Prediction of Disposition Within 48 Hours of Hospital Admission Using Patient Mobility Scores.

Delayed hospital discharges for patients needing rehabilitation in a postacute setting can exacerbate hospital-acquired mobility loss, prolong functional recovery, and increase costs. Systematic measurement of patient mobility by nurses early during hospitalization has the potential to help identify which patients are likely to be discharged to a postacute care facility versus home. To test the predictive ability of this approach, a machine learning classification tree method was applied retrospectively to a diverse sample of hospitalized patients (N = 761) using training and validation sets.

An Analysis of Transient Ischemic Attack Practices: Does Hospital Admission Improve Patient Outcomes?

INTRODUCTION: Immediate treatment has been shown to decrease the recurrence of cerebrovascular accidents following transient ischemic attacks (TIA), prompting the use of a specialized neurologic emergency department (Neuro ED) to triage patients. Despite these findings, there is little evidence supporting the notion that hospital admission improves post-TIA outcomes. Through the lens of a Neuro ED, this retrospective chart review of TIA patients examines whether hospital admission improves 90-day outcomes. MATERIALS AND METHODS: Two hundred sixty charts of patients discharged with TIA diagnos

Same-day admission for cardiac surgery: a benefit to patient, family, and institution.

A pilot program was conducted in an effort to understand the advantages of admitting patients requiring cardiac surgery on the day of the scheduled surgical procedure. A total of 143 patients over a 4-month period were admitted on the day of surgery. Patients required repair of atrial septal defect (adult), coronary artery bypass grafts, and/or heart valve replacements. Patients, family members, and significant others verbalized positive feelings about the program. Both nurses and physicians assert that a high quality of patient care was rendered through this program. Additionally, the institu

Active surveillance of vancomycin-resistant Enterococcus faecium after its outbreak at patient admission.

Vancomycin-resistant Enterococcus faecium (VRE) causes high mortality and has been increasingly detected worldwide in recent years; however, its screening effectiveness at admission remains controversial. We aimed to evaluate the effectiveness of active surveillance culture (ASC) for VRE at admission following a nosocomial outbreak. A VRE outbreak occurred at Shizuoka General Hospital in 2022, and ASC for VRE was performed upon admission for high-risk cases after the outbreak between January 2023 and December 2023. ASC was conducted in 2941 patients at admission, with 13 of them testing positi

Comorbidity Patterns in Patients at Cardiovascular Hospital Admission.

Hypertension frequently coexists with obesity, diabetes, hyperlipidemia, or metabolic syndrome, anditsassociation with cardiovascular disease is well established. The identification and management of these risk factors is an important part of overall patient management. In this paper, we find the most relevant patterns of hospitalized patients with cardiovascular diseases, consideringaspects of their comorbidities, such as triglycerides, cholesterol, diabetes, hypertension, and obesity. To find the most relevant patterns, several clusterizations were made, playing with the dimensions of comorb

Admission thyroid function testing in elderly patients.

The free thyroxine index (FTI) was measured in 307 elderly patients admitted to two geriatric units. The initial FTI was abnormal in 41 (13.3%), being raised in 27 and low in 14 patients. On further testing, these abnormalities were found to be either transient or of no clinical significance in 36 of the 41 patients (88%). Only five of the 307 patients (1.6%) were finally treated for previously undiagnosed thyroid disease and of these three improved (1%). Thyroid disease identified by routine screening, and not suspected from the history and clinical findings, was present in only two patients

Drinking by alcoholic patients following hospital admission.

Ad lib fluid intake of abstinent alcoholics and alcoholics in acute withdrawal exceeded that of psychiatric controls for 3 d following admission to hospital. The urinary output of abstinent alcoholics reflected their high fluid intake, but alcoholics in withdrawal displayed an initial oliguria. Routine blood and urine analysis did not provide any evidence of dehydration among alcoholics in withdrawal. Some possible causal factors contributing to a generalized alcoholic polydipsia are outlined.

Admission of Pediatric Concussion Injury Patients: Is It Necessary?

