Mother knows best: medical record management for patients with spina bifida during the transition from pediatric to adult care.

OBJECTIVES: We report the results of the visual evoked potentials (VEP) examination in patients after severe poisoning by methanol. MATERIAL AND METHODS: The group of 47 patients (38 males and 9 females) was assembled out of persons who survived an outbreak of poisoning by the methanol adulterated alcohol beverages, which happened in the Czech Republic in 2012-2013. The visual evoked potentials examination was performed using monocular checkerboard pattern-reversal stimulation. Two criteria of abnormality were chosen: missing evoked response, and wave P1 latency > 117 ms. Non-parametric stati

Donor information considered important to donors, recipients and offspring: an Australian perspective.

Donor conception research supports open-identity donor programmes and disclosure to donor-conceived offspring. This study examines Australian donors', recipients' and donor-conceived offspring's views on the importance of different types of biographical information about the donor. Participants (125 recipients, 39 donors (known, identity-release and anonymous), 23 donor-conceived offspring) completed an online or paper self-administered anonymous questionnaire. Individuals rated the importance of 15 types of biographical information and subsequently chose the three they deemed most important.

Complex wounds: a partnership approach to patient documentation.

A concern regarding the lack of effective communication experienced within Ayrshire in relation to patients with complex wounds initiated the development of a patient-held record. The aim of the patient-held record was to promote patient empowerment by providing a method by which patients and professionals share information. Interested stakeholders worked in partnership to produce a draft record which comprises three main sections: a core section, wound section and personal notes section. Other sections which can be included, where relevant, are nutrition, continence and podiatry. After obtain

Cancer Patients' Sharing of Electronic Health Records with Informal Caregivers.

Patients' online record access (ORA) enables patients with complex diagnoses like cancer to involve their informal caregivers in care management by sharing health information, either through proxy access functionality or informally. The extent to which cancer patients use electronic health records (EHRs) for information sharing is unknown. Using the NORDeHEALTH 2022 Patient Survey, we compared cancer and other patients' reasons for using the EHR in Sweden. We found that although the majority of respondents did not access their records with the purpose of sharing with family or friends, cancer

Understanding the response of mammography facilities to breast density notification.

BACKGROUND: State-specific breast density notification legislation requires that women undergoing mammography be informed about breast density, with variation among states. Because mammography facilities are among the main points of contact for women undergoing mammography, research is needed to understand how facilities communicate information on breast density, cancer risk, and supplemental screening to women. METHODS: A cross-sectional, 50-item, mailed survey of 156 American College of Radiology-certified mammography facilities in North Carolina was conducted in 2017 via the Tailored Desig

Promoting access through complementary eHealth technologies: recommendations for VA's Home Telehealth and personal health record programs.

BACKGROUND: Many healthcare organizations have embraced eHealth technologies in their efforts to promote patient-centered care, increase access to services, and improve outcomes. OBJECTIVE: Using the Department of Veterans Affairs (VA) as a case study, this paper presents two specific eHealth technologies, the Care Coordination Home Telehealth (CCHT) Program and the My HealtheVet (MHV) personal health record (PHR) portal with integrated secure messaging, and articulates a vision of how they might be implemented as part of a patient-centric healthcare model and used in a complementary manner t

The impact of penicillin allergy de-labelling on the WHO AWaRe antibiotic categories: a retrospective cohort study.

BACKGROUND: The WHO's AWaRe classification categorizes antibiotics into three stewardship groups: Access, Watch and Reserve. The Access group includes antibiotics with lower resistance potential than antibiotics in the other two groups. The UK five-year AMR strategy has set targets for reducing non-Access antibiotic use. The majority of penicillins are in the Access group and therefore patients with a penicillin allergy record are likely to receive more non-Access antibiotics. This study aimed to quantify the impact of penicillin allergy records on non-Access antibiotic prescribing and to esti

Federated healthcare record server--the Synapses paradigm.

The delivery of healthcare relies on the sharing of patient information between those who are providing for the care of the patient and this information is increasingly being expressed in terms of a 'record'. Further, it is desirable that these records are available in electronic form as Electronic HealthCare Records. As it is likely that patient records or parts of records will be stored in many different information systems and in the form of disparate record architectures, uniform access to patient records would be problematic. This paper presents an overview of the Synapses computing envir

Organ Procurement Organizations and the Electronic Health Record.

The adoption of electronic health records (EHRs) has adversely affected the ability of organ procurement organizations (OPOs) to perform their federally mandated function of honoring the donation decisions of families and donors who have signed the registry. The difficulties gaining access to potential donor medical record has meant that assessment, evaluation, and management of brain dead organ donors has become much more difficult. Delays can occur that can lead to potential recipients not receiving life-saving organs. For over 40 years, OPO personnel have had ready access to paper medical r

A Narrative Review of Sociodemographic Disparities in Relation to PEHR Access.

