Esophageal Heat Transfer for Patient Temperature Control and Targeted Temperature Management.

Controlling patient temperature is important for a wide variety of clinical conditions. Cooling to normal or below normal body temperature is often performed for neuroprotection after ischemic insult (e.g. hemorrhagic stroke, subarachnoid hemorrhage, cardiac arrest, or other hypoxic injury). Cooling from febrile states treats fever and reduces the negative effects of hyperthermia on injured neurons. Patients are warmed in the operating room to prevent inadvertent perioperative hypothermia, which is known to cause increased blood loss, wound infections, and myocardial injury, while also prolong

Surgical outcomes in the elderly patient after osteocutaneous free flap transfer.

OBJECTIVES/HYPOTHESIS: Morbidity of free tissue transfer in the elderly patient is controversial. Recent studies have shown no significant difference in elderly fasciocutaneous free-flap donor site morbidity. The purpose of this study is to assess surgical outcomes in elderly patients receiving osteocutaneous free-tissue transfer. STUDY DESIGN: Retrospective chart review of patients 70 years and older undergoing osteocutaneous free flaps from 2000 to 2013. METHODS: Fibular, radial forearm, and scapular flaps were reviewed. Younger patients randomly selected from the same time period served a

Ophthalmic digital image transfer: benefits to triage, patient care and resource.

PURPOSE: Hospital capacity in the UK is currently significantly challenged due to new treatments, targets and resource limitations. There have been significant improvements in training, equipment and shared care services in community primary care optometry services. Despite this the challenges to ophthalmic service delivery are considerable. One area of potential benefit is the effect on outcome when a clinical image is attached to a referral. We aimed to quantify the effect of attaching digital images to ophthalmic referrals. METHODS: Retrospective analysis of 358 consecutive optometry refer

Lateral hamstring transfer and gait improvement in the cerebral palsy patient.

A retrospective analysis of twenty-three spastic patients who underwent forty-three transfers of the semitendinosus muscle to the lateral intramuscular septum and of the semimembranosus muscle to the biceps is presented. Decreased knee-flexion deformity as well as improved walking function were achieved in 91 per cent. An unsatisfactory result was associated with complications of the procedure. Only one knee of the forty-three that were operated on showed late genu recurvatum. This procedure appears to be both effective and relatively free of late comlications.

Patient Experience of Withdrawal During Methadone-Buprenorphine Transfer: A Prospective Cohort Study.

INTRODUCTION: Methadone-to-buprenorphine transfers (MBT) are increasingly common as buprenorphine use expands. Evidence describing patient experience of withdrawal during MBT in real-world populations is limited. This study aimed to characterise withdrawal trajectories during MBT and examine whether psychosocial and clinical factors influence the severity or timing of withdrawal. METHODS: This analysis used data from a prospective, non-randomised clinical trial comparing low-dose and stop-and-start MBT techniques (n = 117). Covariates included methadone dose, duration on methadone, transfer s

[Fecal microbiota transfer (FMT) in a patient with refractory irritable bowel syndrome].

Irritable bowel syndrome is a disorder of the gastrointestinal tract with unknown etiology. Recent clinical data support a link between changes in fecal microbiota with decreased biodiversity and the development of irritable bowel syndrome. Whether these changes of the microbiota are caused by the disease or whether they develop during the course of the disease remains unclear. Several studies demonstrated that fecal microbiota transfer (FMT) successfully attenuates Clostridium difficile infection by restoring the disturbed bacterial flora of the gut and case reports suggest that FMT may relie

Hospital networks and the dispersal of hospital-acquired pathogens by patient transfer.

Hospital-acquired infections (HAI) are often seen as preventable incidents that result from unsafe practices or poor hospital hygiene. This however ignores the fact that transmissibility is not only a property of the causative organisms but also of the hosts who can translocate bacteria when moving between hospitals. In an epidemiological sense, hospitals become connected through the patients they share. We here postulate that the degree of hospital connectedness crucially influences the rates of infections caused by hospital-acquired bacteria. To test this hypothesis, we mapped the movement o

Patient attitudes towards twin pregnancies and single embryo transfer - a questionnaire study.

BACKGROUND: The present trend towards selective single embryo transfer (SET) calls for evaluation of patient attitudes towards twins and how the patients balance advantages and disadvantages of one or two embryos in IVF/ICSI treatment. METHODS: The study was conducted in a Danish public fertility clinic, where the common practice was double embryo transfer (DET), and the number of reimbursed treatments was limited to three. Referred patients were given oral and written information about the IVF/ICSI treatment including twin probability following DET and the risk of preterm delivery and neonat

[The Impact of Patient Transfer After Rupture of an Abdominal Aortic Aneurysm].

