Financial Waste is Patient Harm: Reducing Wasteful Healthcare Spending Increases Safety and Health.

Wasteful healthcare spending has more than economic costs-there are clinical costs as well. Waste increases out-of-pocket costs, delays necessary care, and worsens outcomes. We argue that, while most quality frameworks do not consider this patient harm, they should. Financial waste is clinically on par with medical errors and adverse events, as overtreatment and low-value care injure patients, care coordination failures lead to preventable complications and death, pricing failures cause delayed and complicated care, and low-value procedures cause complications. We reviewed the literature to ti

First, do no harm: bilateral radial artery occlusion in a COVID-19 patient.

OBJECTIVE: Objective of this case report is to draw attention to a less known thrombotic complication associated with COVID-19, i.e., thrombosis of both radial arteries, with possible (long-term) consequences. THE CASE: In our COVID-19 ICU a 49-year-old male patient was admitted, with past medical history of obesity, smoking and diabetes, but no reported atherosclerotic complications. The patient had been admitted with severe hypoxemia and multiple pulmonary emboli were CT-confirmed. ICU-treatment included mechanical ventilation and therapeutic anticoagulation. Self-harm in burn patients: An analysis of Finnish patient records (2011-2020).

AIM: To collect data on self-harm burn patients at a national level in Finland and analyze patient characteristics. MATERIAL AND METHODS: First, we went through The National Care Register for Health Care (Hilmo) records from 2011 to 2015 to find all patients in Finland with both burn and self-harm ICD10 codes. Then we investigated the medical records of all patients treated at the National Burn Centre (NBC) in Helsinki in the period 2011-2020. Patients admitted to the hospital because of self-harm burn injuries were compared to those without self-harm injuries. Patients below 18 years old wer

Patient Harm From Orthodontic Care (Part I): Types and Frequency of Patient-Reported Harms From a National Database.

BACKGROUND: Patient safety is a fundamental prerequisite for high-quality healthcare and central to clinical practice, including orthodontics. Despite the widespread emphasis on safety in healthcare delivery, knowledge specific to patient safety in orthodontics remains limited. AIMS: (1) To analyse the reasons why patients file claims for compensation related to orthodontic care, (2) to summarise the events that lead to patient harm, and (3) to describe the types of harms associated with the provision of orthodontic care. MATERIALS & METHODS: Data from the Danish Dental Compensation Associat

A Multicenter Collaborative Effort to Reduce Preventable Patient Harm Due to Retained Surgical Items.

BACKGROUND: Unintentional retention of surgical items is severe but preventable patient harm in surgical procedures. One multicenter health care organization experienced a harm event due to retained surgical items (RSIs) every eight days in 2019-2020. The research team sought to reduce the incidence of harm due to RSIs, improve near-miss reporting, and increase process reliability in operating rooms across the organization. METHODS: A total of 114 health care facilities in the organization were invited to participate in a multistate, multicenter patient safety initiative to reduce patient har

The Adverse Drug Event Collaborative: a joint venture to measure medication-related patient harm.

AIM: To measure the extent of patient harm caused by medications (rate of Adverse Drug Events) in three DHBs, using a standardised trigger tool method. METHODS: Counties Manukau, Capital and Coast and Canterbury DHBs decided to work collaboratively to implement the ADE Trigger Tool (TT). Definitions of ADE were agreed on and triggers refined. A random sample of closed charts (from March 2010 to February 2011) was obtained excluding patients who were admitted for <48 hours, children under the age of 18 and psychiatric admissions. In each DHB trained reviewers scanned these in a structured way

IS VAGINAL DELIVERY HARMFUL TO PATIENTS WITH PSEUDOXANTHOMA ELASTICUM?

BACKGROUND/PURPOSE: To investigate the effect of a vaginal delivery (VD) on retinal pathology in patients with pseudoxanthoma elasticum. METHODS: Retrospective case series. All 14 consecutive women with pseudoxanthoma elasticum who visited the ophthalmology department during pregnancy and after delivery between 2010 and 2018 were included. Prepartum and postpartum imaging consisted of color imaging, near-infrared imaging, and optical coherence tomography and was assessed on occurrence of (sub)retinal hemorrhages and change in angioid streaks. RESULTS: Fourteen patients (15 deliveries) were i

The delivery of futile care is harmful to other patients.

OBJECTIVE: Intensive care units (ICUs) in different parts of the world provide care to patients with advanced age and terminal illness at different rates and in different patterns. In the United States, ICU beds make up a disproportionate number of acute care beds. Nearly half of all patients who die in U.S. hospitals have received ICU, some of which may be futile. The objective of this study was to examine ways in which the delivery of futile care in the ICU can cause harm to patients other than those receiving the futile care. DESIGN: Review of available studies of patient and family attitu

Discussions of Potential Mammography Benefits and Harms among Patients with Limited Health Literacy and Providers: "Oh, There are Harms?".

Starting breast cancer screening at age 40 versus 50 may increase potential harms frequency with a small mortality benefit. Younger women's screening decisions, therefore, may be complex. Shared decision-making (SDM) is recommended for women under 50 and may support women under 55 for whom guidelines vary. How women with limited health literacy (LHL) approach breast cancer screening decision-making is less understood, and most SDM tools are not designed with their input. This phenomenological study sought to characterize mammography counseling experiences among women with LHL and primary care

Take care of the patient: Pharmacists should advocate for overdose prevention centers as harm reduction.

Overdose prevention centers (OPCs) are places where people can bring previously obtained substances to use under the supervision of trained health care professionals who are prepared to intervene in the event of an overdose. Health care professionals, counselors, and peer support provide services including naloxone distribution, provision of sterile injection equipment, counseling services, referrals to necessary providers, and above all a safe and compassionate space for people to get the care they need. This is a method of harm reduction that has been used worldwide for decades and is now ga

Engaging health care workers to prevent catheter-associated urinary tract infection and avert patient harm.

Preventing catheter-associated urinary tract infection (CAUTI) remains a significant challenge for US hospitals. The "On the CUSP: Stop CAUTI" initiative represents the single largest national effort (involving >950 hospitals) to mitigate urinary catheter risk. The program brings together key organizations to assist state hospital associations and hospitals by providing education and coaching support, addressing both the technical aspects of preventing CAUTI and CAUTI-specific socio-adaptive challenges. At the local level, engaging health care workers, from physicians and nurses to other ancil

Deliberate self-harm in alcohol and drug misusers: patient characteristics and patterns of clinical care.

The aims of this study were to describe the characteristics of alcohol and drug misusers presenting to a general hospital following suicide attempts and to investigate the patterns of clinical care they received before and after the attempts. The Oxford Monitoring System for Attempted Suicide and patient case-notes were used to obtain information on alcohol and drug misusers assessed by the general hospital psychiatric services after deliberate self-harm in 1992. Of 724 patients, 200 (28%) were substance misusers (36% of males, 23% of females). Both alcoholics and drug misusers were more likel

Creatine supplementation in patients with kidney disease - harm or benefit?

Creatine is one of the most widely used nutritional supplements and has a well-established role in cellular energy metabolism, skeletal muscle performance, and exercise capacity. More recently, growing evidence suggests that its biological effects extend beyond ergogenic benefits to include the preservation of muscle mass and strength, neuroprotection, modulation of inflammation and oxidative stress, and maintenance of cellular homeostasis. These properties are particularly relevant in clinical conditions characterized by sarcopenia, frailty, and neurocognitive impairment, including chronic ki