BACKGROUND: Currently there is no consensus on the management of patients with a concussion and negative computed tomography (CT) of the head. This study examined the necessity of admitting pediatric patients with concussive symptoms. The purpose of this study was to determine if pediatric patients evaluated in the emergency department (ED) for concussion with a negative head CT scan require routine hospital admission. MATERIALS AND METHODS: A retrospective chart review of pediatric trauma patients admitted to the hospital for a concussion from 2010 to 2017 was conducted after IRB approval (1

[Suicidal behavior of depressed patients at inpatient admission].

Early recognition of suicidal behavior is of extreme importance for avoidance of this cause of death. The risk of suicide attempts is particularly high in psychiatric illness, and among hospital in-patients especially in those with depression. Thus, the risk of suicide should be estimated in the first interview after admission to a psychiatric hospital. Our study analysed the routine documentation of such interviews on admission to the Psychiatrische Klinik der Freien Universität in Berlin: 41% of all depressive patients were suicidal. Suicidal and non-suicidal depressive patients could be dif

Unscheduled Hospital Admission as a Prognostic Factor in the Oncologic Patient: A Retrospective Study.

Aims This research aimed to determine the correlation between survival, symptoms, and unscheduled admission in oncologic patients. Furthermore, this study aimed to develop a prognostic model that helps clinicians establish the indication of intervention by palliative care teams. Methodology A retrospective study of patients' digital clinical history registry was conducted to meet the two core objectives. The study population was patients with solid tumors undergoing unscheduled admissions to the oncology ward between January 1, 2018, and May 31, 2018. Demographic and clinical variables of thos

Pressure Injuries at Intensive Care Unit Admission as a Prognostic Indicator of Patient Outcomes.

BACKGROUND: Pressure injuries, also known as pressure ulcers, are a serious complication of immobility. Patients should be thoroughly examined for pressure injuries when admitted to the intensive care unit to optimize treatment. Whether community-acquired pressure injuries correlate with poor hospital outcomes among critically ill patients is understudied. OBJECTIVES: To determine whether pressure injuries present upon admission to the intensive care unit can serve as a predictive marker for longer hospitalization and increased mortality. METHODS: This study retrospectively analyzed admissio

[Management of the patient with craniocerebral injuries at the accident site and clinic admission].

Between January 1991 and December 1992, there were 686 rescue operations involving patients with craniocerebral trauma in the catchment area of Ulm. There were 376 patients who had to be graded as seriously injured according to the NACA classification. In 178 cases there was a severe craniocerebral trauma, and 131 of these patients were admitted to the traumatology department of the University of Ulm. The pattern of injuries was analysed; multiple injuries were found in 63 patients, with injuries to the extremities and the thorax being most-frequent. The primary preclinical treatment for patie

Assessing Patient Understanding and Adherence to Preoperative Medication Advice Provided in Pre-Admission Clinic.

Background: Appropriate medication management before surgery is essential to minimise perioperative risk. Patient adherence to preoperative medication advice demonstrates considerable variability and is influenced by multiple interacting factors. This study assessed patient understanding and adherence to preoperative medication advice provided in the Pre-Admission Clinic (PAC) and identified factors contributing to non-adherence. Methods: A cross-sectional survey study was conducted over 12 weeks in 2022 at a tertiary hospital. Adult patients scheduled for elective surgery who received preoper

Prediction of postoperative patient deterioration and unanticipated intensive care unit admission using perioperative factors.

BACKGROUND AND OBJECTIVES: Currently, no evidence-based criteria exist for decision making in the post anesthesia care unit (PACU). This could be valuable for the allocation of postoperative patients to the appropriate level of care and beneficial for patient outcomes such as unanticipated intensive care unit (ICU) admissions. The aim is to assess whether the inclusion of intra- and postoperative factors improves the prediction of postoperative patient deterioration and unanticipated ICU admissions. METHODS: A retrospective observational cohort study was performed between January 2013 and Dec

A retrospective study of patient care needs on admission to an inpatient hospice facility.

BACKGROUND: The care and support of dying patients and their families are among the most important skills of a family physician. In this century, an increasing proportion of deaths have occurred in hospitals with resulting medicalization of the dying process. Hospice care has emerged to focus on the relief of suffering rather than the cure of illness. This descriptive study reports information about the diagnoses, care needs, and attending physicians of a cohort of patients admitted to a free-standing, inpatient hospice program. METHODS: We undertook a retrospective chart review of 335 patien
