Management of Percutaneous Coronary Intervention Complications.

With the recent increase in complex coronary interventions including percutaneous coronary intervention (PCI) for chronic total occlusions and complex higher risk (and indicated) patients, the spectrum of potential periprocedural complications and their prompt management has become even more relevant. Vascular access-related problems remain the most prevalent of all PCI complications and with randomized controlled trial data from over 20,000 patients supporting the superiority of radial over femoral access in reducing bleeding and vascular complications, a default radial strategy should be pro

Antiplatelet Therapy in Percutaneous Coronary Intervention.

Platelets play a key role in mediating stent thrombosis, which is the major cause of ischemic events immediately after percutaneous coronary intervention (PCI). Antiplatelet therapy is therefore the cornerstone of antithrombotic therapy after PCI. However, the use of antiplatelet agents increases bleeding risk, with more potent antiplatelet agents further increasing bleeding risk. In the past 5 years, potent and fast-acting P2Y12 inhibitors have augmented the antiplatelet armamentarium available to interventional cardiologists. This article reviews the preclinical and clinical data surrounding

Percutaneous coronary intervention in the elderly.

Our population dynamics are changing. The number of octogenarians and older people in the general population is increasing and therefore the number of older patients presenting with acute coronary syndrome or stable angina is increasing. This group has a larger burden of coronary disease and also a greater number of concomitant comorbidities when compared to younger patients. Many of the studies assessing percutaneous coronary intervention (PCI) to date have actively excluded octogenarians. However, a number of studies, both retrospective and prospective, are now being undertaken to reflect th

Antianginal therapy before percutaneous coronary intervention.

BACKGROUND: The regional variability of percutaneous coronary intervention (PCI) rates may be explained by variations in the medical treatment of stable coronary artery disease. We sought to determine whether greater regional use of antianginal medications in PCI patients is associated with lower regional rates of PCI. METHODS AND RESULTS: Using CathPCI Registry and Dartmouth Atlas data, we examined patients undergoing elective PCI for stable coronary artery disease from January 1, 2009, through March 31, 2011, and calculated rates of providing ≥ 2 antianginal medicines before PCI. We regress

Gender differences following percutaneous coronary intervention.

PCI is effective for reducing symptoms in patients with stable angina pectoris but does not improve prognosis. In earlier trials PCI has been associated with more procedure related complications in women than men, but this difference between genders has been less pronounced in more recent studies. In acute coronary syndromes there is no evidence of gender differences regarding the benefit of primary PCI for ST-segment elevation myocardial infarction. However, several trials of unstable angina and non-ST-segment elevation myocardial infarction indicate that women do not have the similar benefit

Bleeding complications during percutaneous coronary intervention.

Despite improvements in techniques and patient management over the past decade, bleeding complications remain an important problem in the practice of interventional cardiology. One approach to minimize bleeding risk is through refinements in technique. Another important strategy involves selection of the antithrombotic regimen. While any discussion of safety must be set against the backdrop of efficacy, this article focuses especially on the bleeding complications associated with various agents, including heparin and other adjunctive therapies (e.g., glycoprotein IIb/IIIa inhibitors, aspirin,

Percutaneous transluminal angioscopy during coronary intervention.

To investigate the feasibility of angioscopic-guided percutaneous transluminal coronary angioplasty and to elucidate the mechanism of efficacy of coronary stenting for acute myocardial infarction, we performed coronary angioscopy in 102 patients with stable angina or acute myocardial infarction. Thrombi and intimal flaps were observed in most patients after coronary angioplasty. Large intimal splits were seen in one third of patients. Stents were inserted in 10 patients who were revealed to have a large flap or protruding split to the inner lumen. Thrombolytic agents were administered in 2 pat

Predilation in Primary Percutaneous Coronary Intervention.

BACKGROUND: In primary percutaneous coronary intervention (pPCI), balloon predilation is frequently carried out before stenting but there is a lack of data regarding optimal balloon size and the effect of balloon size on procedural and clinical outcomes. AIMS: This study compares small balloon predilation (≤50% of proximal vessel diameter) with large balloon predilation (>50% of proximal vessel diameter) in pPCI. METHODS: This multicentre prospective observational study included consecutive ST elevation MI (STEMI) patients undergoing pPCI at three tertiary centres in Egypt. Demographic, clin

Acute Pericarditis After Percutaneous Coronary Intervention.

Pericarditis may be a complication of percutaneous coronary intervention (PCI), and should be considered as a differential for persistent chest pain following a PCI.  In this report, we present a male in his 60s, who developed pericarditis after a primary PCI for an anterior ST elevation myocardial infarction (STEMI). He had an uncomplicated PCI to left anterior descending artery, and was discharged as per standard procedure 48 hours post-procedure. On that same evening, the patient re-presented to hospital with different chest pain and fever. Electrocardiogram (ECG) revealed anterior ST eleva

Percutaneous coronary intervention facilities in Nigeria.

