An investigation of Caesarean sections in three Greek hospitals: the impact of financial incentives and convenience.

BACKGROUND: Caesarean section (CS) rates have been increasing dramatically in the past decades around the world. The objective of our study was to investigate the factors increasing the likelihood of undergoing CS in two public hospitals and one private hospital in Athens, Greece. Specifically, the purpose was primarily to assess the impact of non-medical factors such as private health insurance, potential for making informal payments, physician convenience and socio-economic status on the rate of CS deliveries. METHODS: All available demographic, socio-economic and medical information from t

The impact of physician bonuses, enhanced fees, and feedback on childhood immunization coverage rates.

OBJECTIVES: The purpose of this study was to examine the effects on immunization coverage of 3 incentives for physicians--a cash bonus for practice--wide increases, enhanced fee for service, and feedback. METHODS: Incentives were applied at 4-month intervals over 1 year among 60 inner-city office-based pediatricians. At each interval, charts of 50 randomly selected children between 3 and 35 months of age were reviewed per physician. RESULTS: The percentage of children who were up to date for diphtheria, tetanus, and pertussis and Haemophilus influenzae type b; polio; and measles-mumps-rubell

Identification of recruitment and retention strategies for rehabilitation professionals in Ontario, Canada: results from expert panels.

BACKGROUND: Demand for rehabilitation services is expected to increase due to factors such as an aging population, workforce pressures, rise in chronic and complex multi-system disorders, advances in technology, and changes in interprofessional health service delivery models. However, health human resource (HHR) strategies for Canadian rehabilitation professionals are lagging behind other professional groups such as physicians and nurses. The objectives of this study were: 1) to identify recruitment and retention strategies of rehabilitation professionals including occupational therapists, phy

Compensating Heart Failure Cardiologists: Is the RVU Model the Right Model?

Physician compensation models in the United States are diverse, shaped by practice settings, specialties, and institutional factors. A notable shift in academic medical centers has been the transition from salary-based compensation to relative value unit (RVU)-based models. While this shift may align compensation with clinical productivity and facilitate budgetary planning, it has profound implications for physician satisfaction, burnout, and clinical practice, particularly in primarily cognitive/nonprocedural based specialties like advanced heart failure (AHF). This article explores the benef

When the scribe does the reasoning: ambient artificial intelligence, inference impersonation, and the development of trainees' clinical judgment.

Ambient artificial intelligence (AI) scribes are entering clinical practice on aligned incentives, with physicians gaining relief from documentation burden and health systems gaining revenue through more complete coding. Yet adoption has outpaced evaluation. This paper identifies a specific phenomenon, which it calls inference impersonation. In the Assessment and Plan, ambient AI scribes do not transcribe clinical reasoning but rather generate it. The resulting text mimics physician judgment without the iterative hypothesis testing, contextual weighing, and management reasoning that define cli

Performance scores in general practice: a comparison between the clinical versus medication-based approach to identify target populations.

CONTEXT: From one country to another, the pay-for-performance mechanisms differ on one significant point: the identification of target populations, that is, populations which serve as a basis for calculating the indicators. The aim of this study was to compare clinical versus medication-based identification of populations of patients with diabetes and hypertension over the age of 50 (for men) or 60 (for women), and any consequences this may have on the calculation of P4P indicators. METHODS: A comparative, retrospective, observational study was carried out with clinical and prescription data

Patient-centered medical home: developing, expanding, and sustaining a role for pharmacists.

PURPOSE: The development of a patient-centered medical home (PCMH) health care model and the role of pharmacists in PCMHs at the University of Michigan are described. SUMMARY: In 2009, Blue Cross Blue Shield of Michigan (BCBSM) provided financial incentives to physician groups to implement PCMH principles. A partnership was formed among the department of pharmacy, college of pharmacy, and faculty group practice at the University of Michigan Health System (UMHS) to integrate clinical pharmacists into the PCMH model at eight general medicine practices. The rationale was that PCMH pharmacists co

Improving the rate of use of fascia iliaca compartment blocks in patients presenting with hip fractures.

BACKGROUND: Patients presenting to the emergency department (ED) with hip fractures are typically treated with opioids, which are associated with adverse events such as delirium and respiratory depression. The fascia iliaca compartment block (hereafter fascia iliaca block) is a regional analgesia technique which avoids these negative outcomes. We sought to increase the rate of use of this technique to 50% of all patients with hip fractures who presented to our EDs within an 18-month period. METHODS: We held three Plan-Do-Study-Act cycles designed in accordance with surveys sent to our physici

Veterans Affairs Graduate Medical Education Expansion Addresses U.S. Physician Workforce Needs.

