Persistent postural-perceptual dizziness (PPPD) is a newly defined diagnostic syndrome that unifies key features of chronic subjective dizziness, phobic postural vertigo and related disorders. It describes a common chronic dysfunction of the vestibular system and brain that produces persistent dizziness, non-spinning vertigo and/or unsteadiness. The disorder constitutes a long-term maladaptation to a neuro-otological, medical or psychological event that triggered vestibular symptoms, and is usefully considered within the spectrum of other functional neurological disorders. While diagnostic tes

Persistent postural-perceptual dizziness (PPPD) is characterized by one or more symptoms of dizziness, unsteadiness, or non-spinning vertigo, which persist on most days for at least 3 months. The most common symptom of PPPD worsens when standing or walking, often leading to confusion with other forms of orthostatic dizziness (OD). There are some main differential diagnosis as follows: hemodynamic OD, postural orthostatic tachycardia syndrome, vestibular syncope, BPPV, bilateral vestibulopathy, primary orthostatic tremor, sensory neuropathy, neurodegenerative disorders, cerebral small vessel di

Vestibular migraine and persistent postural-perceptual dizziness both involve the vestibular system and are similar in clinical manifestations. After acute attack of vestibular migraine, it can gradually evolve into persistent posture-perceptual dizziness; persistent posture-perceptual dizziness caused by various factors can be combined with symptoms similar to vestibular migraine. Studies have shown that abnormal multi-sensory signal integration, abnormal neurotransmitters and genetic factors may be the co-disease mechanism of the two.

INTRODUCTION: Persistent postural-perceptual dizziness (PPPD) is a chronic functional vestibular disorder defined by persistent dizziness, unsteadiness, or non-spinning vertigo exacerbated by upright posture, active or passive motion, and visually complex environments. In clinical practice, however, the diagnostic label alone is often insufficient to guide management, because patients differ substantially in the relative prominence of visual dependence, motion-postural burden, multisensory provocation, affective-hypervigilant mechanisms, and migraine overlap.
OBJECTIVE: To propose a clinicall

Persistent postural-perceptual dizziness (PPPD) is a chronic disorder of perceived unsteadiness. Symptoms can be exacerbated in visually complex stationary or moving environment. Visual dependence and increased motion sensitivity are predictors for PPPD but its pathophysiology remains unknown. We hypothesized an abnormal sensory-perceptual scaling mechanism in PPPD and tested visual- and vestibular perceptional thresholds in 32 patients and 28 age-matched healthy control subjects (HC). All participants showed normal vestibular function tests on quantitative testing. Visual motion coherence thr

OBJECTIVE: To investigate differences in the clinical characteristics of patients with persistent postural-perceptual dizziness (PPPD) according to age.
METHODS: We retrospectively reviewed 143 patients diagnosed with PPPD. Patients were classified into three groups by age: young group (19 to 44 years, n = 60), middle-age group (45 to 64 years, n = 56), old group (65 to 85 years, n = 27). Demographic data, scores of the Dizziness Handicap Inventory (DHI), the Niigata PPPD Questionnaire (NPQ), the Hospital Anxiety and Depression Scale (HADS), precipitating conditions, and the results of vestib

BACKGROUND: The condition of persistent postural perceptual dizziness (PPPD) is a common chronic vestibular disorder that is thought to be due to a central vestibular maladaptation. The condition is frequently encountered in those with a history of migraine, anxiety, or panic attacks. Many cases of PPPD are preceded by vestibular disorders, with vestibular migraine thought to precede at least 20% of cases.
PURPOSE: Those with a history of migraine have been shown to have difficulties with sensory integration, habituation, and intolerance to stimuli. This often leads to discomfort associated w

The concept and diagnostic standard of persistent postural-perceptual dizziness(PPPD) improve the ability of clinical workers to recognize these diseases to some extent. However, the diagnosis of PPPD mainly depends on the identification of postural symptoms and the elimination of structural vestibular diseases, so the subjectivity is relatively strong. Moreover, the lack of objective criteria and a slight carelessness will lead to generalized diagnosis or omission diagnosis and misdiagnosis. At present, there is a lack of domestic large sample epidemiological investigation and other related s

