In a double-blind study the therapeutic effect and tolerance of pizotifen was compared with placebo in 20 outpatients of both sexes suffering from light to moderate depression. Pizotifen seems to possess certain antidepressive effects in dosages of 4-10 mg daily. These properties, and the well-documented effect of pizotifen in migraine, could make it an alternative in treating patients suffering from the often seen combination of vascular headache/depression.

Recent advances in migraine therapy include the recognition of analgesic or ergotamine abuse as a cause of chronic daily migraine, the introduction of effective non-narcotic drugs such as chlorpromazine, dihydroergotamine and corticosteroids for the treatment of intractable migraine attacks, the increased number of beta-blockers now recognized as effective prophylactic agents and the introduction of calcium-channel blockers for prophylaxis. There is a sufficient variety of antimigraine drugs, and therapy should be successful for most sufferers.

Serotonin agonists (for the acute treatment of attacks) and antagonists (for prophylactic treatment) are the most widely used drugs to treat migraine. However, their effectiveness is not complete and their use is limited by side effects. The activity and presumptive mode of action of these drugs provide support for the role of serotonin in the pathophysiology of migraine and suggest that the trigeminal-vascular system is at the center of the attack; however, other factors and mechanisms may also be involved.

The results of a double-blind cross-over clinical trial involving 27 patients with classical or common migraine are described to compare the prophylactic effect of the calcium entry blocker flunarizine with that of pizotifen.
