Chronic pelvic floor dysfunction.

The successful treatment of women with vestibulodynia and its associated chronic pelvic floor dysfunctions requires interventions that address a broad field of possible pain contributors. Pelvic floor muscle hypertonicity was implicated in the mid-1990s as a trigger of major chronic vulvar pain. Painful bladder syndrome, irritable bowel syndrome, fibromyalgia, and temporomandibular jaw disorder are known common comorbidities that can cause a host of associated muscular, visceral, bony, and fascial dysfunctions. It appears that normalizing all of those disorders plays a pivotal role in reducing

[Pelvic floor and pregnancy].

Congenital factor, obesity, aging, pregnancy and childbirth are the main risk factors for female pelvic floor disorders (urinary incontinence, anal incontinence, pelvic organ prolapse, dyspareunia). Vaginal delivery may cause injury to the pudendal nerve, the anal sphincter, or the anal sphincter. However the link between these injuries and pelvic floor symptoms is not always determined and we still ignore what might be the ways of prevention. Of the many obstetrical methods proposed to prevent postpartum symptoms, episiotomy, delivery in vertical position, delayed pushing, perineal massage, w

[Overactive pelvic floor syndrome].

BACKGROUND: The etiology of the overactive pelvic floor syndrome is not fully understood and no gold standards are available for diagnosis or treatment. The article presents an overview of literature, and discusses diagnostics and treatment. MATERIAL AND METHODS: Literature was identified through a non-systematic search in PubMed, and discussed in light of the authors' clinical experience with the patient group. RESULTS: The main symptoms of overactive pelvic floor syndrome are pain and defecation difficulties; the latter often leads to chronic constipation. Other symptoms depend on which pa

Laparoscopic pelvic floor repair.

Articles on laparoscopic approach to pelvic floor reconstruction continue to proliferate throughout the worldwide literature. Although procedures like laparoscopic Burch seem to be fading fast, other procedures like the laparoscopic paravaginal repair and sacral colpopexy seem to be more common and visible in the literature. This article reviews the pertinent anatomy, surgical procedures, and literature concerning the laparoscopic approach to pelvic floor reconstruction.

Pelvic floor muscle exercises.

The ICS Standardisation Committee (Abrams et al, 2002) defines the symptom of stress urinary incontinence as the complaint of involuntary leakage on effort or exertion, or on sneezing or coughing. Genuine stress incontinence (now known as urodynamically proven stress incontinence) is the involuntary loss of urine occurring when, in the absence of a detrusor contraction, the intravesical pressure exceeds the maximal urethral pressure. These definitions tend to look at the urinary mechanism in isolation, and many clinicians now advocate that a more holistic view should be taken of this problem.

Physiotherapy for the pelvic floor.

The inefficiencies of the physiotherapy profession in its attitude to the treatment of pelvic floor insufficiency and urinary stress incontinence are discussed, together with a description of the different types of urinary stress incontinence, their causes and a brief outline of the anatomy of the pelvic floor. The total concept of the authors' treatment regime is described in detail-exercises done per vaginam by the physiotherapists as well as the patient, and the reasons for the choice and application of interferential therapy as an adjunct to these exercises. The authors consider that only

Pelvic Floor Therapy and Initial Interventions for Pelvic Floor Dysfunction in Gynecologic Malignancies.

PURPOSE OF REVIEW: This review provides evidence-based updates for the first-line management approaches for pelvic floor disorders in patients with gynecologic malignancies, as well as important provider considerations when referring for pelvic floor physical therapy. RECENT FINDINGS: Currently, there is strong evidence to recommend pelvic floor muscle training as initial treatment for urinary incontinence and for pelvic organ prolapse; some evidence to recommend a more targeted pelvic floor muscle training program for fecal incontinence; and mostly expertise-based evidence to recommend vagin

Pelvic floor muscle function in the standing position in women with pelvic floor dysfunction.

INTRODUCTION AND HYPOTHESIS: Clinical assessment of the pelvic floor muscles (PFMs) in a standing position may provide a more valid representation of PFM function experienced by patients in daily life than assessment in the lying position. The primary aim of this study was to examine PFM function in a standing versus a lying position in parous women with any type of urinary incontinence and/or pelvic organ prolapse. METHODS: In this exploratory cross-sectional study, participant symptom status was determined using the Australian Pelvic Floor Questionnaire. Pelvic floor muscle function was ass

Rehabilitation of the short pelvic floor. II: Treatment of the patient with the short pelvic floor.