BACKGROUND: In Nigeria, the incidence of coronary artery disease has doubled over the last three decades. However, there appears to be a lack of adequate heart catheterisation facilities. METHODS: A list of percutaneous coronary intervention (PCI)-capable facilities was compiled for each state in Nigeria and the federal capital territory. Population estimates for 2019 were obtained from the National Bureau of Statistics and this was utilised to calculate the number of PCI facilities per person in each state and the country. RESULTS: There are 12 operational PCI facilities in Nigeria, 11 of w

Percutaneous coronary intervention following Fontan procedure.

Fontan procedure is known to increase the risk of thromboembolic events. However, coronary artery thrombotic occlusion is rarely reported in patients with Fontan procedure. We present a case of a 10-year-old boy with hypoplastic left heart syndrome palliated with a Fontan procedure who presented with myocardial infarction secondary to thrombotic occlusion of the left circumflex coronary artery. He underwent successful percutaneous coronary intervention with thrombus aspiration, balloon angioplasty, and stent placement, highlighting the necessity of collaboration between congenital and adult ca

Percutaneous coronary intervention-associated Actinomyces oris.

Coronary artery interventions are safe procedures yet have a risk of stent infection, bacteremia and sepsis, events that are rare but with high morbidity and mortality sequel. A few prior cases had reported post percutaneous coronary intervention (PCI) infections, abscesses and sepsis due to Staphylococcus aureus, followed by Pseudomonas aeruginosa. Cardiac Actinomyces infections are extremely rare. Here we report a case of a 50 year old patient who developed a post intervention Actinomyces oris epicardial abscess occluding right coronary artery with subsequent bacteremia eventually requiring

Subepicardial hematoma after percutaneous coronary intervention.

BACKGROUND: Coronary artery perforation associated to cardiac tamponade is an uncommon life-threatening complication of percutaneous coronary intervention, and the occurrence of subepicardial or intramyocardial hematoma without hemopericardium is even rarer. CLINICAL CASE: We describe the case of a 72 year-old woman with a subepicardial hematoma after percutaneous coronary intervention, who required urgent left internal mammary artery graft to anterior descending artery surgery. CONCLUSIONS: Subepicardial or intramyocardial hematoma must be considered when a coronary perforation is suspected

Primary percutaneous coronary intervention in octogenarians.

BACKGROUND: Limited data are available on the outcome of primary percutaneous coronary intervention (PCI) in octogenarian patients, as the elderly are under-represented in randomized trials. This study aims to provide insights on clinical characteristics, management and outcome of the elderly and very elderly presenting with STEMI. METHODS: 2225 STEMI patients ≥70years old (mean age 76.8±5.1years and 53.8% men) were admitted into the network of the ISACS-TC registry. Of these patients, 72.8% were ≥70 to 79years old (elderly) and 27.2% were ≥80years old (very-elderly). The primary end-point wa

[Percutaneous coronary intervention and stent thrombosis].

Despite improved stent implantation technologies and effective antiplatelet regimens, stent thrombosis has been one of the most important problem in percutaneous coronary intervention. Stent thrombosis is a rare but usually catastrophic event, frequently associated with large MI or death. Stent thrombosis is generally categorized according to the timing of the event as early (0 to 30 days), late (31 days to 1 year), very late (> 1 year). Late and very late stent thrombosis after stent implantation has not yet been adequately characterized, mainly because of its low incidence. And the optimal d

Antiplatelet therapy after percutaneous coronary intervention.

There is intense interest in the relationship between inflammation, thrombosis, platelet aggregation, and hyperlipidemia in patients with coronary artery disease. The specific role of inflammation with its linkage to the coagulation cascade has been well studied. A number of inflammatory markers have been identified which can be used for risk stratification in patients with acute coronary syndromes. Patients with acute coronary syndromes at the time of presentation often have an underlying inflammatory state which needs therapy with antiplatelet regimens including now increasingly frequently c

Acute sialadenitis after percutaneous coronary intervention.

A 63 year old woman developed a swelling of the submandibular glands following PTCA. An ENT specialist made the presumptive diagnosis of viral sialadenitis. The submandibular swelling subsided completely within 48 hours. Sialadenitis is a rare side effect of ionic and nonionic radiocontrast agents. The pathogenic mechanism is not known. The swelling is self-limited with an excellent prognosis without specific therapy.

Bacteremia following complex percutaneous coronary intervention.

BACKGROUND AND OBJECTIVE: Complex percutaneous coronary intervention (PCI) often requires introduction of numerous devices into and out of the arterial circulation and this may result in an increased risk of bacteraemia or even septicaemia. This study was undertaken to detect the frequency of bacteraemia that may be associated with such procedures. METHODS: 147 patients undergoing complex PCI had blood culture tests immediately after and 12 hours after the procedure. RESULTS: Of 147 patients, 26 (17.7%) had detectable bacteraemia immediately after PCI. Coagulase-negative staphylococcus was i
