BACKGROUND: Although the reconstruction of palmar defects of the thumb with exposed vital structures is difficult because of a lack of locally available tissue, several local flaps are available for the reconstruction of these defects. The majority of these flaps are raised from the dorsal or dorsolateral aspect of the thumb and index finger or from the forearm.
METHODS: Between 1998 and 2008, 23 patients underwent reconstruction of palmar defects of the thumb at Chonnam National University Medical School, using the radial midpalmar (perforator-based) island flap (distal thenar perforator-bas

Dorsal skin defect of the hand/digit requires thin and pliable skin for coverage. The prepuce skin, the second thinnest skin, can be a reconstructive option, but there is no previous report of a free prepuce flap for the reconstruction of the hand/digit. We report the first successful case of a free prepuce flap based on the superficial penile artery perforator (SPAP) for the treatment of dorsal soft tissue defect of the hand/digit. A 68-year-old male presented with a 5 x 3 cm dorsal soft tissue defect of the left index finger with the extensor tendon exposure. A prepuce perforator flap was ra

INTRODUCTION: Perforator flaps as an innovative method for soft tissue transfer that maximizes function preservation, were originally introduced primarily as free flaps. Their reliability and versatility has been found to not differ from other sources of free flaps where total failure is an uncommon event. Partial failure should also be recognized as a possible dilemma that is perhaps more of a unique untoward sequela of perforator flaps.
METHOD: A retrospective review of our flap experience over the past decade included 310 perforator free flaps. Partial perforator flap failure that required

From January 2014 to June 2018, 12 patients with skin and soft tissue defects in shoulder and elbow were admitted to General Hospital of Ningxia Medical University with skin and soft tissue defect size of 6 cm×5 cm to 11 cm×8 cm, including 9 males and 3 females aged 15-56 years. Wounds of 5 patients were repaired with rotator scapular artery perforator flap (flap area of 8 cm×7 cm to 12 cm×6 cm), and the donor flap area was sutured directly. Wounds of 3 patients were repaired with lateral thoracic perforator flap (flap area of 8 cm×7 cm to 13 cm×9 cm). The donor flap area of two patients was s

This article reports a case of re-elevating a prior vertical rectus abdominis musculocutaneous flap as a deep inferior epigastric artery perforator-based propeller flap to cover a recurrent chest wall defect. This case demonstrates that a conventional musculocutaneous flap tissue with a preserved perforator can be recycled as a perforator flap. Furthermore, this technique can be a promising new surgical option for recurring abdominal and chest defects.

The supraclavicular artery perforator (SAP) flap is a versatile flap for the reconstruction of head and neck defects. Recently, the authors have modified the SAP flap by using an anterior branch of the transverse cervical artery. The anterior SAP flap allows the harvest of a tissue island in the deltopectoral fossa, which is even thinner, is more pliable, and shows a superior color match to the face and neck compared with the original SAP flap. Pre-expansion increases flap size considerably, enabling the coverage of extended defects without the need of microsurgery.

Few options exist for the resurfacing of web-space and small soft tissue defects of the dorsum of the distal foot. The study examines the anatomy of the second to fourth dorsal metatarsal arteries in 16 fresh frozen cadavers to determine if the anatomy correlates to that in the hand, permitting the design of local flaps based on perforators of these vessels. A clinical case is also presented, illustrating the efficacy of such a perforator-based flap.Sixteen Asian cadaveric lower limbs were used for this study. The specimens were prepared with latex dye injection. Dissection under loupe magnifi

BACKGROUND: In the lower leg posttraumatic pathology, we are frequently confronted with tissue defects that are difficult to cover by local or distance means, because of either the poor tissue quality or the precarious local vessels. One of the still available methods for solving these cases remains the cross-leg flap. The authors will try to demonstrate the advantages of this method, by using the cross-leg flap based on perforator vessels.
METHODS: We are presenting two cases for which we practiced a cross-leg perforator flap, based on perforator vessels emerging from the posterior tibial ar

The size of the thoracodorsal artery perforator (TDAP) flap or pedicle, in general, may be found to be inadequate. Pre-expansion of the flap before harvest can be a solution to increase the size of the TDAP flap in such instances. The pre-expanded TDAP flap can be used to reconstruct large-sized defects with the advantage of primary closure of the donor site. This article presents details on the surgical technique and provides discussion of the authors' experiences.

