Pacific Islanders, including those residing in the US Affiliated Pacific Islands (USAPI), experience some of the highest mortality rates resulting from non-communicable diseases (NCDs) worldwide. The Pacific Island Health Officers' Association declared a Regional State of Health Emergency in 2010 due to the epidemic of NCDs in the USAPI. Obesity, a known risk factor for NCDs, has become an epidemic among both children and adults in Micronesia and other parts of the USAPI. There is some recent information about overweight and obesity (OWOB) among young children in the USAPI, but the

Acute flaccid paralysis incidence and Zika virus surveillance, Pacific Islands.

PROBLEM: The emergence of Zika virus has challenged outbreak surveillance systems in many at-risk, low-resource countries. As the virus has been linked with Guillain-Barré syndrome, routine data on the incidence of acute flaccid paralysis (AFP) may provide a useful early warning system for the emergence of Zika virus. APPROACH: We documented all Zika virus outbreaks and cases in 21 Pacific Islands and territories for the years 2007 to 2015. We extracted data from the Global Polio Eradication Initiative database on the reported and expected annual incidence of AFP in children younger than 15 y

Rapid mortality transition of Pacific Islands in the 19th century.

The depopulation of Pacific islands during the 16th to 19th centuries is a striking example of historical mass mortality due to infectious disease. Pacific Island populations have not been subject to such cataclysmic infectious disease mortality since. Here we explore the processes which could have given rise to this shift in infectious disease mortality patterns. We show, using mathematical models, that the population dynamics exhibited by Pacific Island populations are unlikely to be the result of Darwinian evolution. We propose that extreme mortality during first-contact epidemics is a func

Centipede envenomation: bringing the pain to Hawai'i and Pacific Islands.

Scolopendra subspinipes is the only clinically significant centipede found in Hawai'i. Envenomation typically leads to extreme localized pain, erythema, induration, and tissue necrosis and possible lymphedema or lymphangitis. Mortality is uncommon and results from secondary infection or anaphylaxis. Management is supportive and includes wound care, pain control, and treatment with topical or oral antihistamines and anti-inflammatory medications.

Ciguatera fish poisoning in the Pacific Islands (1998 to 2008).

BACKGROUND: Ciguatera is a type of fish poisoning that occurs throughout the tropics, particularly in vulnerable island communities such as the developing Pacific Island Countries and Territories (PICTs). After consuming ciguatoxin-contaminated fish, people report a range of acute neurologic, gastrointestinal, and cardiac symptoms, with some experiencing chronic neurologic symptoms lasting weeks to months. Unfortunately, the true extent of illness and its impact on human communities and ecosystem health are still poorly understood. METHODS: A questionnaire was emailed to the Health and Fisher

Rheumatic heart disease across the Western Pacific: not just a Pacific Island problem.

Some of the highest recorded rheumatic heart disease (RHD) prevalence and mortality rates are from the World Health Organization's Western Pacific Region (WPR). RHD burden has been well documented in much of the WPR subregion of Oceania, but less is known about RHD outside the Pacific Islands and Australasia. We aimed to review RHD burden in WPR outside Oceania to identify countries with high RHD burden and those with contemporary data gaps. We searched the peer-reviewed literature for English-language primary studies published between 1980 and April 2017 that reported RHD prevalence or mortal

Hepatocellular Carcinoma in Pacific Islanders vs. Whites.

Pacific Islanders (PI) have amongst the highest incidences of hepatocellular carcinoma (HCC), but large studies characterizing this population are limited. This study explores potential disparities in treatment and outcome by comparison of PIs and Whites with HCC treated at a single center. A prospectively collected database of 1608 patients diagnosed with HCC from 1993-2022 at the only liver disease/transplant center in Hawai'i identified 252 PI and 338 White patients. Demographic, medical history, laboratory, tumor characteristics, treatment, survival data were collected. Chi-square, t-test,

Dental anatomical anomalies in Asians and Pacific Islanders.

Dental anatomical anomalies having a significant impact on endodontic diagnosis and treatment are the talon cusp, tuberculated premolars, three-rooted mandibular molars, and C-shaped molars. Asian and Pacific Islander ethnic groups have the highest percentage of these dental anatomical anomalies compared to the general population. As the population of Asians and Pacific Islanders continues to grow in California and other western states, dentists should be aware of the diagnostic and treatment complexities associated with specific patient groups.

Building Capacity for Cervical Cancer Prevention in U.S.-Affiliated Pacific Islands: The Pacific Against Cervical Cancer Project.

The U.S.-affiliated Pacific Islands (USAPI) have higher cervical cancer incidence and mortality rates and lower screening coverage compared with the United States. This is likely because of economic, geographical, health care delivery, and cultural barriers for women living in these resource-constrained, isolated regions. The most recent U.S. and World Health Organization cervical cancer screening guidelines recommended primary human papillomavirus (HPV) testing as one screening option or the preferred screening modality. Primary HPV screening-based strategies offer several advantages over cur

Expanding Orthopaedic Care Across the Pacific: A Global Health Model Translated Into the Pacific Islands Orthopaedic Association.

Pacific Island countries face persistent and profound gaps in access to essential orthopaedic care driven by geographic dispersion, fragile infrastructure, and workforce shortages. Reliance on overseas medical referral schemes, often politically influenced and consuming substantial portions of limited health budgets, can improve access for individual patients but fails to build local capacity or ensure timely intervention for trauma and infection. Evidence from low- and middle-income settings demonstrates that "train-in-place" education models can expand the workforce, improve care processes,

Visual impairment and its correction among Pacific youth in Aotearoa: findings from the Pacific Islands Families Study.

AIM: Childhood visual impairment has a life-long impact that, with early access to eyecare, is largely avoidable. We aimed to understand visual impairment and its correction among Pacific youth in Aotearoa New Zealand. METHOD: The Pacific Islands Families Study is a birth cohort study that tracks an original sample of 1,398 Pacific children born at Middlemore Hospital (Auckland). This analysis focuses on assessed visual acuity (at 9- and 18-years, using 0.3logMAR or 6/12 as the cut-off for visual impairment) and participants' self-reports about accessing eyecare services. RESULTS: Less than

Characterization of Otolaryngology Referrals among Pacific Islanders in the Commonwealth of the Northern Mariana Islands.

The burden of otolaryngology disease in Pacific Islander populations is relatively uncharacterized. A single-institution retrospective review was undertaken at the Commonwealth Healthcare Corporation in Saipan, the only hospital in the Commonwealth of the Northern Mariana Islands. Demographic, diagnostic, and treatment data were compiled from the clinical charts of all patients seen by an otolaryngologist between January 2015 and April 2020. For all Pacific Islanders in the sample (N=674), the average age was 40.2 (SD 22.4) years and ages ranged from 10 months to 89 years. Patients were 50.7%
