Aggressive and acute periodontal diseases.

Inflammatory periodontal diseases are highly prevalent, although most of these diseases develop and progress slowly, often unnoticed by the affected individual. However, a subgroup of these diseases include aggressive and acute forms that have a relatively low prevalence but show a rapid-course, high rate of progression leading to severe destruction of the periodontal tissues, or cause systemic symptoms that often require urgent attention from healthcare providers. Aggressive periodontitis is an early-onset, destructive disease that shows a high rate of periodontal progression and distinctive

Systemic effects of periodontal diseases.

A number of studies suggest an association between periodontal disease and cardiovascular disease, pulmonary disease, diabetes, and pregnancy complications. Presently, the data must be regarded as preliminary. Additional large-scale longitudinal epidemiologic and interventional studies are necessary to validate these associations and to determine whether the associations are causal. The goal of this article is to review the history of this concept, describe the biologically plausible circumstances that may underlie these potential associations, and provide a summary of the published literature

Herbal medicine for periodontal diseases.

Traditional Chinese Medicine (TCM) is a treasure-house which has shown miraculous clinical effects. It has aroused tremendous interests in elucidating its mechanisms both in China and abroad. This paper presents a review of published articles on the effects and mechanisms of herbal medicine on periodontal disease. Specifically, two modifications of an ancient compound prescription were serially studied, Guchiwan (Tooth-firming pills) and Guchigao (Tooth-firming extract). Some in vitro studies have shown suppressive effects of various herbal extracts on bone resorption by isolated osteoclasts.

[Periodontal diseases with Entamoeba gingivalis].

At the beginning of our century Entamoeba gingivalis was considered to be a pathogenic bacteria, capable to induce parodontal lesions. Later on it was also found in healthy persons, and the germ was less interesting from the medical view-point. In the present study the authors report their findings concerning E. gingivalis in 135 patients with various stomatological affections including: dental caries, parodontopathies, pulpitis, gangrene, ulcero-necrotic stomatitis etc. The study was started following the discovery of the amoeba in the gingival exsudate of a male aged 19 years with chronic su

Current concepts in periodontal diseases.

Periodontal diseases are common oral diseases that afflict all humans to some degree. The major aetiological agent is dental plaque--the complex microflora which forms on teeth in the absence of effective oral hygiene. The interaction of the microbial flora and the periodontal tissues produces an inflammatory response and tissue breakdown. Recent information has categorized periodontal diseases on the basis of increased knowledge about the particular microorganisms associated with the different clinical conditions. In addition, the important role of host defences, in particular the phagocytic

Periodontal Manifestations of Systemic Diseases.

This paper provides a detailed analysis of systemic diseases associated with periodontal tissue loss, focusing on their clinical presentation and etiopathogenesis. It also introduces a framework for categorizing these diseases according to their principal pathological pathways and their periodontal effects. Periodontitis arises from a disruption of host-microbe homeostasis, which leads to a dysbiotic microbiota, chronic inflammation, and subsequent periodontal tissue loss. Complex systemic diseases, particularly those causing systemic inflammation or having an autoimmune component (e.g., diabe

Microbial diagnostics in periodontal diseases.

Microbial analytical methods have been instrumental in elucidating the complex microbial etiology of periodontal diseases, by shaping our understanding of subgingival community dynamics. Certain pathobionts can orchestrate the establishment of dysbiotic communities that can subvert the host immune system, triggering inflammation and tissue destruction. Yet, diagnosis and management of periodontal conditions still rely on clinical and radiographic examinations, overlooking the well-established microbial etiology. This review summarizes the chronological emergence of periodontal etiological mode

Autoimmune responses in periodontal diseases.

Periodontal diseases are characterized by localized infections and inflammatory conditions and directly affect teeth supporting structures which are the major cause of tooth loss. Several studies have demonstrated the involvement of autoimmune responses in periodontal disease. Evidences of involvement of immunopathology have been reported in periodontal disease. Bacteria in the dental plaque induce antibody formation. Lymphocytes, cytokines and complement system are reported to have an important role in the progression of periodontal disease. The present review describes the involvement of aut

The nature of periodontal diseases.

