Upper Extremities Peripheral Nerve Injuries and Reconstruction.

It is important to recognize neurological symptoms postemergency or elective surgery of the upper limb, as it could herald injury to the peripheral nerves. Prompt diagnosis, workup, and management of these conditions may affect eventual clinical outcomes. We present a series of cases in different orthopaedic-related scenarios to discuss the variety of causes and treatment options available for this group of patients. This study reflects level of evidence V.

Peripheral nerve injury.

Knowledge of the actions of conventional neurotrophic factors on peripheral neurons is accumulating rapidly, and growth factors that were discovered outside the nervous system have been found to have neurotrophic actions. The nature and initiation of proximal neuronal responses to nerve injury, the interactions of Schwann cells and growth cones, and the functions of macrophages in the peripheral nervous system are all areas of active investigation. Results of the surgical treatment of injuries to the brachial plexus and spinal accessory nerve are reported.

Distal nerve transfers for peripheral nerve injuries: indications and outcomes.

Distal nerve transfer is a refined surgical technique involving the redirection of healthy sacrificable nerves from one part of the body to reinstate function in another area afflicted by paralysis or injury. This approach is particularly valuable when the original nerves are extensively damaged and standard repair methods, such as direct suturing or grafting, may be insufficient. As the nerve coaptation is close to the recipient muscles or skin, distal nerve transfers reduce the time to reinnervation. The harvesting of nerves for transfer should usually result in minimal or no donor morbidity

Polyethylene Glycol-Fusion Repair of Peripheral Nerve Injuries.

Axons successfully repaired with polyethylene glycol (PEG) fusion tecnology restored axonal continuity thereby preventing their Wallerian degeneration and minimizing muscle atrophy. PEG fusion studies in animal models and preliminary clinical trials involving patients with digital nerve repair have shown promise for this therapeutic approach. PEG fusion is safe to perform, and given the enormous potential benefits, there is no reason not to explore its therapeutic potential.

Peripheral nerve injuries from intramuscular injection of drugs.

17 patients who were treated at the University of Nigeria Teaching Hospital for peripheral nerve paralysis resulting from intramuscular injection of drugs are analyzed. Ten of them were below the age of 5 years. Sciatic nerve was affected in 16 patients and only one patient had median nerve paralysis. Penicillin, ampicillin, tetracycline, paraldehyde and optalgin were some of the offending drugs. The prognosis was poor as only three patients achieved a partial recovery of function in the paralyzed nerves. With determination and public education, this iatrogenic tragedy could be eradicated.

Current surgical management of traumatic peripheral nerve injuries.

Traumatic peripheral nerve injuries can result in severe disability, due to varying degrees of neuropathic pain, sensory deficits, motor impairment, or a combination of these symptoms, often with a profound impact on quality of life. Accurate clinical evaluation and appropriate diagnostic investigations are essential for the timely identification of such injuries and for determining the most suitable treatment strategy. The surgical management of peripheral nerve injuries encompasses a range of surgical techniques, which may be used individually or in combination, with the goal of restoring ne

Sonographic evaluation of pediatric traumatic peripheral nerve injuries.

Ultrasound offers distinct advantages over MRI in the assessment of upper extremity nerve injuries in children. The radiologist has the advantage of direct visualization and physical examination, as well as dynamic maneuvers to reproduce nerve symptoms. Image acquisition is generally faster than MRI and does not require sedation as the patient does not need to be motionless. MRI of small nerves requires long scan times for adequate resolution, while ultrasound offers time-independent enhanced resolution over MRI. Ultrasonographic findings can discriminate high-grade from low-grade nerve injuri

Optimal timing for repair of peripheral nerve injuries.

BACKGROUND: Data regarding outcomes after peripheral nerve injuries is limited, and the optimal management strategy for an acute injury is unclear. The aim of this study was to examine timing of repair and specific factors that impact motor-sensory outcomes after peripheral nerve injury. METHODS: This was a single-center, retrospective study. Patients with traumatic peripheral nerve injury from January 2010 to June 2015 were included. Patients who died, required amputation, suffered brachial plexus injury, or had missing motor-sensory examinations were excluded. Motor-sensory examinations wer

[Surgical treatment of lower extremity peripheral nerve injuries].

