Canadian Paramedic User Requirements for Speech Recognition Aided Documentation.

OBJECTIVE: Developing technology may assist paramedics in the competing tasks of providing life-saving care and accurately documenting that care. The objective of this study was to identify and classify paramedic user requirements for speech recognition (SR)-aided documentation. MATERIALS AND METHODS: This was a two-phase mixed-methods study. Phase one involved a web-based questionnaire distributed to paramedics across Canada. Phase two consisted of opt-in one-on-one interviews to gain further insight into the survey results. Descriptive and associative statistics were used to identify which

Pneumothorax Identified by a Remote Physician Using Paramedic-obtained Tele-ultrasound: Case Report.

INTRODUCTION: The use of telemedicine and ultrasound is emerging and novel in the field of community paramedicine. However, there is a paucity of data supporting its use and even less evidence that shows a morbidity and mortality benefit. This case highlights a unique way to diagnose a common medical emergency, which can lead to a good outcome. CASE REPORT: We describe the use of lung point-of-care ultrasound by a trained community paramedic that led to the identification of a pneumothorax in an 86-year-old male at a scheduled home visit. The images were interpreted over telehealth in real-ti

Prevalence of anxiety, depression, and post-traumatic stress disorder among paramedic students: a systematic review and meta-analysis.

PURPOSE: There are elevated mental health concerns in paramedic students, but estimates vary between studies and countries, and no review has established the overall prevalence. This systematic review addressed this by estimating the global prevalence of common mental health disorders, namely anxiety, depression, and post-traumatic stress disorder (PTSD), in paramedic students internationally. METHODS: A systematic search of six databases, including MEDLINE, EMBASE, PsycINFO, CINAHL, Scopus, and medRxiv, was conducted to identify studies relating to mental health among paramedicine students.

A Quality Improvement-based Approach to Implementing a Remote Monitoring-Based Bundle in Transitional Care Patients for Heart Failure.

BACKGROUND: Congestive heart failure (HF) is a leading cause of hospitalization and readmission, leading to increased health care utilization and cost. This is complicated by high incidence, prevalence, and hospitalization rates among racial and ethnic minorities, with a widening in the mortality disparity gap. Remote patient monitoring (RPM) has the potential to proactively engage patients after discharge to optimize medication management and intervene to avoid rehospitalization. However, it also may widen the equity gap due to technological barriers and bias. METHODS: A prospective, observa

Preparing undergraduate student paramedics to consider their mental health during clinical placement in Australia.

INTRODUCTION: This article explores the impact of mental health issues on paramedics in Australia, particularly Post-Traumatic Stress Disorder, caused by their exposure to high levels of stress. The prevalence of Post-Traumatic Stress Disorder is higher among paramedics than any other occupation, and this could be a cause for concern, especially for undergraduate student paramedics. The article examines the need to build resilience among student paramedics to help them handle the trauma they may experience during clinical placement. METHODS: This study conducted a two-step process to review l

A randomised study on the comparative performance of German paramedics after technical versus non-technical skills training.

BACKGROUND: The working environment of pre-hospital emergency medicine represents the framework conditions of high-risk organisations. Non-technical skills are an important foundation for successful performance. In Germany, a new job profile - advanced paramedic - was introduced in 2014, which for the first time also included non-technical skills in the curriculum. Another special feature is that for the first time it is regulated how paramedics proceed with selected extended procedures, such as the application of selected medication. The advanced paramedic has a mandatory annual training requ

Emergency Care for Persons Living with Dementia.

Assessments are available which can help to identify emergency department (ED) patients who may have dementia. Persons living with dementia (PLWD) have unique emergency care needs such as difficulties with activities of daily living and behavioral and psychological symptoms of dementia. Multiple emergency care practices have shown improved outcomes for PLWD. Communication between clinicians and PLWD requires additional attention and should include care partners and other clinicians when possible. Transitions of care from the ED for PLWD are often problematic with poor outcomes. Interventions l

[Acute renal failure caused by plant extract].

The authors review the case of a 30 years old female patient presenting with a 48 hours-standing anuria, who permanently used products of grist of a virtuous plant, Guarana and occasionally used a parenteral non-steroid painkiller. The clinical history and laboratory results showed acute renal and hepatic failure. The histological picture of the renal biopsy specimen verified an acute tubular necrosis. After temporary dialysis treatment, her renal function recovered progressively with compensatory polyuria. The authors would like to draw the attention to the risks of the use of over-the-counte

Mental health acts and out-of-hospital care: Legislative boundaries and the paramedic role in Australian mental health legislation.

BACKGROUND: Changes to Australian mental health legislation have expanded the role of paramedics in managing mental illness. Paramedics in most Australian states and territories can now temporarily detain and transport people suspected of being mentally ill for further assessment if these people are viewed as being at risk or as posing a risk to others and as lacking capacity for decision making. AIM: This paper examines current mental health legislation from the eight Australian states and territories. We explore the conditions for transport to involuntary care and additional powers granted

[Califesse study: from a field problem to the implementation of paramedical research].

