Serum pepsinogen in screening for gastric cancer.

To establish a sensitive and efficient screening method for gastric cancer using serum pepsinogen, we investigated the characteristics of serum pepsinogen I and II levels and the I/II ratio and their cut-off points. We found that the pepsinogen I level and the I/II ratio were significantly lower in patients with gastric cancer than in control subjects, especially in patients with cancers of the differentiated type, the elevated type, and the depressed type without ulceration. However, sex, depth of invasion, and location of tumor did not correlate with the pepsinogen levels. A suitable cut-off

Immunohistochemical studies on the development of cells containing progastricsin (minor pepsinogen) in comparison to prochymosin and pepsinogen in bovine abomasal mucosa.

The localization of progastricsin was studied in cells of abomasal mucosa from cattle of different ages and feeding regimes and compared to the localization of prochymosin and pepsinogen in the same material by use of an immunofluorescence technique with specific rabbit antibodies. Immunoreactivity for progastricsin was first found in calves at the age of about 45 days in surface mucous cells in the pit of the fundic gland. In older calves and adults, mucous neck cells also produced progastricsin. In the pyloric mucosa, on the other hand, traces of progastricsin immunoreactivity were found in

The Controversy of Pepsinogen A/Pepsin A in Detecting Extra-Gastroesophageal Reflux.

BACKGROUND: Pepsinogen A (PGA)/pepsin A is often used as a diagnostic marker of extra-gastroesophageal reflux. We aimed to explore whether its positivity in upper aerodigestive tract (UADT) was specific enough to diagnose reflux. METHODS: PGA/pepsin A protein levels were examined in 10 types of tissues and 10 types of body fluid by immunological staining, western blot or Elisa, using three different commercially available brands simultaneously. Liquid chromatography-tandem mass spectrometry parallel reaction monitoring (LC-MS/MS PRM) served as a gold reference for the detection of PGA/pepsin

The Serum Pepsinogen Test as a Predictor of Kazakh Gastric Cancer.

Gastric cancer (GC) is one of the most common malignant tumors and the Kazakh population in Xinjiang has been reported to be one of the highest incidence of GC in the world. Serum pepsinogen (PG) test provides a valuable method for detecting GC, but little study about the role of PG in Kazakh GC. Therefore, we aimed to investigate the role of PG in Kazakh GC and to elucidate the usefulness of the serum PG test method. The serum PG concentration were measured using the flow fluorescence assay and ELISA methods in patients with superficial gastritis, atrophic gastritis and GC. The most suitable

Duodenal ulcer is a multifactorial disorder--the role of pepsinogen I.

Serum pepsinogen I (PGI) levels were measured in 231 duodenal ulcer (DU) patients and 100 sex- and age-comparable healthy controls. Significantly higher mean serum PGI levels were found in DU patients than in controls (124.7 +/- 3.4 ng/ml v. 92.9 +/- 2.3 ng/ml; P < 0.001) (mean +/- SE). These levels were higher in male DU patients than in female DU patients (128.5 +/- 3.9 ng/ml v. 107.4 +/- 6.4 ng/ml; P < 0.05). Smoking was associated with elevated serum PGI levels in DU patients (145.3 +/- 5.1 ng/ml v. 109.0 +/- 4.2 ng/ml; P < 0.001). Healed DUs were associated with lower mean serum PGI level

Pepsinogen I, pepsinogen II, gastrin-17, and Helicobacter pylori serological biomarkers in the diagnosis of precursor lesions of gastric cancer.

INTRODUCTION: Atrophic gastritis and intestinal metaplasia are precursor lesions of gastric cancer. The aim of this study was to determine the usefulness of the biomarkers pepsinogen I(PgI), pepsinogen II (PgII), gastrin-17, and H. pylori antibodies in the identification of precursor lesions. METHODS: We studied 129 patients with gastric symptoms. The biomarker status was determined using GastroPanel by means of the ELISA-technique. RESULTS: Biomarkers detected atrophy in 14% of the subjects, and 49.6% had positive antibodies for H. pylori. A PgI/PgII ratio < 3 was an important risk biomarke

Plasma ghrelin concentration correlates with the levels of serum pepsinogen I and pepsinogen I/II ratio--a possible novel and non-invasive marker for gastric atrophy.

BACKGROUND/AIMS: Ghrelin, a novel growth-hormone-releasing peptide, has been reported to be localized mainly in the A-like cells in the gastric fundic mucosa. With the extension of gastric inflammation caused by H. pylori infection, gastric mucosal atrophy extends from the antrum to the corpus, which is the predominant site of localization of the ghrelin-producing A-like cells. The present study was designed to investigate the correlation between the plasma ghrelin levels and the extent of gastric mucosal atrophy in patients with chronic gastritis caused by H. pylori infection. Using serum pepsinogens wisely in a clinical practice.