BACKGROUND: Despite clear diagnostic criteria established in 2017, delivering and explaining the diagnosis of persistent postural-perceptual dizziness (PPPD) can be challenging.
METHODS: We outline a step-by-step approach to clearly explain the diagnosis, underlying mechanisms and treatment options to patients with PPPD, followed by a brief overview of current treatment approaches. This approach is adapted from the Cambridge-Calgary Consultation Model and previous recommendations for functional neurological disorders but is mainly based on our expertise accrued over decades and input from pat

BACKGROUND AND PURPOSE: Persistent postural-perceptual dizziness (PPPD) is a chronic functional vestibular disorder characterized by persistent dizziness or postural instability that is exacerbated by upright posture, motion, and complex visual stimuli. Although neuroimaging studies suggest altered visual, vestibular, and emotional processing in PPPD, comprehensive resting-state functional magnetic resonance imaging evidence remains limited. This study investigated spontaneous brain activity and functional connectivity alterations in patients with PPPD.
METHODS: Twenty-one patients with PPPD

BackgroundIndividuals with dizziness often face balance issues and anxiety, which can result in reduced physical activity levels and decreased participation in their daily activities. Few studies have evaluated physical activity levels in individuals with chronic dizziness, and none have specifically focused on the two most prevalent types: vestibular migraine (VM) and persistent postural perceptual dizziness (PPPD).ObjectiveEvaluate self-reported physical activity levels in individuals diagnosed with PPPD and VM who are awaiting vestibular rehabilitation.MethodsIn a retrospective cross-sectio

BACKGROUND: Patients with long-COVID suffer from symptoms that continue or develop after a COVID-19 or SARS-CoV-2 infection and are present for four or more weeks after the initial infection. This case series describes a group of previously healthy adolescent patients with long-COVID who were seen in a pediatric vestibular clinic for evaluation of severe dizziness and were diagnosed with persistent postural-perceptual dizziness (PPPD). By presenting their symptoms, management and treatment effects, this study aims to provide a diagnostic and therapeutic framework for providers who encounter th

Current consensus diagnostic criteria for vestibular migraine (VM) describes this as an episodic disorder. However, a minority of patients report prolonged (>72 h duration) or even persistent VM symptoms, prompting whether a chronic variant of vestibular migraine (CVM) should be introduced to the current classification and how best to define it. Here we summarize current evidence of such a potential chronic variant of VM and critically review proposed definitions for CVM. Potential approaches to establish a diagnostic framework for CVM include (a) following the distinction between episodic and

Persistent postural-perceptual dizziness (PPPD), defined in 2017, is a vestibular disorder characterized by chronic dizziness that is exacerbated by upright posture and exposure to complex visual stimuli. This review focused on recent neuroimaging studies that explored the pathophysiological mechanisms underlying PPPD and three conditions that predated it. The emerging picture is that local activity and functional connectivity in multimodal vestibular cortical areas are decreased in PPPD, which is potentially related to structural abnormalities (e.g., reductions in cortical folding and grey-ma

INTRODUCTION: Persistent postural-perceptual dizziness (PPPD) (ICD-11) and anxiety disorders (ANX) share behavioural symptoms like anxiety, avoidance, social withdrawal, hyperarousal, or palpitation as well as neurological symptoms like vertigo, stance and gait disorders. Furthermore, previous studies have shown a bidirectional link between vestibulo-spatial and anxiety neural networks. So far, there have been no neuroimaging-studies comparing these groups.
OBJECTIVES: The aim of this explorative study was to investigate differences and similarities of neural correlates between these two pati

BACKGROUND: Patients suffering from persistent postural-perceptual dizziness (PPPD) often experience postural instability that worsens when exposed to visual motion stimuli. We investigated how different visual motion stimuli affect patients' postural sway and their perceived egomotion during stance.
METHODS: 28 PPPD patients and 26 gender and healthy control subjects (HC) underwent posturographic measurements on a firm or foam platform while being exposed to either vestibular or visual motion stimuli or their combination. Vestibular stimuli were applied via 1.3 mA galvanic vestibular stimula