Soft-tissue defects of the distal foot that involve an exposed tendon and bone demonstrate a reconstructive challenge for plastic surgeons. This report investigates the feasibility and reliability of metatarsal artery perforator (MAP)-based propeller flap for reconstruction of the distal foot soft-tissue defects. Between July 2011 and June 2012, six patients underwent distal foot reconstruction with seven MAP-based propeller flaps. Five flaps were based on the third metatarsal artery and two flaps were based on the first metatarsal artery. The flap size ranged from 4 × 2 cm to 8 × 4 cm. All fl

Thin and pliable flaps with long, high calibre pedicles are ideally suited to lining the inside of the mouth. The radial forearm free flap has been our flap of choice until now, but we are unhappy with its potential for complications at the donor site. As an alternative, 30 patients have been treated in our unit with peroneal perforator flaps. Magnetic resonance (MR) angiography is necessary preoperatively to identify major perforating vessels. Flaps were raised using a lateral approach after the position of the most suitable perforator had been marked on the skin. The skin flaps were outlined

The perforator flap has proven to be effective as both a free flap and a pedicled flap in the reconstruction of a variety of regions. Usually, a V-Y advancement pattern or simple ellipse is the design selected for pedicled perforator island flaps. On the other hand, the transposition of multiple perforator flaps or skin grafting on the donor sites is required for larger defects. The authors used a stellate design to elevate a perforator flap with large dimensions, whilst allowing the easier closure of the donor defect. This method was used for reconstructing the sacral (six cases), trochanteri

Advances in the field of microsurgical reconstruction have focused on decreasing donor site morbidity and increasing the function and aesthetics of the reconstructed site. Since the advent of perforator flap surgery, most of these expectations have been satisfied. On the other hand, we need refinements in the surgical techniques and clinical reports studying these flaps. In the future, the clinical use of these flaps and the familiarity of surgeons will increase; perforator flap reconstruction will be as reliable as other types of free flaps.

The anterolateral thigh adipofascial flap is a vascularized flap prepared from the adipofascial layer of the anterolateral thigh region. It is a perforator flap based on septocutaneous or musculocutaneous perforators of the lateral circumflex femoral system. With methods similar to those used for the free anterolateral thigh flap, only the deep fascia of the anterolateral thigh and a 2-mm-thick to 3-mm-thick layer of subcutaneous fatty tissue above the fascia were harvested. In 11 cases, this flap (length, 5 to 11 cm; width, 4 to 8 cm) was used for successful reconstruction of extremity defect

Pharyngo-esophageal and tracheostomal defects pose a challenge in head and neck reconstruction whenever microanastomosis is extremely difficult in hostile neck that is previously dissected and irradiated. The deltopectoral (DP) flap was initially described as a pedicled flap for such reconstruction with acceptable postoperative results. A major drawback is still that the DP flap is based on 3 perforator vessels leading to a decreased arc of rotation. The DP flap also left contour deformities in the donor site. The internal mammary artery perforator flap was described as a refinement of the del

Muscle perforator flaps have become an important resource for the creation of cutaneous flaps based on musculocutaneous perforators, but without inclusion of the involved muscle. As a chimeric flap with or without the muscle, the cutaneous perforator flap can specifically serve as a sentinel or monitoring flap to allow the early detection of anastomotic compromise involving the common source vessel, without the need for direct observation of the major free-flap component. This can be a valuable adjunctive use of muscle perforator flaps for the continuous assessment of free muscle flaps or as a

We report a case of raising a previous gluteal fasciocutaneous flap again as a perforator-based flap to cover a recurrent defect. This case illustrates that conventional flap tissue with a preserved perforator can be recycled as a perforator flap, and that this method can be an efficient surgical option for recurrent defects.

Petrosectomy is a debilitating intervention, consisting of a resection of the bone forming the external auditory canal, the middle ear and sometimes the internal ear as well. The cavity formed after this surgery can lead to infectious complications. Reconstruction is an essential element for patients' rehabilitation. Most cases require local rotation flaps such as temporal muscle flap. However, when the remaining defect is too large or when the structures have been altered by radiotherapy, free flaps are the most adequate solution for repair. Upon review of the literature, there are very few a
