This paper presents in concise form elements of psychotherapeutic treatment that the author views as helpful in working with families of children exhibiting severe school phobia. The article consists of a list of pragmatic suggestions and proposals that might aid the therapist in better organizing treatment of a problem that usually involves remarkable complexities.

As part of a survey of agoraphobic women in the general population, it was found that school phobia occurred in their children to a greater extent than would be expected. The average incidence in children aged 11 to 15 was 14 per cent. The women with school phobic children had had school phobia themselves to an undue extent.

The study provides an overview of current cognitive-behavioral interventions to treat school-refusal in children and adolescents. First of all we define the problem area and describe the standard of knowledge about prevalence, course and risk factors. The main focus of our work is presented by psychotherapeutic treatments of school phobia and school anxiety. Apart from general treatment principles we give a survey of the most important behavioral therapeutic as well as cognitive-behavioral therapeutic treatments. In addition, we provide a detailed description of current research about efficacy

BACKGROUND: Children and adolescents with school phobia sometimes complain of severe and persistent headaches that are diagnosed as chronic daily headache (CDH).
METHODS: We investigated 24 children with CDH and school phobia, and 26 children with CDH but without school phobia.
RESULTS: Of 24 children with CDH and school phobia, 4% had chronic migraine (CM), 46% had chronic tension-type headache (CTTH) and 50% had both CTTH and migraine. However, of 26 children with CDH but without school phobia, 61% had CM, 24% had CTTH, 11% had CTTH and migraine, and 4% had new daily-persistent headache. T

Chronic fatigue occurring in previously healthy children and adolescents is a vexing problem encountered by pediatric practitioners and the impact of fatigue in youngsters should not be underestimated. In its severe form, it is often associated with mood disorders. Findings in children and adolescent cases suggest that severe unexplained fatigue might precede the development of fatigue-related illness, such as childhood chronic fatigue syndrome (CCFS). This is a disabling condition characterized by severe disabling fatigue and a combination of symptoms, the prominent features being self-report

Epidemiological data indicate that, based on current diagnostic criteria, anxiety disorders are the most common childhood disorders. Furthermore, the comorbidity rate among the various diagnostic categories is quite high, and relatively little attention has been given to delineating the specific and distinct parameters of these disorders. The current study examined the characteristics of overanxious disorder and social phobia by comparing children who have these disorders to matched normal controls. The results indicated that children with social phobia could be differentiated from the other g

Resulting from a shortage of possibilities in the ambulant treatment of school phobia behavior-therapeutic interventions were established at a psychological counseling center for families twenty years ago, which have been in existence to this day. The criteria of anxiety-based absenteeism as well as problems of terminology and classification will be presented with emphasis on school phobia as a combination of separation anxiety and social anxiety ("Schulphobie"). The multimodal treatment focuses on cognitive interventions, graduated exposition and close cooperation with teachers. The counselor

Thirty-two school-phobic youngsters of mean age 13 years were randomly allocated to two treatment groups after stratifying for sex. Length of in-patient management in a psychiatric hospital unit for young adolescents was evaluated. One group was admitted for three months and the other for six. Care was taken to ensure that cases had been fairly distributed between the groups by comparing them on a variety of clinical features. Follow-up assessments were carried out in a reliable fashion at six months, one year and two years after discharge. Outcome, overall, was similar to that found in a prev

Debbie, a 14-year-old girl, was referred one year ago to a rural mental health center by her parents, essentially due to continued refusal to attend school. It was readily apparent that the symptoms were those of a classic case of school phobia, i.e., refusal to attend school, nausea and/or vomiting on school days but not on weekends, the overprotective mother, the distraught family, and other rather predictable concomitants. Following careful professional assessment, the assignment of a paraprofessional was made (a senior elementary education major taking part in a field experience course in

BACKGROUND: This study explored school personnel's perceptions of school refusal, as it has been described as a "common educational and public health problem" that is less tolerated due to increasing awareness of the potential socioeconomic consequences of this phenomenon.
METHODS: In-depth interviews were conducted with school personnel at the middle school (N = 42), high school (N = 40), and district levels (N = 10). The findings focus on emergent themes from interviews with school health personnel (N = 12), particularly those themes related to their perceptions of and role in working with

The development of school phobia has been reported in some patients with Tourette syndrome treated with haloperidol. The author reports on a patient who developed school phobia on each of three trials with pimozide, a neuroleptic chemically similar to haloperidol, and proposes the term "neuroleptic separation anxiety syndrome." The syndrome is clinically indistinguishable from DSM-III criteria for school phobia (separation anxiety disorder). Tricyclic antidepressants may have a therapeutic or prophylactic effect. It is not known whether this neuroleptic side effect occurs in patients other tha

Panic Disorder (PD) is a common anxiety disorder, which has its onset relatively often during adolescence. Twenty-five percent of adult patients with PD have previously suffered from school phobia. In young patients it often represents a form of agoraphobia, although it may be present also in other psychiatric disorders which have their onset in young age. In this report we describe the results of 8 to 15-month citalopram treatment on three young patients with school phobia associated with PD. In our patients, low doses with citalopram were effective as in all patients the severity of school p

Fifteen patients with Tourette's disorder developed school and work avoidance syndromes when treated with low doses (mean 2.5 mg/day) of haloperidol for short periods of time (mean, 8 weeks). The phobic syndromes disappeared completely with discontinuation or reduction of the haloperidol dose. Haloperidol's effects on dopaminergic functioning support a role for catecholamines in the pathogenesis of phobic syndromes. It is not known whether phobias are precipitated by haloperidol only in patients with Tourette's disorder as a consequence of the specific metabolic alterations in this disorder or

The authors developed reliable clinical rating scales to compare 35 children with school phobia and their families with a matched sample of children with other neuroses and their families. Twice as many school phobic children as children with other neuroses showed excessive separation anxiety, dependency, and depression. Although a mutually hostile-dependent interaction was found in most of the families of children with school phobia, the development of school phobia appeared to be dependent on defects in character development in the children as well. The authors discuss the etiological signif

Psychodynamic understanding and systemic approach in the multi-family treatment of a day care clinic are illustrated following the case report of a seven-year old girl with school phobia due to separation anxiety. The treatment modalities of the day clinic at the University Medical Center Muenster are described focussing on the multi-systemic approach. Using psychodynamic and systemic hypotheses the process of treatment is developed. Specific interventions, differentiated into reorganizations of inner and outer world issues, are traced to psychodynamic and systemic hypotheses. In conclusion it

BACKGROUND: Research on depression has identified hyperactivity of the HPA axis as a potential contributory factor to the intergenerational transmission of affective symptoms. This has not yet been examined in the context of social phobia. The current study compared HPA axis activity in response to a universal social stressor (starting school) in children of 2 groups of women: one with social phobia and one with no history of anxiety (comparison group). To determine specificity of effects of maternal social phobia, a third group of children were also examined whose mothers had generalised anxi
