The use of phentolamine in the prevention of dopamine-induced tissue extravasation.

PURPOSE: The purpose of this study was to determine whether dopamine-induced tissue extravasation injury could be prevented with phentolamine. MATERIALS AND METHODS: This was a prospective, randomized, blinded, and controlled animal study. Forty rats were evaluated to document the effects of dopamine compared with normal saline on tissue integrity, whether any tissue damage was concentration or volume dependent, and to determine the minimum concentration of dopamine resulting in tissue injury. Dopamine concentrations of 0.8 mg/mL and 3.2 mg/mL were tested. In a second part of this study, an a

Intracavernous self-injection with phentolamine and papaverine for the treatment of impotence.

To evaluate the efficacy and safety of intracavernous self-injection of phentolamine and papaverine for the treatment of impotence, 30 patients were enrolled in a prospective, randomized, double-blind, placebo-controlled cross-over study of papaverine and phentolamine versus normal saline. A total of 29 patients completed the study. The phentolamine plus papaverine combination resulted in erection in 24 patients (82.8 per cent) and no erection occurred after injection of saline. Of the patients 12 (41.4 per cent) experienced technical difficulties with the injection. Ecchymosis of the penis at

Afterload reduction with phentolamine in patients with acute pulmonary edema.

Phentolamine in amounts of 10 to 40 microgram/kg/min was infused intravenously for the emergency treatment of acute pulmonary edema due to left ventricular failure. Fourteen patients with arteriosclerotic heart disease, ranging in age from 52 to 87 years, had clinical and roentgenographic signs of pulmonary edema. The pulmonary artery wedge pressure was increased to an average of 24 mm Hg and the cardiac index was decreased to 1.9 liters/min/m2 or less prior to the administration of phentolamine. A reduction in the pulmonary artery wedge pressure to 14 mm Hg and an increase in the cardiac inde

Intracavernosal injection of aviptadil and phentolamine for refractory erectile dysfunction.

BACKGROUND: Intracavernosal injection (ICI), most commonly with alprostadil, is recommended for Phosphodiesterase 5 Inhibitors (PDE5i) nonresponders in erectile dysfunction (ED) treatment; however, its use can cause pain (12%) and priapism (1%). AIM: We aimed to evaluate the efficacy of another ICI, Invicorp, and a combination of aviptadil with phentolamine mesylate (AvP), in contemporary practice. METHODS: We conducted a retrospective single-center analysis on patients referred for AvP because they developed intolerable Pain following Alprostadil ICI (group PA) or had Failed maximal dose (4

Chronic neurogenic facial pain: lack of response to intravenous phentolamine.

AIMS: Chronic neurogenic facial pain is commonly resistant to treatment and is often the source of significant patient morbidity. Adrenergic mechanisms are postulated to play a role in producing this type of pain, and adrenergic blocking agents are frequently used in clinical practice for pain control therapy. The analgesic effectiveness of an adrenergic blocking agent, intravenous phentolamine, was compared to saline and intravenous lidocaine in the present study using a single-blind protocol in patients with chronic neurogenic facial pain. METHODS: Thirty patients were studied whose common

Vasoactive intestinal polypeptide/phentolamine for intracavernosal injection in erectile dysfunction.

Erectile dysfunction (ED) is becoming an increasingly common problem and although oral therapies offer first-line treatment for many men, they are contraindicated or ineffective in substantial groups of patients. Intracavernosal injection (ICI) therapy is the most effective nonsurgical treatment for ED and offers an effective alternative to oral therapy. Sufficient arterial blood supply and a functional veno-occlusive mechanism are prerequisites in the attainment and maintenance of a functional erection. Invicorp (Plethora Solutions, London, UK) is a combination of vasoactive intestinal polype

Fibrosis of corpus cavernosum after intracavernous injection of phentolamine/papaverine.

Intracavernous self-injection with vasoactive drugs is a new treatment for impotence. Although few reports of side effects have been published complications such as priapism and fibrosis of the spongy tissue are of concern. We report a case of penile fibrosis after intracavernous self-injection of a combination of phentolamine and papaverine.

The reversal of the ischemic effects of epinephrine on a finger with local injections of phentolamine.

The use of phentolamine (Regitine) as a specific competitive antagonist of epinephrine by virtue of its alpha-blocking properties is well known. Its use via digital nerve block infiltration has been described in cases of IV infiltration of sympathomimetics. We discuss a recent case of local and direct infiltration of phentolamine along the course of the digital arteries and directly into the thumb tuft to reverse the local ischemic effects of epinephrine.

Intracavernous injections of papaverine and phentolamine: correlation with penile brachial index.

Eighty-two patients were evaluated for erectile failure with a comprehensive history, physical examination, hormonal testing, noninvasive Doppler examination (PBI), and nocturnal penile tumescence (NPT). After these studies all patients received intracavernous injection with a combination of papaverine (50 mg) and phentolamine (1.66 mg), and both tumescence and rigidity were monitored. Rigidity response was compared with the PBI. The number of patients with a poor rigidity response in each PBI category were: 3 of 5 with a PBI of less than 0.6 (vasculogenic), 7 of 11 with a PBI of 0.6-0.75 (amb

The effects of propranolol, phentolamine, and atropine on canine coronary vascular gradients.

The objective of this study was to measure pressure gradients in the coronary circulation following the administration of three receptor-blocking drugs, propranolol, phentolamine, and atropine when administered singly and in sequence. As well, we examined the responses of these gradients to eight interventions: left stellate ganglion or left vagosympathetic trunk stimulation, administration of isoproterenol, acetylcholine, noradrenaline, adenosine, phenylephrine, or adrenaline. Using a multiple linear regression model we examined the actions and interactions of the receptor-blocking agents on

The effect of propranolol phentolamine and pimozide on drug-induced anorexia in the mouse.

Pimozide was a potent antagonist of (+)amphetamine, diethylpropion, mazindol and phentermine anorexia in the mouse. Phentolamine and propranolol produced no such antagonism, but either potentiated or had no effect on the drug-induced anorexia. Although the mechanism of action of the four anorectic agents appears to involve dopamine receptor agonist activity, an antagonist or partial agonist effect at noradrenergic receptors may also be involved.

Effect of phentolamine on the hyperemic response to adenosine in patients with microvascular disease.

For accurate measurement of the fractional flow reserve (FFR) of the myocardium, the presence of maximum hyperemia is of paramount importance. It has been suggested that the hyperemic effect of the conventionally used hyperemic stimulus, adenosine, could be submaximal in patients who have microvascular dysfunction and that adding alpha-blocking agents could augment the hyperemic response in these patients. We studied the effect of the nonselective alpha-blocking agent phentolamine, which was administered in addition to adenosine after achieving hyperemia, in patients who had microvascular dise
