Peri-implantitis. Part 2: Prevention and maintenance of peri-implant health.

The prevention of any disease process should be the cornerstone of any healthcare provision. This ethos is well established in dentistry with plaque associated disease such as periodontitis and caries but is at the current time less developed for peri-implantitis. The current review identities potential modifiable and non-modifiable risk factors for per-implantitis development and details strategies for the prevention of the disease. These include poor oral hygiene, previous history of periodontitis, smoking, genetic factors, occlusal overload and foreign body reactions. Local factors include

Microbial Dysbiosis, Titanium Release, and Peri-implantitis.

The peri-implant mucosal barrier is a unique microenvironment where host-microbiome interactions take place on the surface of an implanted biomaterial. Therefore, peri-implant immunity not only is quintessential to oral health but also contributes to the maintenance of the biomaterial-tissue equilibrium in health. This review delves into the intricate interplay between host factors, biomaterial properties, and the microbiome with a focus on the mechanisms underlying peri-implant dysbiosis. Investigations into this complex milieu have led to the emerging understanding of titanium particles rele

Brain abscess caused by dental peri-implantitis.

We report the case of a patient presenting with severe headache and progressive aphasia due to a large brain abscess. End-stage peri-implantitis on a dental implant in the upper jaw with contact to the maxillary sinus floor causing maxillary and frontal sinusitis could be identified as the source of infection. Multidisciplinary treatment included broad-spectrum antibiotics, craniotomy for abscess drainage, sinus surgery, and the removal of the implant. Despite all the care taken, the patient died, underlining the importance of oral hygiene and a frequent recall in patients with dental implants

Characterization of macrophages infiltrating peri-implantitis lesions.

OBJECTIVES: The mechanisms involved in the initiation and progression of peri-implantitis lesions are poorly understood. It was the aim to determine the content and activation status of macrophages present in human peri-implantitis lesions and compare the current findings with the macrophage polarization associated with periodontitis lesions. MATERIAL AND METHODS: A total of 14 patients were studied in this investigation. Seven were soft tissue biopsies from dental implants affected by peri-implantitis that required explantation. Seven biopsies were from chronic periodontal disease. Immunoflu

Antibiotic resistance in human peri-implantitis microbiota.

OBJECTIVES: Because antimicrobial therapy is often employed in the treatment of infectious dental implant complications, this study determined the occurrence of in vitro antibiotic resistance among putative peri-implantitis bacterial pathogens. METHODS: Submucosal biofilm specimens were cultured from 160 dental implants with peri-implantitis in 120 adults, with isolated putative pathogens identified to species level, and tested in vitro for susceptibility to 4 mg/l of doxycycline, 8 mg/l of amoxicillin, 16 mg/l of metronidazole, and 4 mg/l of clindamycin. Findings for amoxicillin and metronid

Role of fibroblast populations in peri-implantitis.

PURPOSE: To understand the contribution of stromal cells, such as granulation tissue fibroblasts, to peri-implantitis with regard to (1) the secretion of constitutive factors promoting migration/survival of infiltrates into osseointegrated sites; and (2) the effect of exogenous infiltrate cytokines on the cells' secretion. MATERIALS AND METHODS: Fibroblasts were cultured from eight peri-implantitis sites. Multiplexed enzyme-linked immunosorbent assay was used to quantify factors secreted by the cells either unstimulated or stimulated with gamma interferon (IFN gamma), interleukin 4 (IL4), or

Microbiota of Peri-Implant Healthy Tissues, Peri-Implant Mucositis, and Peri-Implantitis: A Comprehensive Review.

Understanding the microbiological profiles of peri-implant conditions is crucial for developing effective preventive and therapeutic strategies. This narrative review analyzes the microbial profiles associated with healthy peri-implant sites, peri-implant mucositis, and peri-implantitis, along with related microbiological sampling and analyses. Healthy peri-implant sites are predominantly colonized by Streptococcus, Rothia, Neisseria, and Corynebacterium species, in addition to Gram-positive cocci and facultatively anaerobic rods, forming a stable community that prevents pathogenic colonizatio

A Murine Model of Lipopolysaccharide-Induced Peri-Implant Mucositis and Peri-Implantitis.

Dental implants are a widely used treatment option for tooth replacement. However, they are susceptible to inflammatory diseases such as peri-implant mucositis and peri-implantitis, which are highly prevalent and may lead to implant loss. Unfortunately, the understanding of the pathogenesis of peri-implant mucositis and peri-implantitis is fragmented and incomplete. Therefore, the availability of a reproducible animal model to study these inflammatory diseases would facilitate the dissection of their pathogenic mechanisms. The objective of this study is to propose a murine model of experimenta

Epigenetic factors associated with peri-implantitis: a review.

Peri-implant diseases are characterized by the resorption of hard tissue and the inflammation of soft tissue. Epigenetics refers to alterations in the expression of genes that are not encoded in the DNA sequence, influencing diverse physiological activities, including immune response, inflammation, and bone metabolism. Epigenetic modifications can lead to tissue-specific gene expression variations among individuals and may initiate or exacerbate inflammation and disease predisposition. However, the impact of these factors on peri-implantitis remains inconclusive. To address this gap, we conduc

Cost-effectiveness of non-surgical peri-implantitis treatments.

AIM: The purpose of the present study was to assess the cost-effectiveness of various alternatives of non-surgical peri-implantitis treatment. MATERIALS AND METHODS: A decision analytical model was constructed and populated with parameter estimates from recent literature for reduction in pocket probing depth (PPD) in response to eight different treatment alternatives. A micro-costing approach combined with an online expert survey was applied to simulate a decision-making scenario taking place in Germany. The treatment alternatives providing the most advantageous cost/outcome combinations were

The aim of the present study was to examine differences in cellular characteristics of human peri-implantitis and periodontitis lesions. Two groups of patients were included: 40 patients with generalized severe chronic periodontitis and 40 patients presenting with severe peri-implantitis. Soft tissue biopsies were obtained from diseased sites (probing pocket depth ≥ 7 mm with bleeding on probing) and prepared for histologic and immunohistochemical analysis. In contrast to periodontitis samples, peri-implantitis lesions were more than twice as large and contained significantly larger area propo

Peri-implantitis: a systemic burden for our patients.

Peri-implantitis-an increasingly prevalent chronic inflammatory disease-has been linked not only to peri-implant bone destruction and implant failure, but also to systemic inflammation and chronic inflammatory diseases. Accumulating evidence suggest that peri-implant inflammation can contribute to a sustained systemic low-grade inflammatory state characterized by elevated inflammatory mediators, like interleukin (IL)-6 and C reactive protein, episodic bacteremia, and metabolic alterations. This persistent inflammatory burden has been associated with distal organ dysfunction, including cardiova

Peri-Implantitis and Periodontitis: Biological Convergence, Contextual Divergence.

Periodontitis and peri-implantitis are chronic inflammatory conditions of microbial origin that affect the supporting tissues of teeth and dental implants, respectively. Although they share comparable clinical features and several histopathological characteristics, they have traditionally been classified as distinct diseases, largely because they arise in different anatomical and clinical contexts. This narrative, question-driven review evaluates clinical, microbiological, histological, and molecular evidence to address whether observed similarities and differences between the two conditions r
