Measuring the placebo effect in carpal tunnel syndrome.

BACKGROUND: The placebo effect can account for part of the improvement seen in patients undergoing any type of treatment, be it surgical or pharmacological. The objective of this study is to quantify the placebo effect in carpal tunnel syndrome treatment. MATERIALS AND METHODS: A double-blinded randomized trial was performed with 68 patients suffering from mild to moderate carpal tunnel syndrome, divided into two groups with no statistically significant differences regarding age, weight, or degree of nerve compression. The patients were evaluated clinically and electromyographically before an

Electroconvulsive therapy, the placebo effect and informed consent.

Major depressive disorder is not only the most widespread mental disorder in the world, it is a disorder on the rise. In cases of particularly severe forms of depression, when all other treatment options have failed, the use of electroconvulsive therapy (ECT) is a recommended treatment option for patients. ECT has been in use in psychiatric practice for over 70 years and is now undergoing something of a restricted renaissance following a sharp decline in its use in the 1970s. Despite its success in treating severe depression there is continued debate as to the effectiveness of ECT: in some stu

Flickering lenses enhance reading performance through placebo effect.

Developmental dyslexia (DD) is the most frequent neurodevelopmental disorder among school-age children. Traditional remediation programs for DD are rarely controlled for the placebo effect, raising the hypothesis that positive expectations might explain their efficacy. Wearing expensive flickering glasses has been associated with extraordinary improvements in reading skills. The placebo effect and efficacy of these glasses on reading performance were tested. A double blind within-subject experimental design was used in children with DD (n = 49; Experiment 1) and unselected young adults (n = 48

Molecular classification of the placebo effect in nausea.

In this proof-of-concept study, we tested whether placebo effects can be monitored and predicted by plasma proteins. In a randomized controlled design, 90 participants were exposed to a nauseating stimulus on two separate days and were randomly allocated to placebo treatment or no treatment on the second day. Significant placebo effects on nausea, motion sickness, and (in females) gastric activity could be verified. Using label-free tandem mass spectrometry, 74 differentially regulated proteins were identified as correlates of the placebo effect. Gene ontology (GO) enrichment analyses identifi

Placebo effect in the treatment of duodenal ulcer.

AIMS: To assess whether frequency of placebo administration is associated with duodenal ulcer healing. METHODS: A systematic literature review of randomized clinical trials was undertaken. 79 of 80 trials that met the inclusion criteria. The pooled 4 week placebo healing rate of all duodenal ulcer trials that employed a four times a day regimen was compared with the rate obtained from trials with a twice a day regimen. RESULTS: The pooled 4 week healing rate of the 51 trials with a four times a day regimen was 44. 2% (805 of 1821 patients) compared with 36.2% (545 of 1504 patients) in the 28

Placebo effect and clinical trials in migraine therapy.

Migraine is a cyclical condition in which the cycles may occur or remit in an unpredictable fashion. The goal of prophylactic antimigraine therapy is to induce remission of an active cycle of migraine or to inhibit occurrence of individual headaches until the cycle remits for other reasons. The placebo effect probably takes advantage of these cycles. The placebo effect is significant and quite potent. The placebo effect may be divided into (1) the initial effect and (2) the continuing effect which is seen following a stabilization period. The initial placebo effect is dramatic with 62% of 188

The reward for placebos: mechanisms underpinning placebo-induced effects on motor performance.

Different from the most popular thinking, the placebo effect is not a purely psychological phenomenon. A body of knowledge from multidisciplinary fields has shown that the expectation of a potential benefit when receiving a treatment induces a cascade of neurochemical-electrophysiological alterations in brain reward areas, including motor-related ones. Alterations in the dopamine, opioid, and glutamate metabolism are the neural representation converting reward-derived declarative forms into an attractive and wanted behavior, thereby changing the activation in reward subcortical and cortical st

Evaluation of the placebo effect in the trials of allergen immunotherapy effectiveness: meta-analysis of randomized and placebo-controlled trials.

INTRODUCTION: The total effect of the method of treatment is composed of its specific activity depending on its impact on the disease mechanism and the non-specific activity, i.e. the placebo effect. Many methods of treatment make use of such an inflammatory action. AIM: To assess the placebo effect in the overall result of the specific immunotherapy and the analysis of its dependence on the type of specific immunotherapy, the disease, the age of a patient, the type of allergy, indicators used (objective and subjective), in patients with allergic diseases - asthma, allergic rhinitis and/or al

Does placebo effect exist in contact lens discomfort management?

