Optimizing patient positioning and understanding radiofrequency energy in gynecologic surgery.

Safety in the operating room environment should be a priority for every surgeon. Two areas where safety can often be overlooked and taken for granted are patient positioning and electrosurgical use. Understanding neuroanatomical relationships can help the surgeon optimally position patients and minimize the incidence of postoperative neuropathy while maintaining functional and unobstructed access to the patient and surgical field. Given the widespread use of radiofrequency devices in daily surgical practice, a working knowledge of the principles of electrosurgery is vital to the gynecologic su

Multiregional assessment of surface guidance for patient positioning in radiotherapy.

Accurate and reproducible patient positioning is critical in external beam radiotherapy to ensure optimal target coverage while sparing surrounding healthy tissue. This retrospective single-institution study compared surface-guided radiation therapy (SGRT) using the Catalyst™ HD system with conventional skin-marker (SKM) alignment in thoracic and pelvic treatments. Sixty patients (20 thoracic, 40 pelvic) and 1,105 treatment fractions were analyzed. Thoracic patients received both SGRT and SKM workflows sequentially, enabling paired analysis; pelvic patients were treated using either SGRT or SK

The Impact of Patient Positioning on the Compromised Respiratory System.

Body positioning exerts profound physiological effects in critically ill patients, influencing respiratory mechanics, ventilation-perfusion matching, hemodynamics, and overall clinical outcomes. Transitioning from an upright to a recumbent position alters thoracoabdominal pressure relationships, reduces ribcage contribution to tidal ventilation, and shifts ventilatory load to the diaphragm. These changes decrease functional residual capacity and promote airway closure in dependent lung regions, predisposing patients to atelectasis-an effect further amplified by intra-abdominal pathology or dia

Optimization of patient positioning for improved healing after corneal transplantation.

Corneal transplantation is the only solution which avoids loss of vision, when endothelial cells are dramatically lost. The surgery involves injecting gas into the anterior chamber of the eye, to create a bubble that pushes onto the donor cornea (graft), achieving sutureless adherence to the host cornea. During the postoperative period, patient positioning affects the bubble. To improve healing, we study the shape of the gas-bubble interface throughout the postoperative period, by numerically solving the equations of fluid motion. Patient-specific anterior chambers (ACs) of variable anterior c

Supine vs decubitus lateral patient positioning in vertebral fracture assessment.

In vertebral fracture assessment (VFA), lateral scans are obtained with the patient positioned supine (C-arm densitometers) or lateral decubitus (fixed-arm densitometers). We aimed to determine the impact of positioning on image quality and fracture definition. We performed supine and decubitus lateral VFA in 50 postmenopausal women and used the algorithm-based qualitative method to identify vertebral fractures. We compared the 2 techniques for the identification of fractures (kappa analysis) and compared the numbers of unreadable vertebrae (indiscernible endplates) and vertebrae that were pro

[Minimally conscious coma patients--patient positioning systems in general practice].

With a view to optimizing the positional management of patients with severe neurological damage, five positioning systems with different operating principles were submitted to suitability trials in a clinical neurological rehabilitation setting. The patients included were in the "Wachkoma", the minimally conscious state (i.e., in neurological early rehabilitation phase B). The experience collected by the various professions involved in positional management over a trial period of six months is summarized in the present report, indicating that none of the positioning systems used has been suita

[Patient positioning on the operating table in neurosurgery: sitting or lying].

Efficacy and safety of microvascular decompression of trigeminal nerve depending on the position on the operating table were assessed in 200 neurosurgical patients in retrospective observational study It was shown that efficacy doesn't depend on positioning. Lying position eliminates probability of such complications as postural hypotension, hypotension during surgery, tension pneumocephalus and peripheral nerves injury. Sitting position increases risk of air venous embolism by 25 times. Lying position increases risk of postoperative nasal liquorrhea by 4 times, but eliminates risk of postoper

[Damage due to patient positioning in anesthesia and surgical medicine (1)].

Positioning a patient for surgery requires great care and caution. Correct positioning provides the surgeon with good access to the site, minimizes blood loss and reduces the risk of damage to nerves, soft tissue, compartments and the cardio-pulmonary system. Each position has its specific risks. These have to be evalued against the benefits. Extreme positions of the joints should be avoided whenever possible. The ulnar nerve or the plexus brachialis are at highest risk in the positioning of extremities. Good anatomical comprehension makes it possible to take effective counter-measures. In the

Patient positioning for robot-assisted laparoscopic benign gynecologic surgery: A review.

Robotic surgical platforms are now in widespread use in the practice of gynecology all over the world. The introduction of robotic surgery has required some modifications of patient positioning when compared to standard laparoscopic surgery. Optimal patient positioning is likely to be the most essential step of robotic surgery as it provides the technical feasibility to have adequate access to the pelvic structures for performing the surgery. It is prudent to pay attention to preventing patient shifting in Trendelenburg position because of tendency of sliding down toward the direction of the h

The Importance of Patient Positioning in Radiography When Utilising Automatic Exposure Control.

INTRODUCTION: Automatic exposure control (AEC) is a device used in radiographic practice which terminates a radiographic acquisition when a certain exposure threshold is reached. The AEC is commonly utilised for in-bucky imaging, such as for the chest, abdomen, pelvis and spine. There are many benefits to utilising the AEC where awareness and knowledge of the technology are thoroughly understood. Poor technique when utilising the AEC, however, will alter dose, and by extension, image quality. This study aimed to assess how changes in patient positioning when using AEC for chest, abdomen, lumba