The United States has a well-trained, highly specialized physician workforce yet continues to have care gaps across the nation. Deficiencies in primary care and mental health specialties are most frequently cited, though critical shortages in multiple disciplines exist, particularly in rural areas. Sponsoring institutions of physician graduate medical education (GME) have created rural residency tracks with modest federal funding and minimal incentives, though efforts targeting shortages in these specialties and geographic locations have been limited. In response to access problems in the Vete

[Analysis--of family-planning in urban low-income brackets based on Daejon and Daegu Cities].

Since the inception of the national family planning program in 1962, the main emphasis has been placed on the provision of contraceptive services and information, education, and communication (IEC) activities through home visits by family planning field workers and through government designated private physicians. This approach has been a powerful management system for the national program, particularly in the rural area. However, new issues in the national family planning program emerged with the subsequent socioeconomic changes since 1962. An increasing number of the rural population began t

Markers for primary care: missed opportunities to immunize and screen for lead and tuberculosis by private physicians serving large numbers of inner-city Medicaid-eligible children.

OBJECTIVE: This study examines coverage levels for immunization, missed opportunities to immunize, and extent of lead and tuberculosis screening in inner-city storefront physician offices and then relates child, visit, and physician characteristics to missed opportunities. METHODOLOGY: With the use of a nested sampling strategy, 232 charts were selected for review in 31 physicians' offices. Charts selected were for children 0 to 35 months of age who had three or more visits in more than 3 months. Physicians were selected from those in specific low-income New York inner-city neighborhoods who

Perioperative respiratory rehabilitation: evolving concepts and clinical applications.

Postoperative respiratory rehabilitation is a comprehensive program designed to enhance both the physical and psychological states of patients with respiratory conditions. Various breathing techniques such as diaphragmatic breathing, pursed-lip breathing, coughing and huffing techniques, and the active cycle of breathing technique aid significantly in mucus clearance and improvement of respiratory function. Patient-specific posture training and exercises aimed at improving chest wall mobility can significantly reduce atelectasis and improve ventilatory parameters. Preoperative training with in

Policy Pathways to Address Provider Workforce Barriers to Buprenorphine Treatment.

UNLABELLED: At least 2.3 million people in the U.S. have an opioid use disorder, less than 40% of whom receive evidence-based treatment. Buprenorphine used as part of medication-assisted treatment has high potential to address this gap because of its approval for use in non-specialty outpatient settings, effectiveness at promoting abstinence, and cost effectiveness. However, less than 4% of licensed physicians are approved to prescribe buprenorphine for opioid use disorder, and approximately 47% of counties lack a buprenorphine-waivered physician. Existing policies contribute to workforce barr

Project ECHO® dermatology in the deep south: a comprehensive SWOT assessment.

There are nearly 45,000 patients for each dermatologist in the rural State of Mississippi, causing many patients to wait months for an appointment in a city that may be hours away [1]. To address this critical gap, Project ECHO® (Extension for Community Healthcare Outcomes) was launched at the University of Mississippi Medical Center (UMMC) in 2020 to teach rural primary care providers (PCPs) to care for common skin conditions. PCPs present images of their patients in a once monthly video teleconference that provides immediate feedback. A SWOT analysis (Strengths, Weaknesses, Opportunities, an

The hepato-thyroid axis in male obesity: unraveling inter-organ crosstalk through non-invasive biomarkers.

INTRODUCTION: The shortage of physicians in rural areas of the USA has led medical schools to focus on recruiting and selecting students who will choose to work in non-urban settings. The purpose of this study was to examine the effect of student characteristics previously correlated to choosing rural practice (ie being older, being male, being raised in a rural community, having a spouse or significant other who was raised in a rural community, having a spouse or significant other whose parents live in a rural community, having parents with high school education or less, and graduating from a

How does hospitalization affect continuity of drug therapy: an exploratory study.

INTRODUCTION: Transitions between different levels of health care, such as hospital admission and discharge, pose a significant threat to the quality and continuity of medication therapy. This study aims to explore the role of hospitalization on medication changes as patients are transferred from and back to ambulatory care. METHODS: Secondary analysis of claims data from Swiss residents with basic health insurance at the Helsana Group was performed. We evaluated medication invoices of patients who were hospitalized in a Swiss private hospital group in the year 2013. Medication changes were d

Development and implementation of a treatment pathway to reduce coronary angiograms - lessons from a failure.

BACKGROUND: The rates of coronary angiograms (CA) and related procedures (percutaneous intervention [PCI]) are significantly higher in Germany than in other Organisation for Economic Co-ordination and Development (OECD) countries. The current guidelines recommend non-invasive diagnosis of coronary heart disease (CHD); CA should only have a limited role in choosing the appropriate revascularisation procedure. The aim of the present study was to explore whether improvements in guideline adherence can be achieved through the implementation of regional treatment pathways. We chose four regions of