PURPOSE: To study the effect of daily disposable contact lens (DDCL) refitting in monthly contact lens (CL) wearers suffering from contact lens discomfort (CLD), and to assess if there is a placebo effect associated with this CLD intervention. METHODS: Symptomatic CL wearers, according to the Contact Lens Dry Eye Questionnaire 8 (CLDEQ-8 ≥ 12 points), were divided into two groups. The study group was provided with DDCLs (delefilcon A) and the control group with a masked pair of their habitual monthly CLs (placebo effect) and were assessed after one month. Then, the control group was provided

The Placebo Effect in Cardiology: Understanding and Using It.

The placebo effect is the clinical benefit caused by interaction with a caregiver and health care system in the absence of a biologically active intervention and has been used successfully for millennia. The placebo response results from the interaction of psychosocial mechanisms, human relationships, and preconceptions functioning in specific neuroanatomic locations with known genes and neurotransmitters. It occurs with or without the administration of an inactive substance to deliberately deceive patients. Our purpose is to review the history, benefits, and mechanisms of the placebo effect.

The placebo effect in thyroid cancer: a meta-analysis.

CONTEXT: The natural history of advanced thyroid malignancies is largely unknown. The outcome of patients included in the placebo arm of clinical trials could be reflective of their therapy-free evolution. OBJECTIVE: To analyze the response rate, symptoms and adverse effects of locally advanced or metastatic differentiated (DTC) and medullary thyroid cancer (MTC) in patients treated with placebo in clinical trials. DESIGN: PubMed (MEDLINE) and SCOPUS databases were searched through September 2015 to identify high-quality randomized controlled clinical trials. We included studies that recruit

[Placebo effect: clinical, biological and therapeutical involvements in depression].

The placebo effect is an excellent model for understanding the mechanisms underlying the interaction between a subjective and complex mental activity (beliefs, expectations, hopes, learning, patient-physician relationship, socio-cultural context .) with different neural and biological systems. Initially, research on the placebo effect has focused on the mechanisms of pain and analgesia. The cognitive processes of conditioning and reward anticipation (hope of a relief) were highlighted. The involvement of different neurobiological pathways has been clearly shown: endogenous opioids, CCK, dopami

Transcutaneous electrical nerve stimulation following appendicectomy: the placebo effect.

A controlled trial was undertaken to compare the efficacy of transcutaneous electrical nerve stimulation (TENS) with standard intramuscular opiate analgesia in the management of postoperative pain following appendicectomy. Consecutive patients undergoing emergency appendicectomy were randomised into control, sham TENS and active TENS groups. There was a significant decrease in pain severity and analgesic intake in both active and sham TENS groups when compared with the control group (P less than 0.01). No difference was demonstrated in pain severity between active and sham TENS groups but the

Potential placebo effect in assessing idiopathic normal pressure hydrocephalus.

Although shunt placement constitutes the primary treatment for idiopathic normal pressure hydrocephalus (INPH), it has been challenging to decide which patients to refer for such an intervention. Procedures involving CSF removal, such as large-volume lumbar puncture (LP) and external lumbar drainage, are commonly used to predict a successful response to surgery, but their positive and negative predictive values have considerable shortcomings. The authors here report the case of a 76-year-old woman with possible INPH whose condition improved equally well after actual and sham large-volume LPs.

The placebo effect of ankle taping in ankle instability.

PURPOSE: Recurrence of ankle sprains is common among athletes. Although ankle taping reduces the risk of injury, the mechanism underlying its effectiveness remains unclear. Anecdotal reports suggest a role of the belief among athletes that taping will protect them from injury. That is, taping may have a placebo effect. The purpose of the present study was to determine whether there was a placebo effect with ankle taping in individuals with ankle instability. METHODS: Thirty participants with ankle instability completed a hopping test and a modified star excursion balance test under three cond

[The relationship between the placebo effect and spontaneous improvement in research on antidepressants. Are placebos powerless?].

Clinical trials of antidepressant medications have generally found large changes in groups given a placebo, which may be due to either spontaneous remission or a true placebo effect. This paper reviews the evidence for a true placebo effect in the treatment of unipolar depressed outpatients. Although there is no evidence from experimental studies, a rather substantial amount of circumstantial evidence indicates a true placebo effect. This article raises the question of whether it is meaningful to require experimental evidence for a loose and unspecified concept involving varying components suc

The placebo and nocebo effects in functional urology.

A placebo is an inert substance normally used in clinical trials for comparison with an active substance. However, a placebo has been shown to have an effect on its own; commonly known as the placebo effect. A placebo is an essential component in the design of conclusive clinical trials but has itself become the focus of intense research. The placebo effect is partly the result of positive expectations of the recipient on the state of health. Conversely, a nocebo effect is when negative expectations from a substance lead to poor treatment outcomes and/or adverse events. Randomized controlled t
