The death of a non-professional bodybuilder with a history of performance-enhancing drug (PED) use was thoroughly investigated. At the decedent's residence, investigators found four boxes of methylprednisolone tablets, one galenic preparation of sibutramine, and two unlabelled galenic compounds. Blood and hair samples were collected during autopsy. Toxicological analysis revealed the presence of methylprednisolone (19 ng/mL), methandrostenolone (27 ng/mL), and sibutramine (29 ng/mL) in blood. Hair analysis confirmed sibutramine (80 pg/mg) and methandrostenolone (1.28 ng/mg). GC-MS analysis of

BACKGROUND: Legal performance-enhancing substances (PES), such as creatine, are commonly used by adolescents and young adults. As PES are mostly unregulated by the US Food and Drug Administration, there has been limited empirical attention devoted to examining their long-term safety and health outcomes. Preliminary studies have demonstrated associations between PES use and severe medical events, including hospitalizations and death. PES could be linked to cardiovascular disease (CVD), the most common cause of mortality in the US, by altering the myocardium, vasculature, or metabolism. The obje

BACKGROUND: Performance-enhancing substance (PES) use is a potential public health concern in recreational fitness. Research suggests that use among recreational gym users is associated with psychosocial factors, gym culture, and peer norms.
OBJECTIVE: This study examined associations of Theory of Planned Behavior (TPB) constructs, selected Health Belief Model (HBM) constructs, knowledge, and risk acceptance with PES-use intentions and explored prevention needs using a mixed-methods design.
METHODS: A cross-sectional mixed-methods survey included 772 recreational gym users in Germany; 29.4%

H2 has been clinically demonstrated to provide antioxidant and anti-inflammatory effects, which makes it an attractive agent in exercise medicine. Although exercise provides a multiplicity of benefits including decreased risk of disease, it can also have detrimental effects. For example, chronic high-intensity exercise in elite athletes, or sporadic bouts of exercise (i.e., noxious exercise) in untrained individuals, result in similar pathological factors such as inflammation, oxidation, and cellular damage that arise from and result in disease. Paradoxically, exercise-induced pro-inflammatory

Recognizing IPED Use in Clinical Practice Abstract. The non-medical use of image- and performance-enhancing drugs (IPEDs) is widespread in the fitness and bodybuilding scene. The reasons for IPED use are often hedonistic in nature and they are used in so-called "cycles" over several weeks. The most common side effects are: testicular atrophy, acne, hypersexuality, hypertension, gynecomastia, lipid metabolism disorders, mood swings, hair loss, and policythemia. Common consequences following IPED use are: decreased libido, oligo- or azoospermia, and erectile dysfunction. To reduce undesirable si

Caffeine is widely used in sport and exercise because of its established ergogenic effects, yet problematic use, dependence, and withdrawal in athletic populations remain poorly characterized. This structured narrative review used a systematic database search and prespecified selection process to identify studies directly addressing problematic caffeine use, dependence, use disorder, withdrawal, tolerance, habituation, or misuse in sport- and exercise-related populations. PubMed, Web of Science Core Collection, and Scopus were searched from inception to 15 June 2026. Four studies met the eligi

Background/Objectives: Caffeine is widely recognized as an ergogenic aid, yet evidence regarding its acute effects on repeated sprint ability (RSA) remains inconsistent. This systematic review and meta-analysis aimed to evaluate the effects of acute caffeine ingestion on RSA across different populations, exercise modalities, and dosage levels. Methods: A comprehensive literature search was conducted in the PubMed, EBSCO, Cochrane Library, Web of science, and Scopus databases. Data were pooled using the weighted mean difference (WMD) with 95% confidence interval (CI). Results: Thirteen studies

This meta-analysis explored the effects of sodium bicarbonate supplementation on swimming performance. Seven databases were searched to find relevant studies. A random-effects meta-analysis of standardized mean differences (SMD) was performed to analyze the data. Nine studies were included in the review. There was no significant difference between placebo and sodium bicarbonate when considering data from all included studies (SMD: -0.10; p = 0.208) or in the subgroup analysis for 91.4-m and 100-m swimming tests (SMD: 0.11; p = 0.261). In the subgroup analysis for 200-m and 400-m swimming tests