Peripheral nerve injuries of the lower extremities are not frequent. The most common are traction injury of the peroneal nerve at the knee level or iatrogenic trauma of the pelvic nerves during abdominal surgery. Civil sharp injuries are rare.Indications for surgical revision follow the general rules of nerve surgery. Sharp injury should be treated as soon as possible, ideally within 72 hours. Closed lesions are indicated for surgery if a complete denervation remains unchanged three months after the injury. Best results can be achieved within six months from the injury. Irritations caused by b

Update on tendon transfers for peripheral nerve injuries.

Tendon transfer surgery to restore fundamental wrist and hand function is made possible by the redundancy that exists among the actions of our upper-extremity musculature. Potential donors for transfer are those muscles with adequate power to motor the recipient tendon, similar tendon excursion to the recipient, and function in phase with the recipient. Resolution of wound healing, union of fractures, and mobilization of stiff joints are prerequisites for a functioning tendon transfer. Injuries to the radial, median, and ulnar nerves occur above (high nerve injury) and below the elbow (low ner

Peripheral nerve injuries in athletes. Treatment and prevention.

Peripheral nerve lesions are uncommon but serious injuries which may delay or preclude an athlete's safe return to sports. Early, accurate anatomical diagnosis is essential. Nerve lesions may be due to acute injury (e.g. from a direct blow) or chronic injury secondary to repetitive microtrauma (entrapment). Accurate diagnosis is based upon physical examination and a knowledge of the relative anatomy. Palpation, neurological testing and provocative manoeuvres are mainstays of physical diagnosis. Diagnostic suspicion can be confirmed by electrophysiological testing, including electromyography an

Peripheral nerve injuries: studies in higher nonhuman primates.

Two skeletally mature baboons underwent comparative neurorrhapies following transsection of both ulnar nerves without loss of nerve tissue. In baboon 1 there was no difference in objective evaluations at 12 months when an epineurial neurorrhaphy was compared to a perineurial fascicular neurorrhaphy. In baboon 2, the one that had interfacicular grafts in one upper extremity and an epineurial neurorrhaphy on the other side, there was significantly better functional return on the epineurial side than one the graft side. Grass and histological evaluation showed that the grafts retained their integ

Use of superficial peroneal nerve graft for treating peripheral nerve injuries.

OBJECTIVE: To evaluate the clinical results from treating chronic peripheral nerve injuries using the superficial peroneal nerve as a graft donor source. METHODS: This was a study on eleven patients with peripheral nerve injuries in the upper limbs that were treated with grafts from the sensitive branch of the superficial peroneal nerve. The mean time interval between the dates of the injury and surgery was 93 days. The ulnar nerve was injured in eight cases and the median nerve in six. There were three cases of injury to both nerves. In the surgery, a longitudinal incision was made on the an

Peripheral nerve injuries in athletes: a case series of over 200 injuries.

We retrospectively reviewed electrodiagnostic (EDX) studies performed on 346 athletes with sports injuries who were referred to our EDX laboratory from 1974 to 1997. These injuries included 216 nerve root, plexus, or peripheral nerve injuries sustained by 180 of the athletes. Eighty-six percent of the injuries were to the upper extremity. Athletes with nerve fiber injuries participated in 27 different sports, but over one third of injuries were sustained playing football. The most common symptomatic upper extremity injury was the "burner" (N=40). Forty-three athletes had median neuropathies, m

[Iatrogenic injury of peripheral nerves].

There are many risks of iatrogenic peripheral-nerve injuries during routine medical procedures. These injuries may occur during venipuncture for drawing blood, endoscopic treatments, punctures of joints or ganglions, various kinds of surgical procedures, and in numerous other situations. It is important to create a "Manual" of such accidents or incidents. In case an accident occurs, both the medical staff and the injured patient should receive adequate support to avoid any anxiety. The doctor must examine the person's injury carefully, and must judge its severity as soon as possible. The docto

Peripheral nerve response to injury.

Oral and maxillofacial surgeons caring for patients who have sustained a nerve injury to a branch of the peripheral trigeminal nerve must possess a basic understanding of the response of the peripheral nerves to trauma. The series of events that subsequently take place are largely dependent on the injury type and severity. Regeneration of the peripheral nerve is possible in many instances and future manipulation of the regenerative microenvironment will lead to advances in the management of these difficult injuries.
