Patellar Tendon Reconstruction After Failed Patellar Tendon Repair With a Concomitant Double-Bundle Posterior Cruciate Ligament Reconstruction and Medial Collateral Ligament Reconstruction.

BACKGROUND: Patellar tendon ruptures are a relatively uncommon injury that results from a contraction of the quadriceps against a fixed eccentric load during partial knee flexion. The medial collateral ligament (MCL) and posterior cruciate ligament (PCL) are frequently injured together, and their reconstitution should follow anatomic reconstruction techniques. INDICATIONS: In a previously failed patellar tendon repair, a reconstruction with an allograft is indicated due to poor tissue quality. The failed patellar tendon repair was further complicated by a chronic PCL tear and a chronic high-g

Morphometric analysis of patella and patellar ligament: a cadaveric study to aid patellar tendon grafts.

PURPOSE: Morphometric analysis of the patella and the patellar ligament is crucial in diagnosing and surgical corrections of knee injuries and patellofemoral joint disorders. Dimensions of the patella and the patellar ligament are frequently used in implant design and ACL reconstruction. This study aims to obtain detailed morphometric data on the patellar ligament and its localization based on gross anatomical dissections in the adult cadavers. METHODS: The present study consisted of 50 lower limbs from formalin-fixed male adult cadavers aged about 70 years (45-85) belonging to the South Indi

Effects of medial patellofemoral ligament reconstruction on patellar tracking.

PURPOSE: Medial patellofemoral ligament (MPFL) reconstruction has been performed to treat recurrent patellar dislocation. However, the effects on patellar tracking have not been well documented, particularly in patients. The purpose of this study is to compare patellar tracking pattern and chondral status at MPFL reconstruction with those at second-look arthroscopy. METHODS: Between 1999 and 2008, 71 patients with recurrent patellar dislocation underwent MPFL reconstruction using a double-looped semitendinosus tendon. Of these, 25 knees in 24 patients underwent second-look arthroscopy (at 6-2

Traumatic Patellar Tendon Rupture Repair Using Synthetic Ligament Augmentation.

Rupture of the patellar tendon is an infrequent pathology, and surgical repair with nonabsorbable sutures is the gold standard for management. Many surgeons augment the repair using one of many proposed methods: cerclage wires, Dall-Miles cables, autologous hamstring grafts, and tendon allografts. In this study, we propose the augmentation of patellar tendon repair using an artificial ligament. The questions to be assessed in this study are as follows: (1) measurement of functional results 1 year after surgery using the Lysholm score and (2) the incidence of both rerupture, and surgical compli

Reconstruction of medial patellofemoral ligament for chronic patellar instability.

BACKGROUND: Disruption of the capsule, medial patellar retinaculum, and/or vastus medialis obliqus has been associated with recurrent patellar instability. Biomechanical studies have shown that the medial patellofemoral ligament (MPFL) is the main restraint against lateral patella displacement and reconstruction of the MPFL has become an accepted surgical technique to restore patellofemoral stability in patients having recurrent patellar dislocation. We report a prospective series of patients of chronic patellar instability treated by reconstruction of medial patellofemoral ligament. MATERIAL

The load applied to the foot in a patellar ligament-bearing cast.

The purpose of this study was to determine whether a patellar ligament-bearing cast reduces the load applied to a foot in a cast. In a study of ten people who had no history of gait abnormalities, disease involving the motor system, or deformities of the lower extremities, we compared the load applied to the plantar aspect of a foot in a cast (as detected with F-Scan computer-monitored pedobarographic sensors) with the total load that an extremity in a cast receives relative to the ground (as detected with force-plates). Six trials were completed three times by each person. The trials consiste

Postoperative rehabilitation of simultaneous rupture of anterior cruciate ligament and patellar ligament: A case report.

