A kinome-wide CRISPR screen identifies CK1α as a target to overcome enzalutamide resistance of prostate cancer.

Enzalutamide (ENZA), a second-generation androgen receptor antagonist, has significantly increased progression-free and overall survival of patients with metastatic prostate cancer (PCa). However, resistance remains a prominent obstacle in treatment. Utilizing a kinome-wide CRISPR-Cas9 knockout screen, we identified casein kinase 1α (CK1α) as a therapeutic target to overcome ENZA resistance. Depletion or pharmacologic inhibition of CK1α enhanced ENZA efficacy in ENZA-resistant cells and patient-derived xenografts. Mechanistically, CK1α phosphorylates the serine residue S1270 and modulates the

The efficacy and safety of enzalutamide in metastatic hormone-sensitive prostate cancer: a meta-analysis based on subgroups and reconstructed individual patient data.

BACKGROUND: Metastatic hormone-sensitive prostate cancer (mHSPC) represents a critical stage in prostate cancer management. Two pivotal phase III clinical trials have demonstrated survival benefit with enzalutamide in mHSPC. This meta-analysis aims to evaluate the efficacy and safety of enzalutamide based on subgroup analyses and reconstructed individual patient-level data. METHODS: A comprehensive literature search was conducted using PubMed, Embase, Cochrane Library, and ClinicalTrials.gov. The primary outcome was overall survival (OS), while the secondary outcomes included prostate-specifi

[A Case of Castration-Resistant Prostate Cancer with Fatal Convulsive Seizure after Administration of Enzalutamide].

Enzalutamide, an androgen receptor antagonist, is a standard drug for the treatment of castrationresistant prostate cancer. A 77-year-old man developed a seizure after administration of enzalutamide. The patient presented with general fatigue and high fever approximately 5 weeks after oral administration of enzalutamide. Several days later, a seizure attack occurred at home, resulting in cardiopulmonary arrest. The patient was taken to the hospital emergency room but could not be resuscitated. We described a 63-year-old man who was diagnosed with clinical T1c prostate cancer, with a Gleason sc

[Intensification of radio-hormone therapy for prostate cancer].

Historically, prostate cancer radiotherapy relied on conventional fractionation of 2 Gy per session over several weeks to limit toxicity to healthy tissues. Technological advances in imaging and treatment precision have enabled the development of hypofractionated schedules, involving fewer sessions, while maintaining similar efficacy. Combining radiotherapy with hormone therapy improves outcomes by increasing tumor cell sensitivity to radiation. The addition of abiraterone or enzalutamide to the treatment plan for some very high-risk patients further improves survival rates. However, these tre

Severe neutropenia probably caused by enzalutamide and abiraterone in a prostate cancer patient.

INTRODUCTION: Abiraterone and enzalutamide are two androgen receptor pathway inhibitors approved, among others, for the treatment of metastatic castration-resistant prostate cancer in adult men whose disease has progressed on or after a docetaxel-based regimen. Although hematological effects, especially neutropenia, are one of the main complications of other oral antineoplastic drugs, these adverse effects are infrequent in the case of androgen receptor pathway inhibitors. CASE REPORT: We report the case of a patient diagnosed with metastatic castration-resistant prostate cancer who discontin

Enzalutamide Monotherapy in the EMBARK Trial Should Be Practice-changing and Existing Data Suggest How to Mitigate Toxicity.

Data from the EMBARK trial demonstrate the potential of enzalutamide monotherapy as an effective treatment for biochemically recurrent prostate cancer (BCR) without the side effects of testosterone-lowering therapy. Unfortunately, concerns about gynecomastia and how to manage this treatment-related toxicity have diminished enthusiasm. Existing data may offer an alternative scheduling strategy for enzalutamide monotherapy in BCR.

Neurocognitive impairment associated with traditional and novel androgen receptor signaling inhibitors ± androgen deprivation therapy: a pharmacovigilance study.

BACKGROUND: Conflicting evidence exists regarding whether hormone therapy for prostate cancer is associated with neurotoxicity. Thus, we aim to characterize the association between different types of hormone therapy and neurocognitive impairment in a real-world pharmacovigilance database. METHODS: We queried VigiBase, the World Health Organization's international pharmacovigilance database, for reports of neurocognitive impairment among men who took hormone therapy from 1968 to 2021. We performed disproportionality analysis comparing rates of neurocognitive impairment with different types of

Novel hormonal therapies in the management of advanced prostate cancer: extrapolating Asian findings to Southeast Asia.

There is a paucity of information on the use of novel hormonal agents in Southeast Asian patients. We reviewed the clinical roles of novel hormonal therapy (NHT), namely abiraterone acetate (AA), enzalutamide, apalutamide and darolutamide, in the management of advanced prostate cancer, and data on its use in Asian patients, in order to extrapolate these findings to the Southeast Asian patient population. There are some differences in the molecular features between the NHTs, which influenced their respective permeabilities through the blood-brain barrier. The Asian sub-analyses of the landmark

Cost-Utility Analysis of Darolutamide Combined with Androgen Deprivation Therapy for Patients with High-Risk Non-Metastatic Castration-Resistant Prostate Cancer in China.

INTRODUCTION: The increasing incidence of prostate cancer (PC) in China leads to a significant disease burden. Although three novel androgen inhibitors (darolutamide, apalutamide, and enzalutamide) have been approved for patients with high-risk non-metastatic castration-resistant prostate cancer (nmCRPC), the economic evaluation of these novel treatments in China remains unknown. In this study, we aimed to evaluate the cost-utility of darolutamide combined with androgen deprivation therapy (ADT), comparing with apalutamide + ADT and enzalutamide + ADT, in patients with high-risk nmCRPC from a

Effect of Enzalutamide on Morphine Exposure in Patients with Prostate Cancer.

BACKGROUND AND OBJECTIVE: Pain management in patients with prostate cancer receiving enzalutamide is challenging owing to its high potential for drug-drug interactions. Morphine is generally preferred because of its favorable metabolic profile, but the effect of enzalutamide on the pharmacokinetics of morphine is unclear. The objective of this study was to assess whether a drug-drug interaction exists between enzalutamide and morphine in patients with prostate cancer. METHODS: In a multicenter two-arm parallel study, 24 men with prostate cancer received morphine with enzalutamide (n = 12) and

End-of-life Androgen Deprivation Syndrome With Enzalutamide in Metastatic Prostate Cancer: A Case Report.

BACKGROUND/AIM: Enzalutamide is a highly effective anti-androgen for the treatment of prostate cancer. Therapy can incur unbearable fatigue in elderly patients, which might require a reduced dosage. However, the value of reduced-dose treatment to improve the long-term tolerability and to decrease the risk of toxicity remains unknown. CASE REPORT: We present the six-year clinical course of a patient treated upfront with low-dose enzalutamide (25% of the standard dose). Despite initial disease control and tolerance, the patient developed progressive frailty and signs of severe androgen deprivat
