Hemostatic management for periodontal treatments in patients on oral antithrombotic therapy: a retrospective study.

PURPOSE: We retrospectively investigated hemostatic management for periodontal treatments in patients on oral antithrombotic therapy. PATIENTS AND METHODS: A total of 155 periodontal treatment procedures were performed in 139 patients who continued taking conventional antithrombotic drugs. Insertion of oxidized cellulose, compression, and suturing were used as local hemostatic measures. When hemostasis was difficult, hemorrhage was stopped using electrocautery and/or splint. RESULTS: For the warfarin patients, 49 scaling procedures were performed in patients with INR of 4.82 or less, and 52

Splinting of traumatized teeth with a new device: TTS (Titanium Trauma Splint).

Displacement injuries of permanent teeth are an increasing emergency in the dental office. Children and adolescents are particularly prone to dental trauma due to participation in risky activities. Repositioning or replantation with subsequent stabilization by a dental splint is the standard of care for most displaced or avulsed permanent teeth. Non-rigid fixation allowing physiologic tooth mobility has been shown to be desirable for periodontal healing. A flexible splint of short duration appears to reduce the risk of dentoalveolar ankylosis or external replacement resorption. Different splin

A computer model of the periodontal ligament space in man.

An engineering model of the periodontal ligament fibres was used to calculate length changes in the fibres with: direct occlusal forces on the tooth, occlusal loads on other teeth in the arch and splinting. The fibre elongations appear to influence tooth eruption, the clinical behaviour of splinted and non-splinted teeth, and the establishment of vertical dimension of occlusion and occlusal plane height. The model also provides insight into the controversy surrounding the intermediate plexus of the ligament.

[Fixation of injured tooth].

In cases of an avulsed or luxated tooth, splinting of the injured tooth is recommended. Splint is defined as a fixation of the injured tooth to adjacent teeth in the dental arch. The preferred splint type is semi-rigid, which should be placed for 10 to 14 days and enable proper oral hygiene and will not cause any kind of impingement to the gingiva. It is effective in reducing the replacement resorption process, which affects the periodontal tissue surrounding roots exposed to dental trauma.

Tooth-supported telescopic prostheses in compromised dentitions: a clinical report.

This article describes indications for treatment with telescopic restorations on patients with periodontally and endodontically compromised dentitions that require splinting, with special emphasis on treatment that restores the entire dental arch. Stabilization of compromised teeth with fixed splinted restorations is usually inadvisable because of the risk factors involved, such as eventual localized abutment failure. Detachable telescopic prostheses may be preferred as a near equivalent or substitute because they can be detached and repaired without reconstruction of the entire restoration. R

[A new splint technique in dental traumatology].

The treatment of displacement injuries and fractures of permanent teeth is an important emergency therapy in the dental office. Due to new sport trends and various outdoor activities, the frequency of dental trauma of children and adolescents alike is steadily rising. The standard treatment of displacement or avulsion injuires of permanent teeth is repositioning or replantation with subsequent splinting. The dental trauma splint must be flexible to allow for the physiologic movement of the repositioned or replanted teeth in order to reduce the risk for ankylosis or external root resorption. St

Fractured alveolar process displacement evaluation-Effect of the rigidity of wire-composite splints.

BACKGROUNDS/AIMS: Traumatic dental injuries with bone fractures require rigid splinting. The degree of rigidity of the wire-composite splints can be challenging. The aim of this study was to evaluate the effect of the rigidity of different wire-composite splints on alveolar process displacement. MATERIAL AND METHODS: A model was created using polystyrene resin to simulate the anterior maxilla, bovine incisors to simulate maxillary incisors, and polyether impression material to simulate the periodontal ligament. An alveolar process fracture was created, while maintaining the labial bone connec

The effect of type of restoration on the stress field developed in terminal abutments with severely reduced periodontal support and coronal structure.

STATEMENT OF PROBLEM: Periodontally compromised teeth (PCT) that serve as terminal abutments (TAs) are often challenging depending on the post-and-core treatment, the type of partial fixed dental prosthesis (PFDP), and the periodontal support. PURPOSE: The purpose of this study was to investigate the biomechanical impact of 3 types of PFDP supported by cast post-and-cores on PCT serving as terminal abutments. MATERIAL AND METHODS: A 3-dimensional (3D) model of a human mandible was fabricated by using computed tomography (CT) images and parameterized in a computer-aided design (CAD) environme

The effect of interdental continuous loop wire splinting and intermaxillary fixation on the marginal gingiva.