INTRODUCTION: The simultaneous rupture of anterior cruciate ligament (ACL) and patellar ligament (PL) is an infrequent condition. Each isolated injury has surgical techniques and rehabilitation protocols that differ widely among each other. Nonetheless, there is no established physical rehabilitation approach when both injuries are associated. OBJECTIVE: The aim of this report is to describe the rehabilitation and the outcomes obtained in the postoperative period of simultaneous rupture of ACL and PL and the follow-up period. CASE REPORT: A 21-year-old male patient suffered the rupture of AC

Neglected patellar tendon rupture with anterior cruciate ligament rupture and medial collateral ligament partial rupture.

Anterior cruciate ligament injuries are common, mostly occurring during sports trauma. Conversely, patellar tendon ruptures are uncommon injuries secondary to forceful contractions of the extensor mechanism. It is important to consider the possibility of combined injuries around the knee. We present a case of neglected patellar tendon rupture with anterior cruciate ligament and medial collateral ligament rupture treated with arthroscopic anterior cruciate ligament reconstruction combined with patellar tendon reconstruction.

Arthroscopic reconstruction of anterior cruciate ligament using BTB patellar ligament in the press-fit technique.

Arthroscopic reconstruction of the anterior cruciate ligament (ACL) with a bone-patellar tendon-bone graft in the press-fit technique is an efficient procedure, which provides high primary stability without use of implants and thus ensures early functional rehabilitation. The procedure is based on graft harvest with an oscillating hollow saw, which allows collection of cylindrical bone blocks. Femoral fixation using a template and file has been standardized to an extent that ensures secure press-fit fixation. Tibial anchorage is achieved by refilling the tibial tunnel with the bone cylinder re

Ultrasound and Elastosonographic Features of the Patellar Ligament in Dogs Affected by Cranial Cruciate Ligament Disease.

This study aims to evaluate the morpho-functional change in the patellar ligament in dogs with cranial cruciate ligament disease. We hypothesized that it may show increased thickening and stiffness with increasing days from onset to diagnosis instead of trauma. Understanding this aspect has implications for the choice of timing for treating patients suffering from cranial cruciate ligament disease, as well as the contextualization of patellar ligament desmitis pictures after surgical treatment with tibial plateau leveling osteotomy or tibial tuberosity advancement. Thirty-three dogs affected b

Ligament augmentation reconstruction system artificial ligaments in patellar tendon reconstruction - a chronic patellar tendon rupture after multiple operations: A case report.

BACKGROUND: Patellar tendon rupture is a rare disease, and reports regarding patellar tendon reconstruction with ligament augmentation reconstruction system (LARS) ligaments are limited, with only three reports available in the literature. LARS ligaments are made of polyethylene terephthalate and have been certified as a more favorable option than other tendon transplants. To our knowledge, this is the first report of patellar tendon reconstruction with LARS for suture fixation due to poor quality of the tendon after multiple operations to enable early mobilization and quick rehabilitation. All-Arthroscopic Reconstruction of the Medial Patellofemoral Ligament Using Artificial Ligament for Recurrent Patellar Dislocation.

Medial patellofemoral ligament injury is the primary stability barrier after patellar dislocation, with high recurrence rates with conservative treatment, making reconstruction surgery increasingly mainstream. This article describes an all-arthroscopic technique for medial patellofemoral ligament reconstruction using high-strength synthetic suture tape combined with suture anchors, eliminating the need for tendon harvesting. It involves anatomic localization of the patellar and femoral insertion sites, with appropriate tensioning and fixation at 30° of knee flexion to restore medial stability.

Superior pole patellar sleeve fracture after medial patellofemoral ligament reconstruction.

A 13-year-old boy sustained a superior pole patellar sleeve fracture after a blunt trauma of the knee. A medial patellofemoral ligament reconstruction had been performed 4 weeks earlier. The patient underwent surgery to reinsert the quadriceps tendon in the superior pole of the patella using two bone suture anchors. One-year postoperative assessment revealed excellent functional outcomes. Sleeve fractures are easily misdiagnosed, and in our case, clinical examination was essential in order to perform the diagnosis.
