The principles and practical application of enteral nutrition.

The enteral route is the preferred method of nutritional support in patients with functional gastrointestinal tracts. Many techniques for obtaining enteral access are available, and the decision regarding which one to use depends on several issues, including the functional integrity of each part of the gastrointestinal tract, the duration of anticipated nutritional support, and the risk of aspiration and gastroesophageal reflux. Nasoesophageal tubes are useful for short-term supplementation; however, patients needing nutritional support for longer than 2 weeks may be better served with a more

The sniff-like aspiration reflex evoked by electrical stimulation of the nasopharynx.

Forced inspiratory efforts resembling the sniff-like aspiration reflex (AR) were evoked by single-shocks or trains of electrical impulses to the dorsal pharyngeal and nasal mucosal surfaces in 12 anaesthetized spontaneously breathing cats. The strongest ARs determined by diaphragmatic (DIA) EMG and peak-inspiratory flows were elicited from the dorsolateral nasopharyngeal wall close to the torus tubalis. Highly responsive sites were also detected in the posterolateral nasal cavity and in the rostrolateral oropharynx. Repetitive stimulation at threshold strength evoked ARs exclusively in the lat

Gastric emptying of a radionuclide-labeled test meal after surgical correction of gastric dilatation-volvulus in dogs.

Gastric emptying of a radionuclide-labeled test meal was studied in 10 dogs that had been treated surgically for gastric dilatation-volvulus and in 10 clinically normal (control) dogs. There were no significant differences between the gastric emptying rates and patterns in treated and in control dogs. Thus, there are no indications that gastric emptying is delayed in dogs that have recovered from gastric dilatation-volvulus, and there is no reason for pyloric surgery in dogs with this condition.

Use of a nasogastric tube for gastric and esophageal decompression in the dog and cat.

Distension of the stomach with air and fluid was treated successfully in 9 of 10 dogs by use of an indwelling nasogastric tube. A nasogastric tube was used to remove swallowed air and gastric fluid after surgery, as a precautionary measure to prevent recurrence of gastric distention in 2 dogs. A nasoesophageal tube was used to remove retained barium sulfate and saliva in a cat with megaesophagus and esophageal obstruction caused by gastroesophageal intussusception. Passage of the tube through the nose into the esophagus or stomach was easily accomplished in 10 of the 13 animals, requiring only

Airway management of major blunt tracheal and esophageal injury: A case report.

Trans-sectional injuries of trachea are quite rare and can be extremely challenging for anesthesiologists to deal with. About 25% of post-traumatic deaths are due to thoracic traumas in which blunt injuries take a rather small place within and the resultant damage of respiratory tract is quite rare with an incidence of 0.5-2%. A recent review from a single trauma center revealed an incidence of 0.4% for tracheobronchial injury (TBI) due to blunt thoracic injuries. Most of the patients having tracheal transection lose their lives on the field due to loss of airway. Patients mostly present with

[Lymphoepithelioma of the larynx].

Non-nasopharyngeal Lymphoepithelioma is a tumor of undifferentiated non-keratinizing cells enclosed in an lymphocytic stroma. The lymphoid tissue does not participate in the tumor process. Its tendency is to precociously spreading in the lymph nodes and viscera, but shows a good radiotherapeutic response. Unlike the nasopharyngeal lymphoepithelioma there is no etiological relationship with the Epstein-Barr virus, in spite of the histological similitudes, therefore the pathogenesis must be different in each. Discrimination between lymphoepithelioma and other neoplasms of the epithelial lineage

[Enteral nutrition in the elderly].

Elderly is particularly at risk of malnutrition: he is not able to feed himself adequately, it is then important to attain correct intakes using also artificial enteral nutritional techniques (nasogastric tube, gastrostomy, etc.). These techniques may lead to complications (ab ingestis pneumonia, metabolic complications, alvus disorders): the use of artificially nutrition in the elderly must be carefully evaluated. 257 patients (M = 180, F = 77) aged 65 or more, mainly affected by neoplastic diseases (n 195) and by neurological and vascular diseases (n 62). [Reconstruction of the gastrointestinal tract in the absence of the oesophagus, stomach and colon, with preservation of the larynx].

The authors report a case of reconstruction of the gastrointestinal tract in a young woman of 36 years of age who, after ingesting muriatic acid, and after a series of consecutive operations found herself without an oesophagus (from the hypopharynx) stomach and colon. She was fed with a jejunostomy and had a large pharyngostomy to prevent sialorrhoea. After one year, the patient manifested the need to resume eating by mouth. The problem was serious due both to the anastomosis above the level of the larynx and the very long period of rehabilitation. The technical problem included the need to av

[Surgical treatment of carcinoma of the hypopharynx and cervical esophagus: substitution by the colon and stomach].