To study the influence of interdental loop-wire splinting and intermaxillary fixation on the marginal gingiva, 30 patients were evaluated clinically using different periodontal parameters, at 5 examination times. It was shown that despite a standardized oral hygiene regime including the use of a mouthrinse, gingival inflammation occurred for the duration of the splinting period. Factors other than the presence of limited plaque, such as gingival trauma due to splint application and subsequent mechanical irritation should be considered as possible aetiological factors. All investigated marginal

Clinical Case Report on Treatment of Generalized Aggressive Periodontitis: 5-Year Follow-up.

Generalized aggressive periodontitis (GAgP) is a distinct type of periodontal disease associated with considerably more rapid periodontal tissue destruction than chronic periodontitis. This study presents the 5-year follow-up of a patient with GAgP. A 29-year-old man reported experiencing increasing gingival recession. He was treated using cause-related therapy, provisional splints, and flap surgery combined with allograft grafting and was followed up for 5 years. This case study shows that elimination of infectious microorganisms and meticulous long-term maintenance provide an effective treat

Assessment of Intraoral Resin Splints Secured With Cerclage Wires for Mandibular Fracture Repair in 16 Dogs.

The aim of this study was to determine if an intraoral resin splint that is secured to the mandible with cerclage wires, a cerclage intraoral resin splint (CIRS), is a viable, minimally invasive option for mandibular body fracture repair in dogs. A medical record search identified 16 dogs with fractures as a result of periodontal disease, endodontic disease, head trauma, or a combination of these causes, and CIRS was performed in partially or completely edentulous areas of the mid or caudal mandibular body. At the time of appliance removal, the fracture sites of 13 dogs had healed with a bony

Effect of splinting times on the healing of intra-alveolar root fractures in 512 permanent teeth in humans: A Scandinavian multicenter study.

BACKGROUND/AIM: Long-term splinting of teeth with intra-alveolar root fractures has been considered important for the deposition of hard tissue between the root fragments. The aim of this study was to compare the healing outcomes in teeth with intra-alveolar root fractures relative to splinting times in three dental centers in Scandinavia, using historical data. MATERIALS AND METHODS: A total of 512 maxillary and mandibular incisors from three dental trauma centers were included in the study. Two of the centers used long-term splinting protocols of two to three months while the other center u

Esthetic considerations when splinting with fiber-reinforced composites.

The primary reasons for splinting and stabilizing teeth are to connect them for the purpose of replacing missing teeth or as an adjunct to periodontal therapy. Although the restorations must be planned to withstand the functional requirements of occlusion and mastication, esthetic considerations must also be taken into account. The challenge in creating an esthetic result with fiber-reinforced composite splints is that there is limited space in the connector region to create the three-dimensional effect required to give teeth the appearance of individuality. Careful planning in the diagnosis a

Does temporary splinting before non-surgical therapy eliminate scaling and root planing-induced trauma to the mobile teeth?

The objective of this study was to determine whether temporary splinting of periodontitis-affected mobile teeth, prior to non-surgical mechanical therapy, affects treatment outcome by eliminating scaling and root planing-induced trauma to the teeth. Mandibular anterior teeth of 29 patients (15 females; 14 males; age range 30 to 48 years) with adult periodontitis were evaluated. Clinical measurements were performed at 4 sites per tooth including mesial, distal, buccal and lingual aspects at baseline, 3 and 6 months after treatment. The following clinical parameters were analyzed: plaque index (

Collaborative management of severe periodontal-endodontic lesion with intense lip sucking: a case report.

BACKGROUND: Lip behaviors can lead to maxillofacial developmental disorders such as anterior open bite, posterior crossbite, increased overjet, and a higher risk of developing a Class II malocclusion. However, lip-sucking, a behavior often underreported and overlooked, may adversely affect the periodontal health of permanent teeth in adolescents under extreme conditions if not promptly identified and managed. CASE PRESENTATION: We report a unique case of an 11-year-old boy with a severe periodontal-endodontic lesion on tooth 32, caused by prolonged, high-frequency, and high-intensity lip suck

The effect of immediate controlled forces on periodontal healing of teeth replanted after short dry time in dogs.

BACKGROUND/AIM: The extra-alveolar dry period and storage medium in which a tooth is kept prior to replantation remain critical factors affecting the survival and repair potential of the periodontal ligament in avulsed teeth. When replantation is not immediate, replacement root resorption (RRR) is the most common complication. The aim of this histological study was to evaluate the effect of immediate controlled-orthodontic forces in periodontal healing of replanted teeth in a canine model. MATERIAL AND METHODS: Sixty maxillary and mandibular premolar roots were endodontically treated in vivo
