GRACE targets home-based, geriatrics team management, primary care collaboration, and protocols for common conditions. GRACE can improve outcomes and reduce acute-care utilization. We aimed to characterize medication-related GRACE recommendations. Medical record review of Indiana University Health Physicians GRACE patients (June 2012 to September 2013), with six months' follow-up was conducted. Demographics, clinical characteristics, and recommendations were summarized. Mean age (N = 156) was 82 years; 71% were women, 24% black, and 34% living alone, with a mean of 12 medications. With improved survival after hematopoietic cell transplantation (HCT), the number of individuals at risk for persistent or late effects is increasing. The importance of optimizing HCT survivor health and well-being is mounting. Fatigue is a common post-transplantation symptom that impairs quality of life, yet it remains poorly understood and inadequately addressed. Multiple challenges to addressing fatigue exist, including its multidimensional presentation, multiple (often concomitant) causes, patient-clinician communication barriers, and few highly effective, evidence-based interventions that

BACKGROUND: A multidisciplinary approach to improve postoperative outcomes in frail elderly patients is gaining interest. Multidisciplinary team care should be targeted at complex patients at high risk for adverse postoperative outcome to limit the strain on available resources and to prevent an unnecessary increase in patient burden. This study aimed to improve patient selection for multidisciplinary care by identifying risk factors for disability after cardiac surgery in elderly patients.
METHODS: This was a two-centre prospective cohort study of 537 patients aged ≥70 yr undergoing elective

BACKGROUND: Opioid use disorder (OUD) care engagement rates in primary care (PC) settings are often low. Little is known about PC team experiences when delivering OUD treatment and potential factors that influence their capacity to engage patients in treatment. Exploring PC team experiences may inform needed supports that can optimize OUD care delivery and improve outcomes for patients with OUD.
OBJECTIVE: We explored multidisciplinary PC team perspectives on barriers and facilitators to engaging patients in OUD treatment.
DESIGN: Qualitative study using in-depth interviews.

INTRODUCTION: Healthy eating supports diabetes management and may reduce colorectal cancer risk, yet patients in underserved clinic settings often face structural barriers that limit dietary behavior change. This study examined patient-care-team and health-system factors influencing diet counseling, nutrition referral, and food-resource linkage in two community clinics.
METHODS: Before launching a patient-facing colorectal cancer and colon-healthy nutrition education intervention, we conducted an exploratory study to inform the intervention design with 23 patient-care-team members from two co

Chronic kidney disease and dialysis have a major impact on patients’ quality of life, affecting their physical, psychological, and social well-being. The DialiCare project, conducted in partnership with an industry stakeholder and health care professionals involved in dialysis care, aims to address these challenges through a collaborative, patient-centered approach. Structured in two phases, the project first identified the priority needs of patients and health care professionals through surveys and regional working groups. Various project proposals were then submitted to a jury made up of a p
