Pediatric Asthma for the Primary Care Physician.

In the pediatric population, asthma is the most common chronic disease. Asthma is a chronic inflammatory disease consisting of variable respiratory symptoms and airflow limitation. Proper and timely diagnosis remains of utmost importance. Early diagnosis allows for earlier treatment and subsequent reduction of morbidity and mortality. Newer research and medications have changed the treatment paradigm, including the addition of biologic agents for more severe cases and use of inhaled corticosteroid-formoterol inhaler as a rescue treatment.

Addiction medicine for the primary care physician.

Addiction and the consequences including medical, social, and legal are seen almost daily in a primary care office. To effectively deal with these problems it is imperative that the physician view addiction as a disease. This article explores the disease concept and the basis for the view as well as providing a guide to diagnosis and assessment. Useful screening tools are also provided as well as a discussion of biochemical markers currently available. A guideline for delivering the diagnosis is also discussed. An overview of the stages of treatment is provided as well, with particular emphasi

Echocardiography. Basics for the primary care physician.

Echocardiography is commonly performed to assess aortic stenosis, mitral valve prolapse, pericardial effusion and cardiac tamponade, myocardial infarction, endocarditis, and valvular regurgitation. We have found that the more defined and appropriate a request for an echocardiogram is, the better the technician and interpreter can relate observations to the requesting physician. Thus, thorough understanding of the indications and limitations of echocardiography by the primary care physician will result in improved patient care.

Learing disabilities and the primary care physician.

Approximately 10% of the population has learning disabilities (LD). Although the main manifestations occur in childhood, many of the primary and secondary manifestations of LD can continue into adult life. The high prevalence of LD and the current economic climate in Canada imply that the primary care physician must have a role in the identification, diagnosis, and management services for persons with LD. Information about the specific aspects of a particular person's LD should be incorporated into the evaluation and management of other health matters with which the primary care physician deal

Approach to nail trauma for primary care physicians.

OBJECTIVE: To provide an overview and approach to common nail bed injuries seen by primary care practitioners. SOURCES OF INFORMATION: An Ovid MEDLINE literature search was performed using search terms and studies were graded based on level of evidence. MAIN MESSAGE: Nail trauma is common in primary care practice and requires proper and prompt treatment to avoid lasting effects on finger function and cosmesis. When presented with a fingernail injury, primary care physicians should perform a thorough physical examination to determine extent of injury; take a history to rule out notable risk f

Evidence-based pain medicine for primary care physicians.

The last several decades have seen a marked increase in both the recognition and treatment of chronic pain. Unfortunately, patients frequently misunderstand both the nature of pain and the best practices for its treatment. Because primary care physicians treat the majority of chronic pain, they are ideally situated to provide evidence-based pain care. The majority of the medical evidence supports a biopsychosocial model of pain that integrates physical, emotional, social, and cultural variables. The goal of this primer is to assist primary care physicians in their understanding of pain, evalua

Primary care physicians and their HIV prevention practices.

A national random-sample survey of 4011 primary care physicians was conducted to determine the extent to which they are providing HIV prevention and clinical services, and to learn what characteristics and attitudes might impede the provision of such services. Physicians were asked about their history-taking practices for new adult and adolescent patients, including asking about the use of illicit drugs (injection and noninjection), the number of sexual partners, use of condoms and contraceptives, past episodes of sexually transmitted diseases (STDs), sexual orientation, and sexual contact wit

Early detection of depression by primary care physicians.

The overall prevalence of depression is from 3.5% to 27%. The burden of suffering is high and includes death through suicide. In most cases treatment is effective, but important episodes of depression are being missed. To determine whether a brief, systematic assessment for the early detection of depression should be part of the periodic health examination we searched MEDLINE and the Science Citation Index for randomized controlled trials that evaluated the effectiveness of early detection of depression with a questionnaire. Seven instruments met our quality criteria; the Beck Depression Inven

AIDS-related competence of California's primary care physicians.