Thirty-one cases with hypopharyngeal carcinoma and 4 cases with esophageal carcinoma in cervical segment were treated surgically from April 1984 to April 1989. In 6 cases, while preserving the larynx, the hypopharynx and the esophagus were resected and reestablished by the colon. In 29 cases, total laryngectomy was also performed and 4 had their hypopharynx and esophagus substituted by the colon and 25 by the stomach. According to UICC (1982), there were 11 cases in stage II, 15 in stage III and 9 in stage IV. The mean 3-year and 5-year survival rates were 62% and 32% respectively. In fatal ca

Assessment of obstruction level and selection of patients for obstructive sleep apnoea surgery: an evidence-based approach.

INTRODUCTION: Obstructive sleep apnoea has long been recognised as a clinical syndrome; however, high quality evidence on the effects of surgery for this condition is still missing. Despite this, a consensus seems to be evolving, albeit based on limited evidence, that surgery should be offered as a second line treatment to suitable patients with obstructive sleep apnoea. AIMS: This article aims to assess the different methods of investigating upper airway obstruction in patients with obstructive sleep apnoea, in respect to these methods' relevance to surgical treatment, via a systematic revie

Routes of early enteral nutrition following oesophagectomy.

Introduction Oesophagectomy for cancer is a challenging procedure with a five-year overall survival rate of 15-20%. Early enteral nutrition following oesophagectomy is a crucial component of the postoperative recovery and carries a significant impact on the outcome. Different methods of enteral feeding were conducted in our unit. The aim of this study was to examine the efficacy and safety of nasojejunal tube (NJT), jejunostomy tube (JT) and pharyngostomy tube (PT) feeding after oesophagectomy. Methods A retrospective review was carried out of prospectively collected data on patients with oeso

A new endoscopic technique for suspension of esophageal prosthesis for refractory caustic esophageal strictures.

There is no clear consensus concerning the best endoscopic treatment of benign refractory esophageal strictures due to caustic ingestion. Different procedures are currently used: frequent multiple dilations, retrievable self-expanding stent, nasogastric intubation and surgery. We describe a new technique to fix a suspended esophageal silicone prosthesis to the neck in benign esophageal strictures; this permits us to avoid the frequent risk of migration of the expandable metallic or plastic stents. Under general anesthesia a rigid esophagoscope was placed in the patient's hypopharynx. Using tra

Laryngoplasty: results and problems after the creation of a neoglottis.

Voice rehabilitation in the laryngectomy patient has been a major goal of reconstructive laryngeal surgery. 36 patients underwent laryngectomy with the creation of a phonatory neoglottis as outlined by Staffieri. In 13 patients good voice rehabilitation was achieved. In 23 patients who failed to develop voice production, altered swallowing function was the major problem.

Peptic ulcer-induced acute aortogastric fistula occurring 7 years after a pharyngogastrostomy following a resection for carcinoma of the esophagus: report of a case.

A 46-year-old woman underwent a pharyngogastrostomy, following a laryngoesophagectomy for esophageal carcinoma. Although she had been disease-free for 7 years, she subsequently was admitted to undergo a workup due to fever along with chest and back pain. A few days after admission, the patient suddenly vomited a large volume of blood and went into shock. Bleeding was stopped with a Sengstaken-Blakemore tube, and an emergency thoracotomy was performed. A fistula between the thoracic aorta and an ulcer of the gastric tube was identified. We decided to close the aortic lesion directly because the

Free jejunal diversionary conduit flaps following radiation damage to the pharynx: a technique for regaining oral feeding while preserving vocal function.

UNLABELLED: Radiotherapy is an accepted primary treatment modality for head and neck malignancies. However, in severe cases, the chronic radiation damage has resulted in dysphagia, aspiration and choking. Failure in conservative therapeutic strategies for this swallowing dysfunction will result in either preservation of voice with loss of oral feeding, or vice versa. We introduce our surgical technique based on the free jejunal diversionary conduit flaps, which helps patients to resume oral feeding and preserves vocal function, while reducing the risk of aspiration. METHOD: Six patients suffe

Free jejunal graft for esophageal reconstruction using end-to-side vascular anastomosis and extended pharyngo-jejunostomy.

BACKGROUND: Pharyngo-esophageal reconstruction using free jejunal grafts (FJGs) has been widely used, but the procedure is technically demanding and requires the involvement of multiple departments. We performed simplified reconstruction with FJGs using end-to-side vascular anastomosis and extended pharyngo-jejunostomy. METHODS: The jejunal artery and vein were anastomosed to the neck vessels in an end-to-side fashion without microvascular anastomosis. Pharyngo-jejunostomy with extended end-to-end anastomosis was performed to reduce size mismatch. We retrospectively analyzed the medical recor