In collaboration with various Anti-Doping Organisations and the Paris WADA-accredited laboratory, the International Testing Agency spearheaded the implementation of the Endocrine Module ahead of the Olympic Games Paris 2024. This article presents a data-driven evaluation of its early integration within Athlete Biological Passport (ABP) programs. The assessment of endocrine profile testing follows the complete ABP sample lifecycle, from collection to the submission of reports by the Athlete Passport Management Unit. The article identifies both challenges and opportunities by integrating testing

During the last 30 years, the artificial increase of red blood cell volume ("blood doping") has changed the level of performance in all endurance sports. Many doping scandals have shown the extent of the problem. The detection of blood doping relies on two different approaches: the direct detection of exogenous manipulating substances (erythropoietic stimulants) or red cells (homologous transfusion) and the indirect detection, where not the doping substance or technique itself, but its effect on certain biomarkers is measured. Whereas direct detection using standard laboratory procedures such

The Enhanced Games (EG) represents a controversial shift in competitive athletics by removing anti-doping regulations and allowing the sanctioned use of performance-enhancing drugs (PEDs) in pursuit of maximum performance. This unprecedented model may fundamentally alter sports culture, competition standards, and the medical landscape in which athletes train and compete. Of particular concern are the potential consequences for orthopedic and musculoskeletal health, as PED use, particularly anabolic agents and growth factors, can both enhance and compromise musculoskeletal function. Potential b

BACKGROUND: "Image- and performance-enhancing drugs" are widely used in sports worldwide to improve body image and performance goals. In view of the growing research interest and use of these substances, and the paucity of data on the situation in Switzerland, we conducted a scoping literature review on evidence of the use and users of these substances in Switzerland.
METHODS: A scoping review was conducted following the Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) statement. We searched PubMed/Medline, Embase and Google Scholar

ISSUE: Performance and image-enhancing drug (PIED) use among women is a growing phenomenon, particularly within weight-training populations. Despite increasing prevalence, research on the psychosocial factors involved in women's PIED use remains emergent and fragmented.
APPROACH: This systematic review of mixed methods studies uses an integrative approach to explore perspectives, motivations and psycho-social factors associated with women's use of PIEDs within weight-training populations. The review also investigates gendered experiences that may exist among women who use PIEDs.

Physicians should possess specific diagnostic and pharmacotherapeutic skills in order to recognize symptoms associated with doping use. It is important to be on the alert in athletes and fitness enthusiasts for physical and psychological changes due to use of anabolic steroids such as acne, stretch marks, gynecomastia, signs of acromegaly, irascibility and lethargy. Stimulants such as amphetamines, ephedrine and cocaine lead to fat loss and increased alertness; their main side effects are cardiac problems, behavioural changes and addiction. In addition to anabolic steroids and stimulants, eryt

Herbal tonic and adaptogens are often used to improve overall well-being. However, few clinical evidence supports their use and their safety is not known before marketing. In this context, the aim of our study was to analyze the spontaneous reports of suspected adverse reactions (ARs) to performance enhancing herbal dietary supplements collected by the Italian Phytovigilance System. Between March 2002 and September 2020, 110 spontaneous reports were collected, 58 of which related to products containing botanicals, alone or in association. Twenty-three serious reactions were reported, 21 of whi

Doping with anabolic-androgenic steroids (AAS) is common among both male and female athletes and is a growing public health problem. Review of historical data of systematic state-sponsored doping programs implemented by the German Democratic Republic in elite female athletes and from clinical trials of testosterone administration in non-athlete women suggests that AAS have ergogenic effects in women. The use of AAS in female athletes has been associated with adverse effects that include acne, hirsutism, deepening of the voice and menstrual disturbances; life-threatening adverse effects such as
