BACKGROUND: Pelvic fracture surgery is a highly complex and skill-dependent procedure due to adjacent neurovascular structures. These challenges often result in prolonged operative time, elevated complication rates, and repeated reduction attempts. To support surgeons in overcoming these difficulties, our study introduces a rapid modeling approach that enables precise preoperative planning and quantitative evaluation of reduction strategies.
METHODS: We developed an automated patient-specific modeling method that integrates Statistical Shape Models with the personalized modeling modules of th

Several new techniques to repair the mitral valve affected by functional mitral regurgitation are in development. However, due to the heterogeneity of valve lesions between patients, predicting the outcomes of novel treatment approaches is challenging. We present a patient-specific, 3D ultrasound-derived computational model of the mitral valve for procedure planning, that faithfully mimics the pathological valve dynamics. 3D ultrasound images were obtained in three pigs induced with heart failure and which developed functional mitral regurgitation. For each case, images were segmented, and fin

Institution of extracorporeal membrane oxygenation (ECMO) results in unique blood flow characteristics to the end-organ vascular beds. We studied the interplay between cardiac-driven and extracorporeal membrane oxygenation (ECMO)-driven flow to vascular beds in different ECMO configurations using a patient-specific computational fluid dynamics (CFD) analysis. A computational ECMO model (femoral artery cannulation [FAC]) was constructed using patient-specific imaging and hemodynamic data. Following model calibration, we augmented the 3D geometrical model to represent alternative ECMO configurat

Congenital cataracts are the leading cause of childhood blindness. To date, surgical removal of cataracts is the only established treatment, but surgery is associated with multiple complications, which often lead to visual impairment. Therefore, mechanistic studies and drug-candidate screening have been intrigued by the aims of developing novel therapeutic strategies. However, these studies have been hampered by a lack of an appropriate human-disease model of congenital cataracts. Herein, we report the establishment of a human congenital cataract in vitro model through differentiation of patie

Computational fluid dynamics (CFD) is a modeling technique that enables calculation of the behavior of fluid flows in complex geometries. In cardiovascular medicine, CFD methods are being used to calculate patient-specific hemodynamics for a variety of applications, such as disease research, noninvasive diagnostics, medical device evaluation, and surgical planning. This paper provides a concise overview of the methods to perform patient-specific computational analyses using clinical data, followed by a case study where CFD-supported surgical planning is presented in a patient with Fontan circu

Providing optimal personalized mechanical ventilation for patients with acute or chronic respiratory failure is still a challenge within a clinical setting for each case anew. In this article, we integrate electrical impedance tomography (EIT) monitoring into a powerful patient-specific computational lung model to create an approach for personalizing protective ventilatory treatment. The underlying computational lung model is based on a single computed tomography scan and able to predict global airflow quantities, as well as local tissue aeration and strains for any ventilation maneuver. For v

Additively manufactured, custom-made implants used for reconstruction are a promising treatment following tumor resection. However, failure rates due to mechanical factors remain high when used in the pelvis for even state-of-the-art prosthesis designs. In a collaborative effort between a clinical and an engineering research team, this study evaluated whether patient-specific biomechanical modeling could predict, in a blinded fashion, the mode and location of a clinically-observed mechanical failure. Multiple failure criteria were considered including the risk of bone fracture due to overloadi

The anomalous aortic origin of coronary arteries (AAOCA) is a congenital disease that can lead to sudden cardiac death (SCD) during strenuous physical activity. Despite AAOCA being the second leading cause of SCD among young athletes, the mechanism behind sudden cardiac death remains mostly unknown. Computational fluid dynamics provides a powerful tool for studying how pathologic anatomy can affect different hemodynamic states. The present study investigates the effect of AAOCA on patient hemodynamics. We performed patient-specific hemodynamic simulations of interarterial AAOCA at baseline and

Collapse of the soft palate in the upper airway contributes to obstructive sleeping apnea (OSA). In this study, we investigate the influence of the adhesion from the tongue on the soft palate global response. This is achieved using a cohesive zone finite element approach. A traction-separation law is determined to describe the adhesion effect from the surface tension of the lining liquid between the soft palate and the tongue. According to pull-off experimental tests of human lining liquid from the oral surface of the soft palate, the corresponding cohesive properties, including the critical n