Personal electronic health records (PEHRs) enable patients access to their own medical records. Differences in access and use of PEHRs may create health disparities. We conducted a narrative literature review regarding the effects of race, language preference, education, income, and homelessness on PEHR usage as well as PEHRs content, particularly stigmatizing language. Of 3177 citations found, 75 articles were relevant. Patient race, language, income, and education predicted PEHR use, which could potentially exacerbate health disparities.

Electronic medical records and patient privacy.

Networks of health care providers and payers increasingly rely upon electronic databases to coordinate and analyze medical information. Benefits of linking databases include saving lives and reducing costs. However, access to and use of medical records raise serious privacy concerns. Potential misuse of records may harm patients and undermine the quality of health care. In the clinical setting, stringent security procedures must be instituted, including limited access to electronic records, audit trails, and security audits. Patient consent should be obtained before personal information is dis

The impact of disclosure on donor gamete participants: donors, intended parents and offspring.

PURPOSE OF REVIEW: The present review examines recent publications that provide insight into how the trend toward nonanonymity and disclosure in gamete donation impacts donors, intended parents, and their donor-conceived children. RECENT FINDINGS: Recent findings show an increase in donor programs that offer open-identity between donors and offspring. The psychological needs of gamete donors and their attitudes toward disclosure are increasingly given consideration. Qualitative research on how parents of donor gamete offspring make decisions about disclosure reveals that even when couples ini

Improving Electronic Health Record Access for People Incarcerated in the United States.

Around the United States, carceral medical records are often sequestered from incarcerated patients and their external physicians, hampering the provision of both acute and transitional care. Despite laws meant to ensure better ownership and portability of electronic health records (EHRs), many carceral institutions have not implemented interoperable systems, nor do incarcerated patients have easy access to their medical information. These issues can compromise care for a patient population that experiences higher rates of many diseases, hospitalization, and death compared with the general pub

Heart disease severity level identification system on Hyperledger consortium network.

Electronic Health Records (EHRs) play a vital role in the healthcare domain for the patient survival system. They can include detailed information such as medical histories, medications, allergies, immunizations, vital signs, and more. It can help to reduce medical errors, improve patient safety, and increase efficiency in healthcare delivery. EHR approaches are proven to be an efficient and successful way of sharing patients' personal health information. These kinds of highly sensitive information are vulnerable to privacy and security associated threats. As a result, new solutions must devel

Evaluating LLMs' Potential to Identify Rare Patient Identifiers in Patient Health Records.

This study explores the utility of Large Language Models (LLMs) to support finding rare patient record details that could make a patient identifiable. Whilst most research has focused on what we call direct patient identifiers, indirect patient identifiers are not widely addressed. Our evaluation of patient records with mentions of indirect risks predicted by our LLM shows the potential to find these risks automatically. However, many risks highlighted were false positives or did not constitute identifiable risk. More work is needed to understand how we can harness the potential of LLMs as par

Patient interest in recording family histories of cancer via the Internet.

PURPOSE: This study investigated the interest of mammogram patients in using electronic tools for recording their family histories of cancer (FHC). METHODS: Semistructured interviews were conducted with 65 patients visiting a breast center at a referral hospital in Cleveland, Ohio. RESULTS: Most (n = 40; 62%) respondents expressed interest in using an electronic tool for recording FHC and associated its use with a range of benefits to themselves, their families, and their health care providers. Women who were not interested (n = 25; 38%) in using an electronic tool for recording FHC were con

Enhancing holistic patient care of black individuals diagnosed with HIV and comorbid chronic conditions.

BACKGROUND: Pharmacists can play a crucial role in delivering the holistic care model. Research suggests that HIV-positive African American patients particularly benefit from this approach compared to other communities. Typically, pharmacists in community pharmacy settings are limited in their access to a patient's medical record, preventing holistic care delivery. OBJECTIVES: This study addressed the impact of access to medical records on a holistic patient care approach among pharmacists engaged in medication therapy management. METHODS: This was a sub-analysis of a randomized controlled t

Pediatric oncology healthcare professionals' attitudes to and awareness of regulations for minors' and guardians' online record access: a mixed-methods study in Sweden.

BACKGROUND: Healthcare providers and policymakers worldwide differ in their provision of access to adolescent patients' electronic health records (EHR). The regulatory framework in Sweden restricting both guardians' and adolescents' online record access (ORA) has during recent years received criticism. The aim was to quantitatively and qualitatively, explore attitudes about ORA and perceptions about ORA regulations among pediatric oncology healthcare professionals (HCPs) in Sweden. METHODS: A convergent mixed-methods design (QUAL, quan) was used, consisting of a survey study (N = 95) and semi
