Background Sepsis remains a leading cause of intensive care unit (ICU) admission and is frequently associated with organ dysfunction and high mortality due to impaired microvascular perfusion. As peripheral hypoperfusion is central to septic shock, reliable bedside tools capable of detecting early microcirculatory failure are critically important. Although mean arterial pressure (MAP) and serum lactate are routinely used in sepsis assessment, both have notable limitations. MAP may remain preserved through compensatory vasoconstriction despite ongoing microcirculatory collapse, while lactate el

BACKGROUND AND AIMS: It is important to predict and prevent post-spinal hypotension in lower segment cesarean section (LSCS). Peripheral vascular tone can be monitored as a perfusion index (PI) from a pulse oximeter. We aimed to study baseline PI as a predictor of post-spinal hypotension in LSCS.
MATERIAL AND METHODS: Prospective observational study conducted in a tertiary care teaching public hospital on patients posted for elective LSCS under spinal anesthesia. Baseline PI and hypotension were compared. A receiver operating characteristic (ROC) curve was plotted and data were analyzed using

BACKGROUND: Endoscopic thoracic sympathectomy (ETS) is indicated for refractory hyperhidrosis. The pulse oximetry-derived perfusion index (PI) quantifies pulsatile blood flow at the oximeter. Thoracic sympathectomy increases blood flow; thus we postulate it will reliably increase PI. We evaluated the ipsilateral finger PI as a predictor of successful sympathectomy during ETS.
METHODS: After institutional review board approval and informed consent, 100 adult patients undergoing bilateral ETS were studied. Finger and earlobe pulse oximetry probes were placed. Hemodynamic variables and PI were c

INTRODUCTION: The Donor Risk Index (DRI) is a widely used liver transplant allograft risk model but does not account for the increasing adoption of machine perfusion (MP).
METHODS: Using Bayesian updating, we incorporated MP into the DRI framework (DRI-MP). A Bayesian proportional hazards model with informative priors derived from the original DRI was applied to Organ Procurement and Transplantation Network data from January 2022 to June 2024. Model performance was assessed using Harrell Concordance-statistic, calibration plots, and Brier scores.
RESULTS: DRI-MP, defined as DRI × 0.7 for MP

ObjectiveSeptic shock is a serious medical condition characterized by extreme inflammation and blood vessel permeability. Fluid resuscitation is a crucial treatment for septic shock, but the optimal approach for fluid management remains controversial. In this study, we explored how blood perfusion index (PI) monitoring can help direct fluid resuscitation therapy for septic shock.MethodsBetween January 2019 and December 2021, 50 patients with septic shock were admitted to The Affiliated Hospital of Hangzhou Normal University and randomly assigned to either a control or study group (25 cases eac

BACKGROUND: Peripheral vascular disorders leading to tissue hypoperfusion play a central role in the pathophysiology of organ failure in septic shock. The Doppler snuffbox resistive index (SBRI) can be an accurate parameter to evaluate the status of peripheral vasculature at the bedside. We evaluated whether the SBRI is related to lactate levels or the peripheral perfusion index (PI) and its ability to predict lactate clearance in septic patients.
METHODS: We conducted a prospective observational study in a tertiary general and teaching hospital in China. From July 2019 to December 2019, all

BACKGROUND: The ductus arteriosus is a necessary structure in fetal circulation, and its patency can produce hemodynamic alterations. The diagnostic gold standard is echocardiography, not always available. In the neonatal intensive care unit (NICU), they have pulse oximetry that measures perfusion index (PI), which could be used as a diagnostic tool in hemodynamically significant patent ductus arteriosus (HSPDA).
OBJECTIVE: The objective of the study was to correlate the PI increment (ΔPI) in 24 and 72 h after birth with HSPDA in premature newborns of NICU in a second-level hospital.

BACKGROUND: Cardiac dysfunction can modify renal perfusion, which is crucial to maintain sufficient kidney tissue oxygenation. Renal cortex perfusion assessed by dynamic ultrasound method is related both to renal function and cardiac hemodynamics. The aim of the study was to test the hypothesis that Renal Perfusion Index (RPI) can more closely reflect cardiac hemodynamics and differentiate etiology of chronic cardio-renal syndrome.
MATERIAL AND METHODS: Twenty-four patients with hypertension and chronic kidney disease (CKD) at 2-4 stage (12 with hypertensive nephropathy and 12 with CKD prior

