Dynamic evaluation and continuous improvement model of pharmaceutical administration based on DPSIR-TOPSIS.

BACKGROUND: This study aimed to evaluate the dynamic development of pharmaceutical administration at Beijing Chaoyang Hospital from 2019 to 2024 and to identify key influencing and optimization factors using a data-driven analytical framework. METHODS: An integrated “entropy weight–TOPSIS”model was constructed based on the DPSIR (Driving force–Pressure–State–Impact–Responses) framework. The model quantified trends in pharmaceutical administration performance using multi-dimensional indicators, including policy, resource allocation, drug utilization, and pharmacist capacity. RESULTS: The compos

Pharmacy-based methadone treatment in the US: views of pharmacists and opioid treatment program staff.

BACKGROUND: The US federal regulations allow pharmacy administration and dispensing of methadone for opioid use disorder (PADMOUD) to increase the capability of opioid treatment programs (OTPs) in providing methadone maintenance treatment (MMT) for opioid use disorder (OUD) as part of a medication unit. However, there is a lack of research data from both pharmacy and OTP staff to inform the implementation of PADMOUD. METHODS: Staff of a pharmacy (n = 8) and an OTP (n = 9) that participated in the first completed US trial on PADMOUD through electronic prescribing for methadone (parent study) w

Evaluating the impact of specialty pharmacies on access to biologics in patients with moderate-to-severe asthma.

OBJECTIVE: To evaluate whether specialty pharmacy use reduces delays from biologic prescription to insurance approval and administration in patients with moderate-to-severe asthma. METHODS: We conducted a retrospective cohort study of patients prescribed asthma biologics in the Bronx, New York. Cohorts were defined by time period: before specialty pharmacy implementation (2017-2020) and after (2020-2023). Primary outcomes were time from prescription to insurance approval and administration. Wilcoxon rank-sum tests compared groups, and multivariable regression models adjusted for covariates.

Preparing for pharmacy-based delivery of long-acting injectable antiretrovirals: a pre-implementation study.

BACKGROUND: Use of long-acting injectable antiretroviral therapy (LA-ART) for human immunodeficiency virus (HIV) treatment and prevention is increasing, but there are challenges that could limit broad expansion of these important treatments. The goal of this study was to explore attitudes, barriers, and facilitators for implementing LA-ART administration within community pharmacies. METHODS: We conducted a mixed-methods study focusing on pre-implementation aspects of community pharmacy-administered LA-ART. Pharmacists, clinic staff members, and persons with HIV completed a baseline survey foll

Prevalence of unclaimed prescriptions at military pharmacies.

BACKGROUND: Prescriptions that are ordered by physicians but not picked up by patients represent a potential quality improvement opportunity in health systems. Previous research has demonstrated that anywhere from as little as 0.28% to as much as 30.0% of prescriptions are unclaimed, and that 0.45% to 22.0% of patients fail to claim prescriptions. In the Military Health System (MHS), prescriptions filled at military pharmacies are dispensed with no copayment, providing an opportunity to examine the factors that contribute to unclaimed prescriptions other than out-of-pocket cost. OBJECTIVES: T

Creating privacy in pharmacies in West Africa.

There is a substantial unmet need for contraception in West Africa. Providing contraception in private pharmacies, which often are highly accessible, may alleviate some access barriers. However, privacy is a key concern for provision of contraceptives in pharmacies, and whether users view pharmacies as private is ambiguous. While some pharmacy users note their privacy benefits, others suggest a lack of privacy. The ACEPT Project aims to introduce the administration of injectable and implant contraceptives into pharmacies across Côte d'Ivoire, Senegal, and Togo. Baseline data collected prior to

Pharmaceutical compounding or pharmaceutical manufacturing? A regulatory perspective.

At one time, nearly all prescriptions were compounded preparations. There is an ongoing demand for compounded prescription medications because manufacturers cannot fulfill the needs of all individual patients. Compounding pharmacies are a long standing yet less frequently discussed element in the complex matrix of prescription drug manufacturing, distribution, and patient use. The drug shortage situation for many necessary and life-saving drug products is a complicating factor that has led to the numerous quality issues that currently plague large-scale compounding pharmacies. The states are t

Buprenorphine Dispensed by Pharmacies and Administered in Emergency Departments in Urban and Rural Areas - United States, 2019-2025.