Serum pepsinogen (PG) has been used as biomarkers of gastric inflammation and mucosal status, including atrophic change, before the discovery of Helicobacter pylori (H. pylori). Serum pepsinogen I (PG I) and pepsinogen II (PG II) levels are known to increase in the presence of H. pylori-related nonatrophic chronic gastritis. The measurement of serum PG provides much information on the presence of intestinal metaplasia as well as atrophic gastritis. The eradication of H. pylori provokes a significant change in serum PG values: it reduces both PG I and PG II and elevates the PG I to PG II ratio.

[Serum pepsinogen I/II ratio test].

Serum pepsinogen levels were measured in 137 stomach cancer patients and compared with those of 288 normal cancer-free subjects. The serum pepsinogen levels of stomach cancer patients, especially pepsinogen I and the pepsinogen I/pepsinogen II ratio were significantly lower than those of normal controls and correlated well with the extent of chronic gastritis associated with the cancerous stomach. These results were in good accordance with the results of previous studies indicating that the cancer derived from the stomach where chronic gastritis/intestinal metaplasia is extensive. The initial

The pepsinogen releasing effect of Helicobacter pylori lipopolysaccharide.

BACKGROUND: Helicobacter pylori lipopolysaccharide (LPS) affects pepsinogen release by a nontoxic mechanism. We hypothesized that this effect was characteristic of the organism and related to the clinical status of the strain. MATERIALS AND METHODS: LPS was isolated from 11 H. pylori strains whose pathogenic profile was known and four other nongastric bacteria. The effects of luminal LPS on guinea pig gastric mucosal pepsinogen release was evaluated using the Ussing chamber technique. CCK-8 (10(-9)M) was used as a positive control. RESULTS: H. pylori LPS dose-dependently stimulated pepsinoge

Serum pepsinogen I elevation in cigarette smokers.

We measured serum pepsinogen I (sPG-I) in 269 patients undergoing upper GI endoscopy and then classified by endoscopic diagnosis, gastric mucosal histology, and smoking habit. Both ulcer-free and duodenal ulcer smokers had significantly higher sPG-I levels than their non-smoking controls. In contrast, sPG-I values were not different in smokers and non-smokers with gastric ulcer. In ulcer-free smokers the overall increase in sPG-I simply reflected the high prevalence of patients with superficial gastritis and elevated sPG-I levels. Conversely, in duodenal ulcer smokers the increase in sPG-I, wh

Studies on the pepsinogens of human gastric mucosal extracts.

1. Electrophoretic separation of proteases from human gastric mucosal extracts of five patients with gastric ulcer, one with duodenal ulcer and three with gastric cancer were investigated by agar-gel electrophoresis at pH 8.3 and pH 5.0. 2. In the fundic mucosal study, there were seven faster moving proteases in all nine cases, but the slowest moving protease showed a slightly different picture in each case. In the antral mucosal study, two of eight cases showed mainly group II pepsinogens, seven of nine cases, however, showed the same results as in the fundic mucosal study. 3. In the cases of

Serum pepsinogen A and pepsin secretion in duodenal ulcer: Helicobacter pylori influence.

Serum pepsinogen A (PGA) and total pepsin activity (PA) in gastric juice were investigated in 87 duodenal ulcer patients. The analysis of data shows a significant increase of PGA and PA in comparison with controls. The grouping of duodenal ulcer patients according to Helicobacter Pylori (HP) infection revealed in HP positive patients a significant increase of PGA, while PA does not change. This contrast may be due to various factors: in particular the two parameters are an expression of different cellular activities and the PA in gastric juice is expression of all pepsinogens. It can be conclu

Immunohistochemical localization of pepsinogen A and C containing cells in Barrett's oesophagus.

No data are available on the localization of Pepsinogen A (PGA = PG I) and Pepsinogen C (PGC = PG II) positive cells in Barrett's epithelium. Endoscopic biopsy specimens were taken from the columnar epithelium from 23 patients (n = 93), and in addition from the cardia from eight healthy control subjects (n = 38). The tissue was stained by the immunoperoxidase technique with specific anti-pepsinogen antisera, and double immunostained for PGA and PGC. In the Barrett's epithelium PGA was found in 28 out of 93 biopsy specimens (30.1%) and PGC in 55 out of 93 (59.1%). Chief cells always stained bot

Pepsinogen II in gastritis and Helicobacter pylori infection.

BACKGROUND AND AIM: In the gastric mucosa, pepsinogen II (PgII) is produced/secreted by glands in the mucus-secreting antral and cardia compartments, but also by the chief cells and the oxyntic glands. Increasing PgII serum levels are associated with the whole spectrum of gastric inflammatory diseases, including gastritis induced by Helicobacter pylori (H. pylori). This review critically addresses the clinical value of PgII serology for assessing gastric mucosal inflammation, and as a marker of H. pylori status, in both H. pylori-positive patients and after eradication therapy. RESULTS: A sea
