BACKGROUND: Palliative care, perhaps more than any subspecialty in healthcare, is deeply relational and engages patients and families at times of great vulnerability. Ethics of care, or relational ethics, developed through contributions from feminist ethics, offers conceptual tools and ways of thinking that seem especially suited to palliative care practice.
AIM: To identify and describe studies and theoretical analyses applying the ethics of care to palliative care (both adult and paediatric), specifically, its use to guide and improve practice and education for palliative care practitioners

Background: Medicinal cannabis clinical trials in palliative medicine present unique and complex challenges encompassing ethical, legal, and feasibility obligations, making consumer input essential. However, little is known about the consumer contribution in the medicinal cannabis research space. Objective: We present a case report on consumer contribution in the design and conduct of a Phase I/IIb medicinal cannabis clinical trial for anorexia in people with advanced cancer. Our discussion highlights the various ways consumers contribute through (1) lived experience, (2) knowledge, (3) inclus

Palliative care medicine deals with the issue of death by listening to the story of patient's lives. There are several problems such as stress overload or burnout due to the difficulty in responding to all demands from patients and the shortness of time. These problems sometimes make doctors specializing in palliative care have less interest in patients, negative feelings or an indifferent attitude to them. In this report, two cases in which a psychosomatic medical doctor intervened were analyzed. The satisfaction of patients and the stress overload of doctors engaged in palliative care were e

By comprehensively utilizing literature analysis to trace the disciplinary evolution and theoretical foundations of palliative medicine globally and domestically, and employing comparative research to examine the discipline accreditation standards and implementation practices from internationally recognized bodies, including the Royal College of Physicians and Surgeons of Canada and the American Board of Medical Specialties,we clearly define the conceptual definitions and logical relationship between palliative care(as clinical practice)and palliative medicine(as an academic discipline). On this

BACKGROUND: Primary palliative care (PPC) is a specialized field that focuses mainly on the quality of life through symptom management, goal setting/advanced care planning, and psychosocial support in patients with serious illnesses. However, it is underutilized due to misconceptions about its scope and how to provide it. There remains great variability in the preparation of residents and providers in palliative medicine.
OBJECTIVE: This study assessed medical students' level of knowledge and attitudes about PPC concepts.
METHODS: A survey was adapted from previously validated questionnaires

Individually prepared medicinal products represent one of the forms of personalisation of pharmacotherapy in palliative care. We use the full range of individually prepared medicinal products for symptom management and rational supportive care, especially in situations where a medicinal product with appropriate therapeutic properties is not available for the patient.

There is disagreement among physicians and medical ethicists on the precise goals of Hospice and Palliative Medicine (HPM). Some think that HPM's goals should differ from those of other branches of medicine and aim primarily at lessening pain, discomfort, and confusion, while others think that HPM's practices should aim, like all other branches of medicine, at promoting health. I take the latter position: using the ars moriendi to set a standard for what it means to die well, I argue that if HPM's practices were to aim at mitigating suffering with little regard to promoting health, some patien

The author presents a case report of an elderly patient who was extremely frail and debilitated. The patient was a resident of a skilled care facility because of advanced chronic illnesses that included end-stage Alzheimer, dementia, and Parkinson disease. This patient had acquired multiple stages III to IV pressure ulcers. Aggressive wound care treatment was initiated with partial success, but the patient passed away before the wound could completely heal. Medical literature was reviewed to offer the best possible wound care approach in providing care to patients who are terminally ill withou

A history of an escalating chronic intractable pain in a patient with cryoglobulinemia, vasculitis, and severe cutaneous ulcerations is presented. A strategy of progressive, multi-agent, N-methyl-D-aspartate-receptor (NMDA-R) blockade that resulted in adequate pain control and a three-fold reduction in opioid consumption is described. Diagnostic process of neuropathic pain and the role of NMDA-R in the development of hyperalgesia are briefly reviewed. Thereafter, existing clinical literature describing the use of Ketamine, a major NMDA-R antagonist for management of malignant pain, is reviewed