Drug overdose deaths remain a major public health concern in the United States. Although buprenorphine is approved by the Food and Drug Administration to treat opioid use disorder and reduces overdose risk and deaths, the medication remains underused in the United States, and many persons who could benefit from treatment lack access. This report describes 2019-2025 trends in buprenorphine dispensing by pharmacies, treatment initiation among persons newly receiving buprenorphine, treatment retention, pharmacy availability, and emergency department (ED) administration by urban-rural county class

Consensus SEO-SEFH of recommendations for use and compounding of ophthalmic preparations.

There are many medicinal products that, although having shown efficacy and  safety in different ophthalmological indications, they are not authorized or  commercially available for ophthalmic administration. This implies, on one hand,  that they must be used according to legislation that regulates the availability of  medicines in special situations and, on the other hand, that they must be  prepared in the pharmacy services for ophthalmic administration, according to  quality criteria to ensure its effectiveness, stability and sterility. Increasing the use of long-acting injectable behavioral health medications in the community - A pharmacist-psychiatrist collaborative model.

BACKGROUND: Persons with severe mental illness or substance use disorder (SUD) benefit from long-acting injectable behavioral health medications (LAMs), which improve treatment adherence, reduce hospitalizations and emergency visits, and mitigate long-term biopsychosocial decline. LAMs remain underutilized due to multiple barriers across patients, pharmacies, providers, and payors. OBJECTIVE: The primary objective of this study was to conduct a time study and workflow analysis to inform a scalable and equitable payment model for community pharmacy-based administration of LAMs. METHODS: Six c

What should infectious diseases clinicians know about pharmacy benefit managers and their impact on our patients?

Pharmacy Benefit Managers (PBMs) are contracted by health plans, employers, and government programs to manage pharmacy and prescription drug benefits. They maintain drug formularies, process and pay pharmacy claims, establish and manage pharmacy networks, and negotiate prices with manufacturers and pharmacies. Despite PBMs' role in cost management, their lack of transparency and complex administrative processes can delay treatment and increase out-of-pocket costs, affecting vulnerable populations like those in pharmacy deserts. These factors influence healthcare delivery for persons living wit

A Method to Quantify Mean Hypertension Treatment Daily Dose Intensity Using Health Care System Data.

IMPORTANCE: Simple measures of hypertension treatment, such as achievement of blood pressure (BP) targets, ignore the intensity of treatment once the BP target is met. High-intensity treatment involves increased treatment burden and can be associated with potential adverse effects in older adults. A method was previously developed to identify older patients receiving intense hypertension treatment by low BP and number of BP medications using national Veterans Health Administration and Medicare Part D administrative pharmacy data to evaluate which BP medications a patient is likely taking on an

The vital role of clinical pharmacy services within the hospital at home interdisciplinary team.

PURPOSE: Hospital at home (HaH) program development, spurred by hospital congestion during the COVID-19 pandemic, is moving from a novel idea to a standard of inpatient practice. Understanding pharmacy's clinical role in the HaH patient care team is exceedingly important as programs across the country continue to develop, implement, and expand. The purpose of this study is to describe how clinical pharmacy services are currently provided for home hospital patients and to explain the vital role of pharmacists within the HaH care team. DESCRIPTION: A descriptive study was designed to evaluate p

[The Peking pharmacy institute and its teaching in the period of the Republic of China].

The Peking Pharmacy Institute, as the first independent pharmacy college in China, symbolised the beginning of the era of Chines people establishing pharmacy colleges and had a unique historical status and value. This is because it started in the period of the Republic of China, when Chinese pharmaceutical education was extremely deficient. Additionally, it had an important impact on pharmaceutical education in following generations as the predecessor of the Beijing Health Vocational College. This paper reports on the collation of relevant archives and historical materials, examination and exp

Tackling barriers to over-the-counter syringe access in community pharmacies.

A recent study in Harm Reduction Journal, "Pharmacy-related syringe access barriers: an audit of Oregon community pharmacies", reveals significant gaps in over-the-counter (OTC) syringe availability across community pharmacies in Oregon. These findings have urgent public health implications: without consistent pharmacy access, people who inject drugs (PWID) face increased risk of HIV, hepatitis C, and other injection-related harms. We examine possible factors that may be contributing to these gaps, including pharmacist stigma, administrative challenges, gaps in awareness surrounding state laws
