Complications of pectoralis major myo-cutaneous flap, anterolateral thigh flap and radial forearm free flap after total laryngectomy with partial pharyngectomy: A systematic review and network meta-analysis.

BACKGROUND: No consensus exists regarding the most effective reconstruction after total laryngectomy with partial pharyngectomy (TLwPP). This study aims to compare different reconstructive techniques (pectoralis major myo-cutaneous flap, PMMCF; anterolateral thigh flap, ALTF; radial forearm free flap, RFFF) after TLwPP in terms of pharyngocutaneous fistula (PCF), stenosis and feeding tube dependence (FTD) incidence. METHODS: The Scopus, PubMed/MEDLINE, Cochrane Library, and Google Scholar databases were searched. A single-arm meta-analysis was performed for PCF incidence, stenosis incidence,

Comparison of Survival Outcomes Between Larynx-Preserving Open Partial Pharyngectomy and Radiotherapy or Chemoradiotherapy in Patients with Hypopharyngeal Squamous Cell Carcinoma: A Single-Center Retrospective Analysis with Inverse Probability of Treatment Weighting Adjustments.

BACKGROUND: There is a group of hypopharyngeal squamous cell carcinoma (HPSCC) patients for whom larynx-preserving open partial pharyngectomy (PP) and radiotherapy/chemoradiotherapy (RT/CRT) are indicated. We aimed to retrospectively evaluate the survival difference as there is no evidence directly comparing the two therapies. METHODS: This study evaluated HPSCC patients who were initially treated by PP or RT/CRT at our institution between January 2007 and October 2019. Overall survival (OS), disease-specific survival (DSS), laryngectomy-free survival (LFS), and local relapse-free survival (L

Scintigraphic assessment of swallow efficiency postlaryngectomy.

There have been reports of a high incidence of hypopharyngeal stenosis in total laryngectomy patients when the surgery requires a partial pharyngectomy for pyriform sinus involvement. In this study, three groups were compared: total laryngectomy patients without partial pharyngectomy, total laryngectomy patients with partial pharyngectomy, and normal controls. All patients had received radiation therapy following surgery. All were maintaining oral nutrition, and none complained of dysphagia. Patients were tested between 1 and 7 months postradiation therapy, with a mean of 3 months. Measures of

"End to Side Technique": More organ preservation with less morbidity in patients with pyriform sinus apex SCC.

INTRODUCTION: Hypopharyngeal squamous cell carcinoma (SCC) is rare, but highly aggressive. Due to the advanced stage of this cancer at the time of diagnosis, radical surgery with reconstruction of pharynx is the standard care with high morbidity and mortality rate. A safer partial pharyngectomy could also be used for invasive hypopharyngeal cancer. In this study, we investigated the short and long-term outcomes in patients with SCC of the pyriform sinus apex undergoing standard partial pharyngectomy using a new suturing technique, called end to side technique. METHODS: This case series was pe

Oropharyngoesophageal Scintigraphy in a Case of Complex Swallowing Disorders After a Major Oral Surgery.

A 75-year-old woman had an occasional finding of a left tonsil mass for dysphagia, which resulted a high-grade squamous carcinoma. Therefore, the patient was sent to have a left pharyngectomy. After the pharyngectomy, the patient reported persistent swallowing disorders and nasal reflux. Consequently, she had an oropharyngoesophageal scintigraphy, demonstrating irregular oral and pharyngeal swallowing phases and confirming reflux episodes into the rhinopharynx and into the oropharynx. In line with these findings, the patient was send to rehabilitation; the abnormal functional mechanisms, previ

Morbidity after flap reconstruction of hypopharyngeal defects.

OBJECTIVES: Laryngopharyngeal reconstruction continues to challenge in terms of operative morbidity and optimal functional results. The primary aim of this study is to determine whether complications can be predicted on the basis of reconstruction in patients undergoing pharyngectomy for tumors involving the hypopharynx. In addition, we detail a reconstructive algorithm for management of partial and total laryngopharyngectomy defects. METHOD: A retrospective review was performed of 153 patients undergoing flap reconstruction for 85 partial and 68 circumferential pharyngectomies at a single in

Montgomery salivary bypass tube in the reconstruction of the hypopharynx. Cost-benefit study.