Several urogynecologic syndromes are associated with the clinical finding of a short, painful, tender and weak pelvic floor and a variety of connective tissue abnormalities. Techniques for rehabilitation include the avoidance of perpetuating factors, rehabilitation of extrapelvic musculoskeletal abnormalities, the use of manual techniques and needling to promote resolution of connective tissue problems, closure of any diastasis recti, and transvaginal/transrectal manual release of muscular trigger points and contractures. Therapy can be facilitated by pudendal or epidural nerve block. Patients

Biomechanics of pathological pelvic floor: Finite element comparative analysis of the prolapsed pelvic floor.

BACKGROUND: To establish a 2D biomechanical finite element model of a pathological pelvic floor and explore mechanisms driving pelvic organ prolapse (POP) progression from mild to severe stages based on a Stage I POP-Q patient. METHODS: We developed a two-dimensional biomechanical finite element model based on the clinical presentation of a patient with POP-Q stage I prolapse at rest. The biomechanical interactions between the morphological characteristics and mechanical support were investigated by considering the effects of genital hiatus, intra-abdominal pressure, and combined injuries. Multimodal Pelvic Floor Ultrasound Parameters Associated with Postpartum Pelvic Floor Dysfunction: A Retrospective Study.

Research on multimodal ultrasound parameters associated with postpartum pelvic floor dysfunction (PFD) across resting, maximal contraction, and Valsalva states remains limited. This retrospective study assessed these parameters in 172 patients examined between January 2022 and June 2025. After stratification into PFD and non-PFD groups, 1:1 propensity score matching (PSM) yielded 160 patients (80 per group) for analysis. Ultrasound parameters were evaluated across the three functional states. Logistic regression identified independent factors associated with PFD, and receiver operating charact

[Assessing knowledge of pelvic floor disorder and pelvic floor muscle training among young women].

Introduction: Although pelvic floor disorders affect millions of women worldwide, healthcare seeking is low. Adequate knowledge regarding pelvic floor disorders could lead to increased healthcare-seeking behavior, which may help to reduce the economic burden of these disorders. Objective: The present study aims to evaluate knowledge about pelvic floor disorders and pelvic floor muscle training and to assess healthcare seeking behavior among young women. Method: 221 young women (mean age: 20.65 ± 1.67 years) were included in the present cross-sectional study. The Prolapse and Incontinence Knowl

Pelvic floor muscle function, pelvic floor dysfunction and diastasis recti abdominis: Prospective cohort study.

AIMS: Compare vaginal resting pressure (VRP), pelvic floor muscle (PFM) strength, and endurance in women with and without diastasis recti abdominis at gestational week 21 and at 6 weeks, 6 months, and 12 months postpartum. Furthermore, to compare prevalence of urinary incontinence (UI) and pelvic organ prolapse (POP) in the two groups at the same assessment points. METHODS: This is a prospective cohort study following 300 nulliparous pregnant women giving birth at a public university hospital. VRP, PFM strength, and endurance were measured with vaginal manometry. ICIQ-UI-SF questionnaire and

Factors affecting reflex pelvic floor muscle contraction patterns in women with pelvic floor disorders.

OBJECTIVE: To explore factors affecting the presence of two reflex pelvic floor muscle contraction (PFMC) patterns in women with pelvic floor disorders. METHODS: This was a retrospective analysis of pelvic floor ultrasonography and urodynamic data for 667 consecutive symptomatic women with pelvic floor disorders. We identified on ultrasonography the presence or absence of two reflex PFMC patterns, anorectal lift (ARL) and inward clitoral motion (ICM), preceding or occurring during coughing, and evaluated their associations with possible factors affecting reflex PFMC reactivity, including pati

Pelvic floor spasms in children: an unknown condition responding well to pelvic floor therapy.

OBJECTIVE: During a study period of 4 years, 21 children are seen for night time pelvic pain. These children typical wake up in the middle of the night with severe lower abdominal or perineal pain. During day some of them suffer urge syndrome. During urodynamic investigation extremely high pelvic floor activity as recorded by high urethral pressure was observed in these children. We therefore started pelvic floor relaxation biofeedback in these children. METHODS: All children diagnosed with pelvic floor spasms underwent biofeedback pelvic floor relaxation therapy in order to learn them to cou

Management of the pelvic floor after pelvic exenteration.

At the Mayo Clinic, 198 exenterative operations were performed from 1950 through 1971. Although the 5-year survival rate of all patients was 33%, bowel obstruction occurred in 11.6% of patients and intestinal fistula formation in 12.6%. The majority of patients with these complications had had previous pelvic irradiation. The difficulty in dealing with the large raw area in the pelvis is discussed with a review of the various methods that have been tried to date to reperitonealize the pelvis and thereby, it is hoped, prevent adhesions or fistula formation.