Background & objectivesFollowing surgical treatment and subsequent immobilisation of one limb, physiological reductions in blood flow are expected due to limited or no movement. This study was designed to investigate the change in perfusion index in a resting limb when the contralateral limb exercises (active and passive). MethodsThis was a two-arm comparative study with 39 healthy participants (22 males, 17 females) with a mean (SD) age of 23.4 (5.2) years. One limb was set to exercise (active and passive in different sessions), and another limb rested with a pre-designed exercise protocol. W

BACKGROUND: Many studies have examined the risk factors for postoperative acute kidney injury (AKI), but few have focused on intraoperative peripheral perfusion index (PPI) that has recently been shown to be associated with postoperative morbidity and mortality. Therefore, this study aimed to evaluate the relationship between intraoperative PPI and postoperative AKI under the hypothesis that lower intraoperative PPI is associated with AKI occurrence.
METHODS: We retrospectively searched electronic medical records to identify patients who underwent surgery at the general surgery department fro

BACKGROUND/AIMS: In clinical settings, the pulsatility index (PI) has become a widely used tool for monitoring obstetrics or other vascular diseases. It is based on the maximum Doppler shift waveform derived from ultrasonography. However, it remains unclear whether the PI levels are correctly predicted from the perfusion in mouse model of hindlimb ischemia.
METHODS: To explore the relationship between PI and perfusion, we generated a unilateral hindlimb ischemia model in 8-week-old C57BL/6 male mice by ligation of the right common iliac artery and femoral artery. These mice were monitored wit

BACKGROUND: Perfusion index (PI) could reflect peripheral flow. Preterm infants with hemodynamically significant patent ductus arteriosus (hsPDA) will have left-to-right shunt across PDA causing less blood flow to the lower legs.
OBJECTIVE: To evaluate pre- and postductal PI differences (ΔPI) in hsPDA.
METHODS: Preterm infants with gestational age <34 weeks were assessed for ΔPI on days 1, 3, and 7 of life with simultaneous echocardiography. Based on echocardiography, each infant was categorized into hsPDA, non-hsPDA, and no PDA.
RESULTS: Thirty infants (16 males), median age 31 weeks (inte

Multi-wave CO-oximetry, utilizing the oxygen reserve index (ORi), estimates arterial partial pressure of oxygen (PaO2) in mild hyperoxemia, between 100 and 200 mmHg, and concurrently quantifies local perfusion at the measurement site using the perfusion index (PI). This study explores how variations in PI influence the accuracy of ORi in estimating PaO2 in anesthetized dogs. Data from 37 mechanically ventilated dogs were retrospectively reanalyzed using a different approach. ORi and PI values were collected using a CO-oximeter. The data were categorized into four groups based on PI quartiles.

Background and Objectives: We examined the association between the baseline perfusion index (PI) and changes in intraoperative body temperature during general anesthesia. The PI reflects the peripheral perfusion state. The PI may be associated with changes in body temperature during general anesthesia because the degree of redistribution of body heat from the central to the peripheral compartment varies depending on the peripheral perfusion state. Materials and Methods: Thirty-eight patients who underwent brain surgery were enrolled in this study. The baseline PI and body temperature of the pa

OBJECTIVES: Tissue oxygen saturation and peripheral perfusion index are proposed as early indirect markers of hypovolemia in trauma patients. Hypovolemia is associated with increased sympathetic nervous activity. However, many other stimuli, such as pain, also increase sympathetic activity. Since pain is often present in trauma patients, its effect on the indirect measures of hypovolemia needs to be clarified. The aim of this study was, therefore, to explore the effects of hypovolemia and pain on tissue oxygen saturation (measurement sites: cerebral, deltoid, forearm, and thenar) and finger ph

AIM: The perfusion index (PI) and its dynamic change during respiration, and supressed the plethysmographic variability index (PVI), are calculated from pulse oximetry, and these indexes were recently proposed for continuous and noninvasive assessment of peripheral perfusion in neonates. We aimed to assess the effect of patent ductus arteriosus (PDA) on PI and PVI, according to ductal Doppler flow pattern.
METHODS: Forty-five neonates with median (Q25-75) gestational age (GA) and birthweight of 27 (25-28) weeks and 857 (750-1080) grams, respectively, were assessed prospectively using serial e

The impact of patent ductus arteriosus (PDA) on vital sign trends represented as histograms, and perfusion index in particular, is unknown. This study aimed to split continuously obtained PI and other vital signs before, during, and after medical treatment of PDA, into histogram bins, and determine the utility of PI and other vital sign histograms in the early prediction of hemodynamically significant PDA (hsPDA). In 34 infants at a mean gestational age of 26 ± 2.1 weeks, we prospectively collected vital signs for three different periods, 24 h before starting treatment of PDA, during PDA treat