This study analyzes the postoperative complications and the functional results in 61 patients who underwent total laryngectomy with partial or total (circumferential) pharyngectomy reconstructed with a pectoralis major myocutaneous flap, in relation to the use of the Montgomery Salivary Bypass Tube (MSBPT). There were no significant differences regarding frequency of postoperative cervical complications in relation to the use of the MSBPT. The median hospital stay for patients without the MSBPT (36 days) was significantly higher than that for patients with the MSBPT (25 days). Although the MSB

[Functional surgery versus radical surgery in carcinoma of the hypopharynx].

A retrospective study was made of the clinical features and effectiveness of different surgical techniques in 66 patients with carcinoma of the hypopharynx treated at our hospital between 1985 and 1990. Thirty-seven patients were treated surgically and 29 were not. In the early stages, surgical treatment was functional: 15 patients had hemipharyngolaryngectomy or partial pharyngectomy. In advanced, but operable, stages, radical surgery was performed: 22 patients underwent total laryngectomy with partial pharyngectomy or total pharyngolaryngectomy. In 15 cases, reconstruction was needed; a musc

Treatment of end-stage pharyngeal strictures after laryngectomy with fasciocutaneous microvascular reconstruction.

Pharyngeal strictures can develop after laryngectomy for larynx cancer, particularly in combination with radiation. Some patients develop intractable strictures where conservative swallowing therapy and pharyngeal dilations fail. Secondary reconstruction of these patients has not been described in the literature. We report our experience with completion pharyngectomy and fasciocutaneous free tissue reconstruction for this patient population. Five subjects who had undergone laryngectomy and radiation were successfully reconstructed with fasciocutaneous free tissue transfer using the radial fore

One-stage reconstruction of the hypopharynx using myomucosal tongue flap and dermal graft.

Reconstruction of the hypopharynx following total laryngectomy and total pharyngectomy provides a one-stage procedure to reconstitute the food conduit. A myomucosal advancement tongue flap forms the anterolateral walls, and the dermal graft forms the posterior wall of the reconstruction. Sixteen patients underwent reconstruction by this method. Thirteen required total laryngectomy and total pharyngectomy for extensive carcinomas. Two other patients who had previous laryngectomy and hypopharyngeal recurrence had pharyngectomy and repair by tongue flap and dermal graft. A third patient required

Risk factors for dysphagia following total laryngectomy.

BACKGROUND: Dysphagia is a common and challenging long-term sequela following total laryngectomy (TL) that can negatively impact patient quality of life. Following TL, many patients require repeat esophageal dilations to maintain swallowing function. This study aimed to identify surgical and non-surgical risk factors associated with dysphagia following TL, defined as clinically diagnosed postoperative dysphagia based on patient-reported symptoms and dysphagia requiring dilation. METHODS: We conducted a retrospective single-institution cohort study of patients who underwent total laryngectomy

Reconstruction of hypopharyngeal defects in cancer surgery: do we have a choice?

Between 1982 and 1985, 83 patients with hypopharyngeal or extensive laryngeal cancers underwent pharyngoesophagectomy (48 patients), circumferential pharyngectomy (8 patients), and partial pharyngectomy (27 patients), depending on the site and extent of tumor involvement. The resultant hypopharyngeal defects were reconstructed with gastric transposition, a tubed pectoralis major myocutaneous flap, and a pectoralis major flap patch, respectively. There were three hospital deaths in the gastric transposition group (6 percent) and none in both pectoralis major flap groups. The leakage rate was 8.

[The reconstruction of the hypopharynx and the cervical esophagus (author's transl)].

Since 1973 16 reconstructions of the hypopharynx and cervical esophagus were performed. The restoration of swallowing was achieved in 7 cases after partial resection of the pharynx, 9 times after total laryngo-pharyngectomy and twice after laryngo-pharyngo-esophagectomy. The indication for surgery and criteria for the surgical methods to be applied are discussed. It is possible to combine the reconstruction of the pharynx with building up a neoglottis with Staffieri's technique for restoring voice function. As a one stage procedure to the hypopharynx and cervical esophagus after total laryngo-