In neonatology, diaper rash in premature babies under 37 weeks of amenorrhea is a frequent occurrence. The lack of definition and homogeneity of practices around this subject, and the call for tenders from the Auvergne-Rhône-Alpes region's inter-regional grouping for clinical research and innovation, motivated teams from the Annecy-Geneva hospital center to become promoters of a paramedical research project, Califesse. The study was divided into two stages: the first to create a visual scale, the second to carry out an epidemiological description of diaper rash and to propose a reference frame

Advanced clinical insights & practice: ischemic heart disease.

This issue sees the debut of a new series of continuing education articles. The series, Advanced Clinical Insights & Practice, is designed to provide continuing education to an ever-expanding realm of paramedicine that needs more of it: the critical care transport paramedic. Secondly, and equally important, are the benefits that can be reaped by other certification levels reading this feature. For EMT-Basics and Intermediates, it will provide a great enhancement to your core knowledge, although most of the interventions discussed will be beyond your traditional scope. Building bridges to outpatient treatment services for post-overdose care via paramedic buprenorphine field initiation.

INTRODUCTION: Despite sustained efforts to reduce opioid-related overdose fatalities, rates have continued to rise. In many areas, overdose response involves emergency medical service (EMS) personnel administering naloxone and transporting patients to the emergency department (ED). However, a substantial number of patients decline transport, and many EDs do not provide medication for opioid use disorder (MOUD). One approach to filling this gap involves delivering MOUD to overdose patients in the field with trained post-overdose EMS teams who can initiate buprenorphine. In this MOUD field initi

"It's more than just blood pressure and stitches": a critical examination of paramedic services from the perspectives of intimate partner violence advocates.

BACKGROUND: As a detrimental and persistent criminal, social, and health issue, intimate partner violence (IPV) is a severe threat to the well-being of survivors. The healthcare system, specifically the emergency department, has provided access to services and supports for survivors of IPV when seeking healthcare. However, many survivors of IPV are unable to attend the hospital and access the corresponding services. Paramedics, versatile in-community service providers, could play a vital role in linking survivors to healthcare and support services. In-community IPV advocates understand the cir

A European paramedic curriculum for geriatric emergency medicine developed via a modified Delphi technique.

BACKGROUND: Older emergency patients currently account for most European emergency medical service dispatches. Due to demographic changes and increasing comorbidities in advanced age, this number is expected to rise substantially in the coming years. Prehospital professionals require specialised training to provide high-quality care for complex, multimorbid patients. The aim of this study is to define minimum competencies for paramedic education in Europe on the management of emergencies in older adults. METHODS: A modified electronic Delphi study was performed from January 2023 to November 2

Research on the road: Partnering with community emergency medical services to expand access to clinical trials.

Traditional site-based clinical trials impose substantial logistical burdens that limit participation among individuals with geographic, mobility, transportation, and socioeconomic constraints, thereby undermining research equity and generalizability. Although pragmatic and decentralized trial designs seek to address these barriers, many health systems lack a mobile clinical workforce capable of conducting in-person research activities in community settings. Mobile Integrated Health (MIH) and Community Paramedicine (CP) programs, which leverage community paramedics and EMTs (MIH-CPs) to provid

Variation in prehospital ACS care within a single city: a bicentric observational study (MONAH-1 subgroup analysis).

BACKGROUND: Acute coronary syndrome (ACS) is a time-critical medical emergency in which early guideline-based prehospital diagnosis and treatment are crucial for the subsequent care pathway. The aim of this study was to compare documented adherence to selected prehospital ACS process indicators between two provider structures operating within the same municipal EMS system. METHODS: As part of the retrospective, bicentric observational study MONAH-1, all prehospital physician missions with typical ACS diagnoses in Magdeburg between 2014 and 2018 were analysed. This prespecified intra-urban sub

Kolb learning styles in Tactical Combat Casualty Care (TCCC): a comparative study of military learners and civilian paramedics.

BACKGROUND: Tactical Combat Casualty Care (TCCC) is a widely used training framework for battlefield trauma care. Despite its growing implementation in both military and civilian contexts, data comparing learning style distributions between military and civilian populations receiving TCCC-related training remain limited. OBJECTIVE: This study aimed to compare Kolb learning styles between professional soldiers receiving tactical medic training and paramedics working in prehospital emergency services. METHODS: This cross-sectional descriptive study included 61 male participants: 39 military pers

Accuracy of final-year paramedic students' ECG interpretation and referrals for transient loss of consciousness, per the NICE CG109 guidelines.

Paramedics in the United Kingdom independently assess patients with transient loss of consciousness (TLOC) in community settings. The UK's National Institute for Health and Care Excellence publishes clinical guidelines on assessment of TLOC (NICE CG109), recommending ECG interpretation to identify red-flag abnormalities that warrant referral. This study assessed whether final-year university paramedic students meet these recommendations uniformly across different ECG patterns. Our hypothesis was that participants will identify the Brugada syndrome (a required but difficult-to-recognize ECG pat
