The effect of adding pancreatin to standard otilinium bromide and simethicone treatment in type 2 diabetes mellitus patients with irritable bowel syndrome.

This study aimed to assess the impact of adding pancreatin (a pancreatic enzyme derivative) to standard treatment on patients diagnosed with Irritable Bowel Syndrome (IBS) and Type 2 Diabetes Mellitus (T2DM). IBS and T2DM frequently coexist, leading to significant gastrointestinal symptoms and a decrease in quality of life. Gastrointestinal symptoms arising in T2DM patients present challenges in management for both clinicians and patients. Standard treatments may not fully address these symptoms. Recent studies suggest that pancreatic enzyme replacement therapy may offer additional benefits. T

Real-world study on efficacy and safety of pancreatin enteric-coated capsules in treatment of postprandial distress syndrome in older patients.

BACKGROUND:: Functional dyspepsia is common among older adults, with postprandial distress syndrome (PDS) as the main subtype. Clinical guidelines recommend prokinetic agents as first-line treatment for symptom relief of PDS. However, concerns over side effects limit their application with older patients. We investigated the efficacy and safety of pancreatin in treating older PDS patients. METHODS:: In this multicenter, real-world study, 175 older patients (age ≥ 60 years) diagnosed with PDS were divided into three groups: itopride monotherapy (56 patients), pancreatin enteric-coated capsule

Comparative evaluation of ammonium chloride-modified and hospital-grade activated charcoal for paracetamol adsorption under simulated gastric (pepsin) and intestinal (pancreatin) fluid.

INTRODUCTION: Paracetamol overdose is a major cause of drug-induced toxicity, and activated charcoal is routinely used for gastrointestinal decontamination. This study compares the adsorption efficiency of ammonium chloride-modified activated charcoal, synthesized from Calligonum comosum wood, with hospital-grade activated charcoal in simulated gastric and intestinal fluid. METHODS: Adsorption experiments were conducted using 20 mg of activated charcoal, in a paracetamol solution containing 2.5 mg of paracetamol (50 mg/L in 50 mL), corresponding to an adsorbent-to-drug ratio of 8:1 at 37 °C.

The combined effects of extended feeding with a high level of Arthrospira platensis and a commercial enzyme mix or porcine pancreatin on broilers' blood cells, plasma metabolites and liver lipid profile.

BACKGROUND: The increasing global demand for sustainable and eco-friendly protein alternatives in poultry production has led to the exploitation of unconventional feed ingredients, such as microalgae. This study is novel for its exploration of the extended feeding effects (from day 7 to day 35) of a high inclusion level (15%) of Arthrospira platensis (Spirulina) in broiler chickens' diet, either alone or supplemented with enzymes, primarily peptidases (EC 3.4). The study assessed the impact on the blood cells, metabolic status and the hepatic content of lipids, pigments and minerals. One hundr

The effect of enzymes on the in vitro degradation behavior of Mg alloy wires in simulated gastric fluid and intestinal fluid.

With an upsurge of biodegradable metal implants, the research and application of Mg alloys in the gastrointestinal environment of the digestive tract have been of great interest. Digestive enzymes, mainly pepsin in the stomach and pancreatin in the small intestine, are widespread in the gastrointestinal tract, but their effect on the degradation of Mg alloys has not been well understood. In this study, we investigated the impacts of pepsin and pancreatin on the degradation of Mg-2Zn alloy wires. The results showed that the pepsin and pancreatin had completely different even the opposite effect

Acid resistant lipase as replacement therapy in chronic pancreatic exocrine insufficiency: a study in dogs.

Conventional treatment of pancreatic steatorrhoea in man has been unsatisfactory because 90% of the lipase content of therapy is inactivated by acid in the stomach and large doses of replacement treatment are needed to provide adequate supplementation. An acid stable agent (fungal lipase) was investigated in the treatment of pancreatic deficiency steatorrhoea in 11 pancreatectomised dogs maintained on a fixed dietary intake of fat and treated with pancreatin or fungal lipase. Ten grams (60,000 U lipase) of pancreatin was compared with 400mg (4800 U lipase) of fungal lipase administered with ea

Reversal of impaired GIP and insulin secretion in patients with pancreatogenic steatorrhea following enzyme substitution.

The influence of impaired digestion on nutrient induced release of gastric inhibitory polypeptide (GIP) and insulin have been investigated in patients with chronic pancreatitis. All patients had massive steatorrhea (> 25 g/24 h), and glucose intolerance. A standard liquid test meal comprising fat and glucose were ingested with or without pancreatic enzyme substitution (9.0 g pancreatin). In the presence of pancreatin the response of serum levels of GIP to the test meal was significantly enhanced (81.2 vs 194.5 microgram/l X 180 min). Concurrently, the insulin response was augmented (3.4 vs 6.4

Effect of cimetidine on enzyme inactivation, bile acid precipitation, and lipid solubilisation in pancreatic steatorrhoea due to cystic fibrosis.

