Surgical planning and patient-specific biomechanical simulation for tracheal endoprostheses interventions.

We have developed a system for computer-assisted surgical planning of tracheal surgeries. The system allows to plan the intervention based on CT images of the patient, and includes a virtual database of commercially available prostheses. Automatic segmentation of the trachea and apparent pathological structures is obtained using a modified region growing algorithm. A method for automatic adaptation of a finite element mesh allows to build a patient-specific biomechanical model for simulation of the expected performance of the implant under physiological movement (swallowing, sneezing). Adolescent-Parent Dyad and Adolescent Eating Disorders: A Standardized Patient Simulation.

Many medical students are uncomfortable with adolescent patients and the associated difficulties of navigating speaking with an adolescent alone while ensuring confidentiality. To address this, students are encouraged to effectively use the home, education and employment, eating, activities, drugs, sexuality, suicide/depression, and safety (HEEADSSS) assessment, which is a history-taking method designed to reveal information about a patient's home life, social life, and self-image. We created this observed structured clinical examination (OSCE) for learners to practice their communication skil

Combining simulated patients and simulators: pilot study of hybrid simulation in teaching cardiac auscultation.

Auscultation torsos are widely used to teach position-dependent heart sounds and murmurs. To provide a more realistic teaching experience, both whole body auscultation mannequins and torsos have been used in clinical examination skills training at the Medical Faculty of the University of Muenster since the winter term of 2008-2009. This training has since been extended by simulated patients, which are normal, healthy subjects who have undergone attachment of the electronic components of the auscultation mannequins to their chests to mimic pathophysiological conditions ("hybrid models"). The ac

Written simulation of patient-doctor encounters. 3. Comparison of the performance in the simulation with prescription and referral data in reality.

The performance of 19 general practitioners in a written simulation of patient-doctor encounters was compared with real data on prescription and referral from 'sick-fund' patients. Two issues were studied: the amounts of certain drugs prescribed (analgesics and antirheumatics; hypnotics/sedatives and tranquillizers; neuroleptics and antidepressants) and the number of referrals to specialists. In the simulation, experts assessed the 'risk of unnecessary harm to the patient' induced by the therapeutic procedures of these general practitioners, as described previously. A close correlation was fou

A differentiable simulation of the eye for patient-specific strabismus surgery planning.

PURPOSE: Up to 4% of adults will develop strabismus in their lifetime. The most common surgical intervention involves adjusting the length of one or more extraocular muscles to correct the angular deviation. This correction depends on surgical expertise and statistical reference tables, which often fail to yield optimal results for patients with atypical eye morphology. Our work proposes a physics-based modeling approach to personalized surgical planning, accounting for patient-specific eye anatomy. METHODS: We built a physics-based simulator of the eye and its muscles, incorporating patient-

Learning from Latent Safety Threats Identified During Simulation to Improve Patient Safety.

BACKGROUND: Latent safety threats (LSTs-characteristics of design, processes, or physical environment in health care compromising patient safety) are commonly revealed during simulation-based training. Methods of collecting, analyzing, and classifying LSTs are underdeveloped and not standardized. Building on a large simulation program in one organization, the authors aimed to collect LSTs systematically and develop a taxonomy to classify them. METHODS: The authors modified the Press Ganey Healthcare Performance Improvement Failure Modes Taxonomy (HPI-FMT), a standardized framework for safety

Toward Patient-specific Pessary to Manage Pelvic Organ Prolapse: Design and Simulation.

This study proposed a novel design and personalized approach to developing an intra-vaginal device, also known as a pessary, for the treatment of Pelvic Organ Prolapse (POP). Although POP is likely to have a more diverse dynamic than other health conditions in women, it is currently treated as a "one-shape-fits-all" problem in all cases. Pessaries are conservative devices inserted into the vagina to support its internal structure and predominantly come in a ring shape design. Failure rates as high as 50% within the first year of use have been attributed to the poor design of these pessaries; w

Simulation to minimise patient self-inflicted lung injury: are we almost there?

Computational modelling has been used to enlighten pathophysiological issues in patients with acute respiratory distress syndrome (ARDS) using a sophisticated, integrated cardiopulmonary model. COVID-19 ARDS is a pathophysiologically distinct entity characterised by dissociation between impairment in gas exchange and respiratory system mechanics, especially in the early stages of ARDS. Weaver and colleagues used computational modelling to elucidate factors contributing to generation of patient self-inflicted lung injury, and evaluated the effects of various spontaneous respiratory efforts with

Optimization of a Transcatheter Heart Valve Frame Using Patient-Specific Computer Simulation.

PURPOSE: This study proposes a new framework to optimize the design of a transcatheter aortic valve through patient-specific finite element and fluid dynamics simulation. METHODS: Two geometrical parameters of the frame, the diameter at ventricular inflow and the height of the first row of cells, were examined using the central composite design. The effect of those parameters on postoperative complications was investigated by response surface methodology, and a Nonlinear Programming by Quadratic Lagrangian algorithm was used in the optimization. Optimal and initial devices were then compared

Patient-specific computational fluid dynamic simulation of a bilateral bidirectional Glenn connection.

Computational fluid dynamics (CFD) have been used to investigate the hemodynamic performance in cavopulmonary anastomosis and resulted in improved operative design. In this study, CFD simulations were performed in a patient-specific bilateral bidirectional Glenn (BBDG) connection model and the power losses as well as flow features at different levels of predetermined pulmonary flow splits were calculated and compared. The control volume power loss varied between 0.64 and 1.02 mW when the flow ratio of left pulmonary artery/right pulmonary artery changed from 80:20 to 20:80. The flow patterns w

Olfactory Cues and Patient Safety Behavior in Virtual Reality Medical Simulation: Controlled Trial.

BACKGROUND: Simulation-based medical education is essential for improving patient safety. In virtual reality (VR)-based simulation, immersion is primarily generated through visual and auditory cues, while other sensory modalities are typically absent. This sensory limitation may reduce the emergence of authentic safety-relevant behaviors. Olfaction plays an important role in clinical reasoning, risk perception, and self-protective behavior and is closely linked to memory and emotion. Although olfactory cues have been shown to influence hand hygiene behavior in real or simulated environments, t

3D-Printing in Neurosurgical Training: Hands-On Practice, Patient-Specific Planning and Simulation.

Training in neurosurgery is increasingly challenged by the complexity of cranial anatomy, the rarity of certain pathologies, ethical constraints on cadaveric dissection, and limited operative exposure. Advances in three-dimensional (3D) printing have enabled the development of patient-specific anatomical models derived from computed tomography and magnetic resonance imaging, offering realistic, reproducible platforms for surgical training and preoperative planning. This chapter reviews contemporary applications of 3D-printed, hybrid, and multimodal simulation models in neurosurgical education,

Finite Element Simulation of Clubfoot Correction: A Feasibility Study Toward Patient-Specific Casting.

Background: Congenital talipes equinovarus (clubfoot) affects 1-2 per 1000 newborns worldwide. The Ponseti method, based on staged manipulations and casting, is the gold standard for correction. However, the biomechanical processes underlying these corrections remain poorly understood, as infants rarely undergo imaging. Computational modeling may offer a non-invasive approach to studying correction pathways and exploring novel applications, such as customized casts. Methods: We developed a proof-of-concept framework using iterative finite element analysis (iFEA) to approximate the surface-leve