Periodontal diseases are infections. They have a number of properties in common with infections in other parts of the body, but have unique features resulting from the passage of the tooth through the soft tissue integument into the oral cavity. The tooth provides a solid, non-shedding surface for the colonization of potentially pathogenic bacterial species as well as a wide range of host-compatible species. Periodontal pathogens frequently colonize the periodontal area for prolonged periods of time prior to disease initiation. Disease is caused by a finite set of bacterial species leading to

Periodontal diseases in young individuals.

The prevalence and severity of periodontal diseases in young people are significant. Recent progress in understanding the etiology and pathogenesis of periodontal diseases in this group of patients has allowed significant improvement in periodontal classifications. Actinobacillus actinomycetemcomitans characteristically is the major pathogen involved; black-pigmented anaerobic rods and other organisms can also be important. Treatment of periodontitis should focus on meticulous plaque control and eradication of periodontal pathogens, possibly by means of systemic or topical antimicrobial therap

Periodontal muco-adhesive formulations for the treatment of infectious periodontal diseases.

Extensive efforts have recently been focused on targeting a drug or delivery system in a particular region of the body for extended period of time, not only for local targeting of drugs but also for the better control of systemic drug delivery. The concept of periodontal drug delivery systems has fascinated many investigators to the possible use of various polymers, which can overcome various physiological barriers in long-term drug delivery, there by rendering the treatment more effective and safe for local disorders and systemic problems. Presence of a smooth and relatively immobile surface

Periodontal Diseases and Host Modulation Therapies.

In periodontal diseases, bacteria and microbes serve as the primary pathogens that initiate the condition, while the host refers to the body that hosts these microorganisms. Host modulation therapy is a drug-based treatment that can be used along with systematic periodontal therapy to help the disease get better and stop tissue damage. There is a new way to treat periodontitis called "Host-Modulation Therapy" (HMT). It involves reducing the bacteria biofilm through daily good oral hygiene and regular scaling and root planing (SRP). Using host modulation therapies to lower inflammation, restore

Antimicrobial advances in treating periodontal diseases.

Antibiotics are generally an efficient means of treating bacterial infections, and therefore are an obvious candidate in the treatment of periodontal diseases. Systemically and locally administered antimicrobial agents of all kinds have been evaluated in multiple clinical trials. The vast majority of studies have tested antibiotics as adjuncts to non-surgical debridement. No regime has demonstrated superiority over systemically administered amoxicillin and metronidazole in the treatment of any clinically or microbiologically defined variant of periodontal disease. The frequency and consequence

Diabetes mellitus and inflammatory periodontal diseases.

PURPOSE OF REVIEW: Periodontal diseases are inflammatory conditions that were once thought to have manifestations localized to the oral cavity alone, and were therefore considered the concern of only dentists and other oral health professionals. Emerging evidence has changed this view and now suggests that periodontal diseases may play a role in numerous conditions that impact systemic well being, including diabetes mellitus. This review examines the relationships that exist between periodontal diseases and diabetes mellitus, with a focus on potential common pathophysiologic pathways including

Etiology and pathogenesis of periodontal diseases.

The two most prevalent and most investigated periodontal diseases are dental plaque-induced gingivitis and chronic periodontitis. The last 10 to 15 years have seen the emergence of several important new findings and concepts regarding the etiopathogenesis of periodontal diseases. These findings include the recognition of dental bacterial plaque as a biofilm, identification and characterization of genetic defects that predispose individuals to periodontitis, host-defense mechanisms implicated in periodontal tissue destruction, and the interaction of risk factors with host defenses and bacterial

Interface of malnutrition and periodontal diseases.

In response to periodontal pathogens neutrophils release oxidants, proteinases, and other destructive factors. The balance between these factors, the antioxidants, and endogenously synthesized antiproteinases determine the extent of periodontal damage. Malnutrition, particularly protein-energy malnutrition involving concomitant deficiencies of antioxidant nutrients, is characterized by impairment in production and cellular actions of the cytokines, diminished acute-phase protein response (APR) to infections, endocrinopathies, defective metabolism of drugs, and impaired response to stress.
