Non-traumatic wrist pain remains a diagnostic challenge. An accurate diagnosis is crucial in order to choose the appropriate treatment. We report the case of a 23-year-old female with a four-month history of mainly nocturnal wrist pain. There was no history of trauma or prior surgery. Radiographs and CT scans showed a lytic lesion with central nidus and sclerotic margins in the pisiform bone. Bone scan showed increased uptake in the pisiform bone. The diagnosis of osteoid osteoma was confirmed by histopathological analysis after complete surgical resection. Osteoid osteoma of the carpal bones

The pisiform bone dislocated in a 56-year-old worker who had a crush injury of his wrist. Open reduction and reconstruction of the ligaments resulted in proximal subluxation of the pisiform bone and post-traumatic arthritic changes. Other authors recommend that excision of the pisiform is considered to be a more appropriate method of treatment.

Proximal row carpectomy (PRC) is an established surgical procedure used to treat post-traumatic osteoarthritis of the wrist with sparing of the midcarpal joint and advanced aseptic necrosis such as lunatomalacia. Proximalization of the distal carpal row following PRC may lead to secondary problems such as radiocarpal impingement. At follow-up, two of our patients complained about ulnar-sided wrist pain after proximal row carpectomy. Computed tomography (CT) scans were taken for both patients with an additional magnetic resonance imaging scan for one patient. The CT scan revealed clear osteolys

Simultaneous fracture and dislocation of the pisiform is a rare condition, and only two cases have been reported in children. We retrospectively collected and reviewed clinical and radiographic data of a child with a type II Salter-Harris fracture of the distal radius, associated with fracture dislocation of the pisiform. In addition, we performed a systematic review of the literature available to date.

We present the case of a 14-year-old patient who suffered fracture dislocation of the pisiform bone (PB) along with fractures of the scaphoid, proximal radius, and proximal phalanx of the thumb due to high-energy trauma directly to the extended wrist. This combination of fractures has not been previously reported in the literature. Currently, there is no consensus in the literature regarding the optimal treatment approach for such cases. In our management, initial attempts at closed and open reduction were unsuccessful, leading to the decision for primary pisiformectomy. Our report includes a

The author examined the carpalia of 40 male alcoholics suffering from Stage I of Dupuytren's contracture. In four subjects he observed a cortical hyperostosis of the right pisiform bone on the tangential films. The hyperostosis can be ascribed to the ethanol-induced hyperlipoproteinemia type II/B. The lipoids leaving the vascular paths infiltrate the connective tissue capsule of the pisiform bone and stimulate the formation of osteoblasts at the border between bone and soft tissue. The ossification must be regarded as enteropathic hyperostosis.

Aneurysmal bone cyst (ABC) is a benign expansile bone tumor without metastasis capability. Only 3-4% of ABCs occur in the hand and they mainly take place in metaphysis' of long bones like metacarpals or phalanges. Carpal ABCs have been reported as individual case reports in the literature due to rarity. A patient presented with pain in her right wrist. Magnetic resonance imaging revealed a well circumscribed one cm sized mass in the pisiform bone that resembled an aneurysmal bone cyst. Total pisiformectomy was performed. Treatment options are total excision or curettaging in ABCs. But rarity o

We describe compression of the ulnar nerve at Guyon's canal caused by a hypermobile pisiform bone and associated with spasm of the ulnar artery. Treatment included excision of the pisiform bone, and repair of the flexor carpi ulnaris, hypothenar musculature, and periosteum. Postoperatively, the patient reported complete relief of symptoms, which had still been maintained at final follow-up one year later.

INTRODUCTION: Arteriovenous malformations (AVMs) are rare congenital vascular abnormalities involving direct connections between arteries and veins, bypassing capillaries. While they can occur throughout the body, AVMs in the forearm are uncommon, representing a diagnostic and therapeutic challenge when they produce symptoms, such as neurovascular compressions, such as unspecified swelling or mass, throbbing pain, localized or radiating tingling, and numbness. These lesions often remain asymptomatic until adolescence, when growth spurts and hormonal changes can trigger enlargement and clinical

