Assessing patient understanding of pharmacogenomic test results: a qualitative study.

AIM: To examine whether patients with depression or chronic/acute pain understand the results of pharmacogenomic testing they had done as part of clinical research. METHODS: Semi-structured interviews were conducted with 45 patients who underwent pharmacogenomic testing and subsequently received their testing results by mail. These interviews assessed whether participants were aware that they had testing, how this testing impacted their current medications, and whether they grasped the future significance of this testing. A grounded theory approach was used to analyze this interview data. RE

Pharmacogenomic Testing for Psychotropic Medication Selection: A Systematic Review of the Assurex GeneSight Psychotropic Test.

BACKGROUND: A large proportion of the Ontario population lives with a diagnosed mental illness. Nearly 5% of Ontarians have major depressive disorder, and another 5% have another type of depressive disorder, bipolar disorder, schizophrenia, anxiety, or some other disorder not otherwise specified. Medications are commonly used to treat mental illness, but choosing the right medication for each patient is challenging, and more than 40% of patients discontinue their medication within 90 days because of adverse effects or lack of response. The Assurex GeneSight Psychotropic test is a pharmacogenom

Clozapine and pharmacogenomics testing: opportunities and challenges for personalized treatment in schizophrenia.

Clozapine remains the gold standard for treatment-resistant schizophrenia (TRS), offering unparalleled efficacy but accompanied by significant interindividual variability in response and risk of severe adverse effects. Pharmacogenomics (PGx), the study of how genetic variations influence drug response, has transformed treatment for other medications like warfarin but remains underutilized in clozapine prescribing. This review synthesizes current evidence on the potential of PGx to enhance clozapine treatment by improving the prediction of therapeutic response, metabolism, and adverse drug reac

Pharmacogenomic Testing for CYP2C19 Variants among Stroke Patients Treated with Clopidogrel: Opportunity for the Clinical Laboratory?

BACKGROUND: Clopidogrel is a widely used antiplatelet agent used to prevent adverse events for patients suffering from acute coronary syndromes and ischemic stroke. As a prodrug, clopidogrel must be converted to the active form through the enzyme cytochrome (CYP) P450 2C19 (among other enzymes). Individuals carrying a loss of function (LOF) allele (i.e., *2 and/or *3) have reduced pharmacologic efficacy. Ticagrelor is an alternative antiplatelet medication that is not a prodrug. METHODS: We reviewed the Clopidogrel in High-Risk Patients with Acute Nondisabling Cerebrovascular Events (CHANCE2)

Patient perspectives on pharmacogenomic (PGx) testing for antidepressant prescribing in primary care: a qualitative description study.

Many patients with major depressive disorder (MDD) try multiple antidepressants before finding one that works well and is tolerable. Pharmacogenomic (PGx) testing was developed to facilitate more efficacious prescribing. This technology has not been robustly implemented clinically. Patient perspectives are critical to policy decisions, but the views of patients with MDD about the use of PGx testing to guide antidepressant prescribing have not been extensively examined, particularly in publicly funded healthcare systems. The purpose of this qualitative description study was to produce actionabl

A study of HLA-B∗15:02 in a Sri Lankan population: Implications for pharmacogenomic testing.

HLA-B∗15:02 is known as a biomarker for carbamazepine (CBZ) induced Steven-Johnson Syndrome and Toxic Epidermal Necrolysis (SJS/TEN) in some Asian populations. Hence United States Federal Drug Administration (USFDA) recommends HLA-B∗15:02 screening for Asian and other populations with a high prevalence of HLA-B∗15:02, prior to the administration of carbamazepine. This study was conducted to estimate the prevalence of HLA-B∗15:02 in a cohort of Sri Lankans. We observed an overall prevalence of 4.3% (4/93) among 93 Sri Lankans comprising 32 Sinhalese, 30 Sri Lankan Tamils and 31 Moors. The allel

Perceptions of Pharmacogenomic Testing Among People With Treatment Resistant Depression: Legitimization as a Facilitator of Acceptance.

Pharmacogenomic testing for psychiatric medications has been proposed as both an early intervention to optimize treatment response, and for use among patients who have tried multiple medications without symptom remission. Therefore, this testing may be particularly salient to the subset of individuals with major depressive disorder for whom depression has been labeled as "treatment resistant". Understanding the impact of this diagnostic label on illness identity and attitudes towards new therapies is important as genomic technology expands and rates of depression increase. We sought to explore

Co-designing a study on perspectives toward pharmacogenomic testing in mental health care among youth and caregivers.