OBJECTIVE: determine whether patient transfer adversely affects the survival of ruptured abdominal aortic aneurysm (rAAA) patients after conventional surgery. METHODS: We performed a retrospective review of all patients undergoing attempted repair of an rAAA at a tertiary center, over January 2008 and December 2014. Patients were divided into those presenting directly to our center and those transferred from another hospital. The main outcome variable was 24-hour or 30-day mortality, with secondary variables including time to surgical treatment, length of intensive care unit stay and total le

Toe-to-Hand Transfer in an Ulnar Hemimelia Patient: A Case Report.

Ulnar hemimelia is a rare congenital deformity. The severity of the deformity increases with the number of fingers that are absent. Clearly, grip function is impossible with a mono-finger hand. Here, we present a case report of a patient with bilateral ulnar hemimelia that included the absence of radial fingers and also deformity in the toes of his left foot. A toe-to-hand transfer was performed successfully to help the patient gain grip function. Both the patient and the parents were very pleased with the result. To our knowledge, this is the first case report of a patient with ulnar hemimeli

Extensor tendon dislocation after end-to-side transfer in a rheumatoid patient.

We report a case of extensor tendon dislocation at the metacarpophalangeal joint of the middle finger in which it appeared that an ulnar translation force created by an end-to-side transfer contributed to the dislocation. We recommend alterations in technique or alternative method of extensor tendon reconstruction to avoid this complication if unfavorable factors for end-to-side transfer exist.

Free tissue transfer in the hypercoagulable patient: a review of 58 flaps.

BACKGROUND: Hypercoagulability or thrombophilia is a group of inherited or acquired conditions associated with a predisposition to thrombosis. Most hypercoagulable states alter the blood itself or affect the vasculature, directly creating a detrimental environment for microsurgery. The authors present their series of hypercoagulable patients who underwent free flap reconstruction. METHODS: A retrospective review was conducted of all free flaps performed between January 1, 2005, and October 1, 2010, at the University of Pennsylvania. A total of 2032 flaps were performed. Forty-one patients or

[The hospital transfer of a patient with gastrointestinal bleeding from oesophageal varices].

The care of patients with gastrointestinal bleeding from oesophageal varices during inter-hospital transfer is a complex procedure to different causes. Some of these include, the high number of parameters to take into account, the monitoring of the electro-medical equipment, the instability of the patient and their needs or the short time available to know the patient and obtain the maximum information in order to plan a safe and proper transfer. The main role of the nurse in the transfer of critically ill patients focuses on the control and monitoring of all these aspects and the maintenance

Transfer factor in mycosis fungoides: a case report on a patient "cured".

A report is given of the successful treatment with transfer factor as an additional therapeutic agent of a patient in the tumour stage of mycosis fungoides who had not responded to conventional treatment with nitrogen mustard, methotrexate, or grenz rays alone. Besides twelve injections of transfer factor, the patient received X-irradiation, 1 400 r (50 kV 1 mm Al) in all, to three tumours. The combined treatment induced sustained remissions. When that patient died from a coronary infarct 5 years after treatment began, autopsy showed no evidence of mycosis fungoides at all.

[Transfer of patients with acute coronary syndromes].

BACKGROUND: The creation of a network to treat patients with acute coronary syndrome with revascularization devices presents a series of critical issues, one of which is the transfer of this type of patients from a spoke to a hub hospital with angioplasty facilities. We investigated the means of transport and the adverse events that could occur during such transfers. METHODS: We analyzed data relating to patients moved from our spoke hospital to hubs over a period of 2 years, utilizing medical records and transport reports. RESULTS: During this period, 531 patients aged 30-91 years were move

Free tissue transfer in pediatric patients.

The majority of reports on free tissue transfer involve adults; few have specifically addressed children. During the past 5 years, 20 free tissue transfers were performed in 19 pediatric patients. Patients ranged in age from 3 to 17 years, with a mean age of 10 years. Eight patients were 6 years and younger; 11 patients were 13 to 17 years old. Soft-tissue defects requiring reconstruction resulted from acute trauma in 12 patients, tumor ablation in 5 patients, infection in 1 patient, and hemifacial atrophy in 1 patient. Soft-tissue defects occurred in the lower extremity in 16 patients, the he