Few cases in which open reduction and internal fixation was performed for displaced pisiform fractures have been reported. We present a new surgical technique for the treatment of depressed intra-articular pisiform fractures. First, the depressed fragment was reduced by pushing the bone tamp. Then, the fracture void resulting from the reduction of the depressed fragment was filled with a shaped hydroxyapatite block. Finally, the fragments were sutured using braided polyblend polyethylene sutures. The postoperative radiography could achieve a well-reduced articular facet, and this procedure had

Although good clinical results have been reported following lunate resection and vascularized os pisiform transfer for advanced stages of Kienböck's disease, the occurrence of wrist osteoarthritis has been highlighted. We aimed to investigate the postoperative condition of the pisiform and the surrounding bones in patients who underwent surgery for advanced stages of Kienböck's disease using magnetic resonance imaging. We retrospectively reviewed the data of six patients (mean age, 45 years; Lichtman stage, IIIb) who underwent lunate resection and vascularized os pisiform transfer for advanced

Thirty-three patients with pisiform area pain have been treated. Of these, the pisiform bone was excised in 21 patients who had intractable pisiform bone pain that was not relieved by the conservative methods of immobilization, local steroid injection, and anti-inflammatory medication. The indications for excision were painful union or nonunion of pisiform fractures, pisiform-triquetrial arthritis, and tendonitis of the flexor carpi ulnaris (FCU) insertion. Wrist strength and mobility was maintained by doing a subperiosteal dissection and removal of the pisiform bone. This technique preserves

Four cases of pisiform fracture are presented. In one case the largest fragment of the pisiform bone with the articular surface was dislocated. After open reduction, osteosynthesis with K-wires was performed. Two patients had an isolated fracture without fragment dislocation. They had their lower arms immobilized with a plaster of Paris cast for 4 weeks. In the fourth case the fracture of the pisiform bone was associated with an intraarticular distal radial fracture and wrist dislocation. The typical traumatic cause of pisiform fracture is a fall onto the outstretched hand. Immobilization for

There are reported three cases of nodular calcifications of the ulnocarpal region with very similar clinical and radiological aspects. The isolated nodules or the double, contigue one, are located near the pisiform bone, proximal to it, in the obliqual palmo-dorsal projection with semisupine hand. These nodules are projected on the ulnar styloid with the dorso-palmar projection. Clinical-radiological characteristics suggest a diagnosis of chronic bursitis that corresponds with the insertion on the pisiform bone of the flexor carpi ulnaris tendon. Differential diagnosis faces, besides the calci

Fractures of the pisiform bone are difficult to diagnose due to the often unsuspicious conventional X-ray images and nonspecific clinical presentation. The following case presents a 29-year-old patient who suffered a bicycle fall onto their right hand and experienced prolonged symptoms despite the initial lack of evidence of a fracture. The fractured pisiform bone, which was eventually detected in a CT scan, was successfully treated with a 4-week period of cast immobilization. The existing conservative and surgical treatment options of a pisiform bone fracture are discussed.

Scaphoid fracture can evolve into scaphoid nonunion leading to wrist arthritis. Vascularized bone flaps used to treat scaphoid nonunion are supplied by delicate, small, or short arteries that are not always reliable. The pisiform bone has never been considered as a possible treatment of scaphoid nonunion since the traditionally harvested pedicle is too short. This study aimed to characterize the vascularization of the pisiform with the goal of developing a method of harvesting it with a longer pedicle that can be used as a graft to treat scaphoid nonunion. A cadaver study on 30 upper limbs was

Isolated pisiform dislocation is an uncommon condition, with a limited number of cases reported in the literature. We present a unique case of a 15-year-old male who experienced an atraumatic isolated dislocation of the pisiform bone in his left wrist and presented with pain, deformity, and ulnar nerve palsy in his little and ring fingers. Radiographic investigations confirmed the diagnosis of isolated pisiform dislocation, and the patient successfully underwent an open reduction, stabilization of pisiform, and exploration of the ulnar nerve.

A 31-year-old woman had a seizure episode three weeks before, leading to a fall on her left hand. Following the accident, she had discomfort on the ulnar side of her left wrist. She decided to seek treatment from a local bone setter; however, her hand discomfort continued and she then came to us for treatment. The radiography and computed tomography scan demonstrated a pisiform dislocation. The pisiform bone was surgically removed. The pisiform dislocation should be considered in the differential diagnosis of the ulnar side wrist discomfort following wrist trauma.
