Late radiation injury to peripheral nerves.

Although the peripheral nerve has often been considered as radioresistant, clinical practice demonstrates the occurrence of radiation-induced peripheral neuropathies. Because these complications appear late, usually several years after the course of radiotherapy, their occurrence is explained by improvement in the prognosis of several cancers. Their physiopathology is not fully understood. Compression by radio-induced fibrosis probably plays a central role but direct injury to nerves and blood vessels is probably also involved. The most frequent and best known form of postradiation neuropathy

Paraneoplastic syndromes of the peripheral nerves.

PURPOSE OF REVIEW: To describe the paraneoplastic disorders of the motor and sensory nerves and neurons, and their immunologic associations. RECENT FINDINGS: Recently proposed diagnostic criteria for paraneoplastic disorders may assist in determining the likelihood a given neuropathy or neuronopathy is related to an underlying malignancy. Of this group of disorders, paraneoplastic sensory neuronopathies are the most frequent; many of these patients have anti-Hu antibodies and small-cell lung cancer. There is often motor, autonomic, or central nervous system involvement, and electrophysiologic

Multiple schwannomas in the peripheral nerves.

Multiple tumours of peripheral nerves are often seen in patients with neurofibromatosis of type 1 or 2. Multiple schwannomas may occur without other manifestations of neurofibromatosis. We have reviewed 12 patients with multiple schwannomas arising from peripheral lesions who did not fulfil the criteria for either type of neurofibromatosis. Four had spinal and one an intracranial lesion in addition to the peripheral tumours. Two patients had one and three café-au-lait spots, respectively, and another had a probable family history. The largest tumours were 45 to 250 mm in size. Three patients h

Course of the peripheral gustatory nerves.

The multiple variations of the course of the gustatory nerves still considered possible are discussed. Recent investigations lead to the conclusion that there is only one path for the gustatory fibers for each gustatory area: 1) from the anterior part of the tongue via the tympanic cord and facial nerve to the medulla oblongata; 2) for the posterior part of the tongue in the IX cranial nerve; and 3) from the soft palate via the greater superficial petrosal nerve to the facial nerve. The trigeminal nerve carries no gustatory fibers to the brain.

An ultrastructural study of blood vessels in peripheral nerves of leprosy patients: blood vessels in peripheral nerves.

Ultrathin sections of nerves of tuberculoid and lepromatous leprosy were examined in an electron microscope for changes in endoneural blood vessels. In the tuberculoid nerves, hypertrophy of endothelial cells was the most prominent feature. This was to such an extent that the lumen of blood vessel was often closed. Endoneural blood vessels showed multilayers separated by collagen and ground substances. In contrast, in the lepromatous nerves, there was no hypertrophy of endothelial cells in the blood vessels and the lumen of the vessels was open. M. leprae were seen within the endothelial cells

The central-peripheral transitional regions of cranial nerves. Trochlear and abducent nerves.

Unlike all other nerves containing somatic efferent fibres, the trochlear nerve emerges from the dorsal aspect of the brainstem. It generally emerges as a single trunk which resembles a dorsal rather than a ventral spinal nerve rootlet in terms of its size and of the morphology and position of the central tissue projection which it contains. The morphology of the central-peripheral transition of the trochlear nerve is therefore correlated with its dorsal location rather than with the nature of its constituent fibres. By contrast, abducent nerve rootlets emerge from the ventral aspect of the ne

Cerebrospinal Fluid Flow Extends to Peripheral Nerves.

UNLABELLED: Cerebrospinal fluid (CSF) is an aqueous solution responsible for nutrient delivery and waste removal for the central nervous system (CNS). The three-layer meningeal coverings of the CNS support CSF flow. Peripheral nerves have an analogous three-layer covering consisting of the epineurium, perineurium, and endoneurium. Peripheral axons, located in the inner endoneurium, are bathed in "endoneurial fluid" similar to CSF but of undefined origin. CSF flow in the peripheral nervous system has not been demonstrated. Here we show CSF flow extends beyond the CNS to peripheral nerves in a c

Electrodiagnostic Testing for Disorders of Peripheral Nerves.

Nerve conduction studies and electromyography are useful diagnostic tools that neurologists use to diagnose diseases of the peripheral nerves, neuromuscular junction, and muscles. These tests are considered an extension of clinical history and examination, and their results should always be interpreted with the clinical context. Neuromuscular diseases are common and affect a large proportion of the elderly population. With an aging population in expansion, these diseases are expected to become even more prevalent. It is important to highlight the basics of electrophysiology and provide a refer

Cutaneous pain in disorders affecting peripheral nerves.

Our ability to quickly detect and respond to harmful environmental stimuli is vital for our safety and survival. This inherent acute pain detection is a "gift" because it both protects our body from harm and allows healing of damaged tissues [1]. Damage to tissues from trauma or disease can result in distorted or amplified nociceptor signaling and sensitization of the spinal cord and brain (Central Nervous System; CNS) pathways to normal input from light touch mechanoreceptors. Together, these processes can result in nagging to unbearable chronic pain and extreme sensitivity to light skin touc

Rerouting peripheral nerves for spinal cord lesions.

To provide control of paralyzed limb muscles following spinal cord lesions, peripheral nerves containing motor axons from motoneurons located above a spinal cord lesion could potentially be rerouted to nerves containing motor axons located below the spinal cord lesion. To test this hypothesis in rats, the distal end of a cut tibial nerve, innervated by the L4-6 spinal level, was anastomosed or rerouted to the central end of the cut femoral nerve, innervated by the L3-4 spinal level. Appropriate controls were used. Recovery of lower hind limb motor function was followed at regular intervals, me

Magnetic stimulation of the human peripheral nerves.

In this report some preliminary data after magnetic stimulation of the peripheral nerves in 8 normal individuals are presented. Compound muscle action potentials were recorded by surface electrodes after magnetic and conventional electrical stimulation of the median, ulnar, peroneal, tibial, femoral and sciatic nerves, and the lumbosacral roots. The data clearly show the ability of the magnetic coil to stimulate the roots in sites relatively inaccessible to electrical stimulation and obtain consistent muscle response suggesting important clinical application of magnetic stimulation in lumbosac

Mechanical effects of compression of peripheral nerves.

Effects of graded compression on nerve function were analyzed in order to evaluate the relative importance of pressure level and duration of compression for functional deterioration. The pressure was applied by means of a small inflatable cuff. The effects of two pressure levels, i.e., 80 mm Hg applied for 2 hr or 400 mm Hg applied for 15 min, were studied in rabbit tibial nerves. The lower pressure tested, which is known to induce ischemia of the compressed nerve segment, also causes some degree of mechanical deformation of the nerve trunk, which leads to incomplete recovery following pressur

[Microsurgical method of transplantation of peripheral nerves].

The authors present the microsurgical technique of grafting thin cutaneous nerves at the site of peripheral nerve defect. The application of grafts in place of end-to-end suture makes possible elimination of nerve stretching at the time of suturing and at the time of rehabilitation. The use of cutaneous nerves makes possible good revascularization of grafts and their correct regeneration. The use of the microsurgical technique makes possible a good evaluation of the fascicular structure of nerve trunks, their adequate resection, and good adaptation of nerve stumps and grafts connecting the cor