AIMS: Finding suitable psychotropic medications can be a lengthy trial-and-error process, leading to poor symptom control, low adherence, and treatment failure. Pharmacogenomic (PGx) testing can help inform prescribing considerations for some individuals. As PGx becomes more common in Canada, understanding patients' and families' hopes, expectations, and concerns is vital. MATERIALS AND METHODS: This article details our work engaging two advisory councils comprised of five youth with lived/living experience of mental health challenges and five caregivers across four videoconferencing sessions

Persistent benefit of pharmacogenomic testing on initial remission and response rates in patients with major depressive disorder.

BACKGROUND: In patients with major depressive disorder (MDD), achieving remission and/or response may take many months because of the lengthy trial-and-error process often needed to identify effective medication. Pharmacogenomic testing is a prescribing tool that has been shown to improve remission and response rates for MDD patients, but data describing its impact over time is limited. The objective of this study was to determine whether pharmacogenomic-guided treatment increases the rate of remission and response over time in patients with MDD, and if so, to assess the persistence of that ef

Clinical utility of combinatorial pharmacogenomic testing in depression: A Canadian patient- and rater-blinded, randomized, controlled trial.

The pharmacological treatment of depression consists of stages of trial and error, with less than 40% of patients achieving remission during first medication trial. However, in a large, randomized-controlled trial (RCT) in the U.S. ("GUIDED"), significant improvements in response and remission rates were observed in patients who received treatment guided by combinatorial pharmacogenomic testing, compared to treatment-as-usual (TAU). Here we present results from the Canadian "GAPP-MDD" RCT. This 52-week, 3-arm, multi-center, participant- and rater-blinded RCT evaluated clinical outcomes among p

Pharmacogenomic Testing and Depressive Symptom Remission: A Systematic Review and Meta-Analysis of Prospective, Controlled Clinical Trials.

Pharmacogenomic (PGx) testing has emerged as a compelling strategy that clinicians can use to inform antidepressant medication selection and dosing, but the clinical efficacy of this strategy has been questioned. We systematically reviewed and meta-analyzed clinical trials for an association between the use of PGx-guided antidepressant therapy and depressive symptom remission in patients with major depressive disorder (MDD). We included prospective, controlled clinical trials published in English up to July 12, 2022. Data extraction and synthesis adhered to the 2020 Preferred Reporting Items f

An Assessment of Real-World Evidence and Other Sources Supporting Payer Coverage Decisions for Pharmacogenomic Testing in Psychiatry.

Background: Sources and evidence cited to inform payer coverage decisions on pharmacogenomic (PGx) testing in psychiatry are presently underexplored. Methods: We conducted a qualitative and quantitative assessment of publicly available coverage policies from 14 US payers, examining the number and both the type and source of citations across policies and coverage decisions. Payers were classified as for-profit or mutual fund versus non-profit or government, and their coverage decisions were categorized as either coverage (limited or specified) or no coverage. Results: Among 32 unique sources ci

A real-life pilot study of the clinical application of pharmacogenomics testing on saliva in epilepsy.

Response to antiseizure medications (ASMs) can be influenced by several gene polymorphisms, causing either lower efficacy or higher occurrence of adverse drug reactions (ADRs). We investigated the clinical utility of salivary pharmacogenomic testing on epilepsy patients. A commercialized pharmacogenomic salivary test was performed in a cohort of epileptic patients. Genetic variants on five genes (i.e., CYP1A2, CYP2C9, CYP2C19, EPHX1, and ABCB1) involved in common ASMs metabolism were selected. Twenty-one individuals (median age [Q1 -Q3 ]: 15 [6.5-28] years) were enrolled. Six patients harborin

Real-World Impact of Pharmacogenomic Testing on Medication Use and Healthcare Resource Utilization in Patients With Major Depressive Disorder.

BACKGROUND: Pharmacogenomic (PGx) testing can help improve response and remission rates for patients with major depressive disorder (MDD) and at least one treatment failure. To investigate real-world outcomes, we examined 1) significant gene-drug interactions (GDIs) and 2) healthcare resource utilization (HRU) in a large US insurance claims dataset. METHODS: Weighted multigene PGx testing results in adult patients with MDD were linked with deidentified US claims data. The PGx test report organized medications as congruent (no known or moderate GDI) or incongruent (significant GDI). Medication

Real-World Characterization of Psychiatric Pharmacogenomic Test Ordering and Clinical Relevance in Adults and Children.

Pharmacogenomic (PGx) testing is increasingly utilized to guide psychiatric medication selection and dosing, yet real-world characterizations of test ordering and clinical implications remain understudied. Additionally, the extent to which psychiatry-focused PGx results inform prescribing beyond mental health medications is unclear. This study examined patterns of psychiatry-focused PGx test ordering, the clinical relevance of results among adult and pediatric patients, and implementation challenges within a large health system.