We conducted a telephone survey of a random sample of primary care physicians practicing throughout the State of California to determine their AIDS-related experiences and competencies. Interviews were completed early in 1986 with 1,000 family and general practitioners and internists, 60 per cent of those eligible. Data on practice experiences reflect the increased incidence of AIDS (acquired immunodeficiency syndrome) in Los Angeles and San Francisco. The proportion of practitioners in rural areas who evaluated possible cases (17 per cent), and counseled patients at risk (50 per cent) indicat

Leukemia: What Primary Care Physicians Need to Know.

Leukemia is caused by an abnormal proliferation of hematopoietic stem cells in the bone marrow. The four general subtypes are acute lymphoblastic, acute myelogenous, chronic lymphocytic, and chronic myelogenous. Acute lymphoblastic leukemia primarily occurs in children, whereas the other subtypes are more common in adults. Risk factors include certain chemical and ionizing radiation exposures and genetic disorders. Common symptoms include fever, fatigue, weight loss, joint pain, and easy bruising or bleeding. Diagnosis is confirmed with bone marrow biopsy or peripheral blood smear. Hematology-

Gout: what primary care physicians want to know.

Gout is an inflammatory arthritis that has been recognized since ancient times. It is the most common type of inflammatory arthritis in men. Despite these facts, no new advances have been made in the diagnosis and treatment of gout for over 50 years. Since there have been no new advances and the condition has been recognized for so many centuries, the diagnosis and treatment of gout has not elicited interest in learning more about the disease. Thus, managing gout is a challenge for the clinician caring for the gout patient. Rheumatologists should be the educators of primary care physicians (PC

Screening for childhood strabismus by primary care physicians.

OBJECTIVE: To review the clinical classification of strabismus, to describe the timing and method of strabismus screening examinations, and to discuss the principles of treatment. QUALITY OF EVIDENCE: Current literature (1983 to 1995) was searched via MEDLINE using the MeSH headings strabismus, ocular motility disorders, and amblyopia. Articles were selected based on their date of publication, clinical relevance, and availability. Preference was given to more recent articles, articles with large numbers of subjects, and well-designed cohort studies. Official recommendations from academic grou

Anaerobic infections. The basics for primary care physicians.

Anaerobic bacteria constitute a major portion of the normal human microflora, and some of them can cause disease in contiguous body parts, especially if there is a mucosal break. Most anaerobic infections are polymicrobial. Because anaerobes are difficult to culture, diagnosis is often made on the basis of clinical clues. Thus, knowledge of the common sites, predisposing conditions, and other representative features of anaerobic infections is critical. For anaerobic infections above the diaphragm, where Bacteroides fragilis is not a common isolate, high-dose penicillin G therapy is usually suf

Correlation of leadership and care coordinator performance among primary care physicians.

PURPOSE: Primary care physicians have to deal with many aspects of the patients' health problem, which needs cooperation with other health professionals or even nonhealth individuals. To achieve effective results, the primary care physicians should have leadership and coordinating skills, especially when dealing with the health challenges in Asia Pacific region. The care coordinator role of primary care physicians is important to create the bridge between population and health. This study aims to determine the correlation between care coordinator performance and leadership factors among primar

Best practices in LGBT care: A guide for primary care physicians.

Lesbian, gay, bisexual, and transgender (LGBT) people have unique healthcare needs. While all LGBT people are at an increased risk for mental health and substance abuse disorders, certain health conditions vary by group. Overweight and obesity are more common in lesbian and bisexual women, whereas sexual minority men are at increased risk for infections such as human immunodeficiency virus (HIV) and syphilis. Breast, cervical, and prostate cancer screening should be offered to all LGBT individuals according to national guidelines and with consideration of a transgender person's natal and surgi

Guidelines for primary aldosteronism: uptake by primary care physicians in Europe.

CONTEXT: Primary aldosteronism is the most frequent form of secondary hypertension but is still markedly underdiagnosed. In 2008, the Endocrine Society released guidelines for primary aldosteronism diagnosis and management; current indications are that they are not being widely followed. OBJECTIVE: To investigate the level of knowledge and application of the guidelines in a large representative cohort of general practitioners (GPs) in Italy and Germany. SETTING AND DESIGN: The study was carried out by web questionnaire on hypertension and primary aldosteronism management with 500 GPs (250 in