In pancreatic steatorrhoea, both pH-dependent bile acid precipitation and enzyme inactivation may limit the efficacy of pancreatic enzyme supplements and both may be preventable by addition of cimetidine. To separate these effects we compared postprandial jejunal aspirate from eight adults with steatorrhoea due to cystic fibrosis on three randomised treatment regimens (pancreatin, cimetidine, and both together). We also compared the results with those of previous studies of patients on no treatment, and of healthy subjects. On pancreatin 60% of the test meal entered the jejunum at pH less than

Fate of oral enzymes in pancreatic insufficiency.

Oral pancreatic enzyme supplements, including those protected from gastric acidity by enteric coating, often achieve only partial correction of pancreatic steatorrhoea. To characterise the mechanisms involved in vivo, eight patients with steatorrhoea due to advanced pancreatic insufficiency and nine healthy controls were studied. Two sets of studies (small bowel intubation and five day faecal fat quantification) were randomly performed while patients were either on enteric coated pancreatin or equivalent placebo. A 260 cm long multilumen tube was used for double marker perfusion of two 20 cm s

Effects of Pretreatment on the Isotopic Composition of Structural Carbonate in Ray-Finned Fish Teeth.

RATIONALE: The carbon and oxygen stable isotope ratios (δ13C and δ18O) of carbonate contained in fish tooth dentin have the potential to offer insights into environmental and ecological conditions, making them promising targets for analysis in fossil specimens. However, the tooth dentin of extant specimens contains significant amounts of organic matter that can decompose or produce gases that interfere with isotope measurements. Therefore, a well-developed pretreatment protocol for removing organic components prior to isotopic analysis is essential. METHODS: In this study, we developed pretre

Pancreatic Society of Great Britain and Ireland: Annual Meeting, 1980: Role of pH-dependent bile acid precipitation in fat maldigestion due to pancreatic steatorrhoea. Preliminary communication.

In pancreatic steatorrhoea due to cystic fibrosis (CF) a major proportion of the meal enters the jejunum below the critical pH of 5, causing bile acid (BA) precipitation and limiting aqueous solubilisation of lipid. Treatment with pancreatin alone results only in a small increase in lipolysis as the lipase is largely inactivated; aqueous lipid solubilisation is not improved as BA precipitation remains a limiting factor. Treatment with cimetidine alone, by reducing BA precipitation, improves lipid solubilisation without improving lipolysis. Treatment with cimetidine and pancreatin reduces pancr

Evaluation of Sourdough Bread and Its Potential Use in Support of the Treatment of Chronic Non-Communicable Diseases.

Gastrointestinal disorders dysregulate the biochemical environment of the gastrointestinal tract by altering pH conditions during the gastric phase of digestion or by reducing the secretion of pancreatin during the intestinal part of the process. Ingested functional food could therefore lose some of its health-promoting potential apart from its nutritional value. In this work, we aimed to manufacture bread marked by decreased gluten content, using a commercial or laboratory sourdough, that could be appropriate for patients afflicted with wheat allergy, hypertension and pancreatic malfunctions.

Preparation and Characterization of Amylose Inclusion Complexes for Drug Delivery Applications.

Amylose complexes with nimesulide (NMS) and praziquantel (PZQ) were prepared by a simple and low cost method, so that high yield (>57%) and drug content (up to 68.16%) were achieved. The influence of drug:polymer ratio, temperature, and presence of palmitic acid on the complexes properties was evaluated. Differential scanning calorimetry, X-ray diffraction, and nuclear magnetic resonance data evidenced the drug-polymer interaction and the formation of inclusion complexes with semi-crystalline structures related to type II complexes. The drug release rates from complexes were lowered in acid me

Yarrowia lipolytica Lipase 2 Is Stable and Highly Active in Test Meals and Increases Fat Absorption in an Animal Model of Pancreatic Exocrine Insufficiency.

BACKGROUND & AIMS: Pancreatic exocrine insufficiency (PEI) reduces pancreatic secretion of digestive enzymes, including lipases. Oral pancreatic enzyme replacement therapy (PERT) with pancreatin produces unsatisfactory results. The lipase 2 produced by the yeast Yarrowia lipolytica (YLLIP2; GenBank: AJ012632) might be used in PERT. We investigated its ability to digest triglycerides in a test meal and its efficacy in reducing fecal fat in an animal model of PEI. METHODS: YLLIP2 was produced by genetically engineered Y lipolytica and purified from culture media. YLLIP2 or other gastric (LIPF)

Exploration of Potentially Bioactive Peptides Generated from the Enzymatic Hydrolysis of Hempseed Proteins.

The seed of industrial hemp is an underexploited protein source. In view of a possible use in functional foods, a hempseed protein concentrate was hydrolyzed with pepsin, trypsin, pancreatin, or a mixture of these enzymes. A detailed peptidomic analysis using data-dependent acquisition showed that the numbers of peptides identified ranged from 90 belonging to 33 parent proteins in the peptic hydrolysate to 9 belonging to 6 proteins in the pancreatin digest. The peptic and tryptic hydrolysates resulted to be the most efficient inhibitors of 3-hydroxymethyl-coenzyme A reductase activity when tes