OBJECTIVES: Cardiac output (CO) response to dobutamine can identify Alagille's syndrome (ALGS) patients at higher risk of cardiovascular complications during liver transplantation. We propose a novel patient-specific computational methodology to estimate the coronary autoregulatory responses during different hemodynamic conditions, including those experienced in a post-reperfusion syndrome (PRS), to aid cardiac risk-assessment.
MATERIAL AND METHODS: Data (pressure, flow, strain and ventricular volumes) from a 6-year-old ALGS patient undergoing catheter/dobutamine stress MRI (DSMRI) were used

Ogden type of hyperelastic constitutive law has recently emerged in modeling ventricular enlargement in hydrocephalic brain with finite element method, but this material property for brain tissue has not been investigated in a patient-specific setting in hydrocephalus. Consequently, the accuracy of the simulated ventricular enlargement using this hyperelastic tissue property remains unknown. In this study, we evaluated this brain material model in four patients with communicating hydrocephalus under a small trans-mantle pressure difference (TPMD) between brain ventricle and subarachnoid space

Cerebral venous sinus thrombosis (CVST) and carotid web (CW) are distinct cerebrovascular disorders where vascular geometry, disturbed hemodynamics, and endothelial activation interact to drive thrombosis. Both conditions disproportionately affect women, yet remain understudied because of the inability of traditional platforms to replicate complex three-dimensional lumen profiles. This perspective positions light-based lithography as a powerful biofabrication strategy for patient-specific modeling. Digital light processing and volumetric bioprinting render enclosed, perfusable hydrogel network

STUDY DESIGN: Finite element (FE) analysis of retrospective clinical cohort.
OBJECTIVE: To determine whether preoperative CT-derived FE model outputs can improve subsidence prediction compared with conventional clinical measurements alone in patients undergoing transforaminal lumbar interbody fusion (TLIF).
SUMMARY OF BACKGROUND DATA: Cage subsidence occurs in ∼20% of spinal fusion patients and can lead to complications requiring reoperation. While individual risk factors are known, no validated tool integrates patient anatomy, bone quality, and implant characteristics to predict subsidence.

Spinal metastases are a major cause of pain, vertebral fracture, instability, and neurological compromise. The Spinal Instability Neoplastic Score (SINS) has substantially improved the standardized assessment of neoplastic spinal instability and remains the reference clinical tool for evaluating the mechanical component of metastatic spinal disease. However, it remains a semi-quantitative score, does not directly quantify vertebral strength, and is particularly limited in the intermediate SINS range (7-12), where uncertainty regarding true mechanical risk and therapeutic strategy is often grea

Robin Sequence (RS) is a congenital condition in which patients experience dynamic, periodic obstruction or collapse of the upper airway due to an underdeveloped jaw and a posteriorly displaced tongue. Current clinical techniques for evaluating airway obstruction do not provide quantifiable data and fail to account for the dynamic nature of obstruction or collapse. There is no standardized criterion to characterize obstruction or collapse severity. This study presents the first method that extracts airway motion from 4-dimensional computed tomography and performs a patient-specific, moving-mes

PURPOSE: The entire course of radiation therapy (RT) for high-grade glioma (HGG) is currently derived from pre-RT magnetic resonance imaging (MRI). Although it is possible to adapt RT during the course of treatment, it is often guided only by anatomical changes to the tumor. This study seeks to determine if a biology-based mathematical model, parameterized by patient-specific, multiparametric MRI (mpMRI) data, can accurately forecast HGG response during RT.
METHODS AND MATERIALS: Twenty one patients with HGG planned for 6 weeks of concurrent RT and chemotherapy were imaged weekly with mpMRI d

BACKGROUND: Somatic cells differentiated from patient-specific human induced pluripotent stem cells (iPSCs) could be a useful tool in human cell-based disease research. Hermansky-Pudlak syndrome (HPS) is an autosomal recessive genetic disorder characterized by oculocutaneous albinism and a platelet dysfunction. HPS patients often suffer from lethal HPS associated interstitial pneumonia (HPSIP). Lung transplantation has been the only treatment for HPSIP. Lysosome-related organelles are impaired in HPS, thereby disrupting alveolar type 2 (AT2) cells with lamellar bodies. HPSIP lungs are characte