A retrospective study was carried out in 100 adult patients with advanced malignant disease. They were given subcutaneous continuous infusions of medication for symptom relief. The drugs were administered through a butterfly needle inserted subcutaneously in the anterior chest wall using a battery-operated infusion pump. The indications for using this technique were inability to swallow due to deteriorating general condition, oesophageal obstruction, intestinal obstruction, severe nausea and vomiting, terminal dyspnoea and poor pain control with oral opiates. All patients received morphine; ot

OBJECTIVES: Alpha-2 agonists have analgesic and sedative properties that can prove interesting in palliative care. The main objective of this study was to describe the use of clonidine and dexmedetomidine in palliative care units (PCU). The secondary objective was to identify physicians' perspectives and attitudes toward alpha-2-agonists.
METHODS: International multicentric qualitative survey of prescribing characteristics and attitudes towards alpha-2 agonist. All 159 PCUs in France, Belgium and French-speaking Switzerland were contacted, and 142 physicians answered the questionnaire (31% pa

OBJECTIVES: Palliative care patients struggle to cope with many symptoms. Malnutrition and dehydration may develop either as a consequence of the disease itself or as a result of its treatment. Parenteral methods, such as the intravenous method, are advanced treatment options, but sometimes patients experience problems related to venous depletion. Although hypodermoclysis (HDC) has been known for a long time, it is often overlooked in clinical practice. The aim of this study was to analyse the clinical differences between the HDC method and the intravenous hydration method.
METHODS: This pros

INTRODUCTION: Clinical reasoning is essential for hospice and palliative medicine (HPM) fellows, yet structured assessment tools are lacking. The Assessment of Reasoning Tool (ART) has shown positive results for early learners but has not been applied to advanced learners.
METHODS: We conducted an 18-month mixed-methods study at a single academic center. Seven fellows and seven faculty participated. Faculty used the ART every day during clinical teaching rounds on a fellow's inpatient rotation. Pre- and post-intervention surveys assessed the ART via 4-point Likert scale and free-response ques

BACKGROUND: Opioid-induced neurotoxicity (OIN) is a significant complication in palliative care, presenting with symptoms of hypersomnolence, hallucinations, cognitive dysfunction (including delirium), myoclonus and hyperalgesia. Due to limited research studies, a consensus on recognising these clinical manifestations is yet to be reached, complicating both early diagnosis and management.
OBJECTIVES: This rapid review aims to synthesise current evidence on the prevalence, clinical manifestations, risk factors and management strategies of OIN in the palliative care setting to enhance clinical

BACKGROUND: The timely integration of palliative care is important for patients suffering from various advanced diseases with limited prognosis. While a German S-3-guideline on palliative care exists for patients with incurable cancer, a recommendation for non-oncological patients and especially for integration of palliative care into intensive care medicine is missing to date.
METHOD: Ten German medical societies worked on recommendations on palliative care aspects in intensive care in a consensus process from 2018 to 2023.
RESULTS: Based on the german consensus paper, the palliative care a

CONTEXT: Hospice and palliative medicine (HPM) physicians frequently care for patients with substance use disorders (SUDs), but there is no consensus on which primary addiction medicine (AM) skills are essential.
OBJECTIVES: Identify key primary AM skills that physicians should acquire during an ACGME-accredited HPM fellowship program.
METHODS: A modified Delphi study consisting of 18 experts on SUD in HPM and medical education. A literature review and expert input identified initial AM skills. In three Delphi rounds, participants rated each skill on a nine-point scale from "not at all impor

OBJECTIVES: To detail important lessons learnt while conducting several large, medicinal cannabis (MC) randomised clinical trials in a palliative cancer population.
METHODS: Investigators involved in these trials had several meetings to agree on the major lessons learnt and how the various challenges could be mitigated in the future.
RESULTS: The lessons were sorted into separate categories: patient confidentiality, family dynamics, driving, cost, unfounded beliefs, accessing specific MC products, trial funding, telehealth and COVID-19, and miscellaneous issues.
CONCLUSION: Using MC as the
