OBJECTIVE: Ventral decompressive surgery of the craniocervical junction is performed to manage a variety of conditions, including basilar invagination, which can be associated with platybasia. We have noted that the anatomic changes of platybasia could affect the height of the odontoid over a line drawn along the nasal cavity floor, the palatine line (PL). This anatomic change may influence the use of nasal endoscopic surgery for patients with platybasia who also have basilar invagination. We investigated whether the height of the craniocervical junction is elevated over the PL in patients wit

BACKGROUND: The flexion of the skull base (basal angle [BA]) is the inclination between the anterior fossae and the basilar ramp of the occipital and sphenoid bones. An increased BA, termed platybasia, is usually associated with basilar invagination. BA reference values in the magnetic resonance imaging (MRI) era and the diagnosis of platybasia are of clinical importance. The transnasal approach has been the surgical technique of choice to remove the odontoid process in cases of ventral brainstem compression in patients with platybasia. The knowledge of normal BA values has been influenced by

OBJECTIVES: In the context of a lack of national consensus on the benefits of skull base imaging in children with osteogenesis imperfecta (OI), this study aims to analyse and correlate the clinical symptoms and radiological images of children with severe OI.
METHODS: A retrospective case notes and image analysis was carried out on children with complex OI between 2012 and 2018 at a specialist tertiary centre. Data were collected on patient demographic factors, clinical data, imaging findings (presence of Wormian bones, platybasia, basilar impression (McGregor's technique) and basilar invagina

BACKGROUND AND OBJECTIVES: In patients with syndromic craniosynostosis, early synostosis of the cranial base synchondroses may occur, potentially increasing the frequency of abnormalities in the cranial base. We aim to estimate the prevalence of platybasia, basilar invagination, odontoid retroflexion, Chiari malformation type I (CMI), and cervical syringomyelia in patients with syndromic craniosynostosis. Furthermore, we aim to assess the changes in prevalence between the initial preoperative study and the latest available study for each patient.
METHODS: This is a retrospective multicenter s

Os odontoideum is a rare craniocervical abnormality of the axis resulting in absence (aplasia) of the dens and, possibly, dislocation of the atlantoaxial articulation. Skeleton 0202 from the Osteology Collection at Khon Kaen University, Thailand exhibits aplasia of the os odontoideum with torticollis, accompanied by an unusual constellation of skeletal features that include craniosynostosis, platybasia with mild basilar invagination, unilateral Kimmerle anomaly, disuse atrophy resulting in "wasting" of the upper and lower limbs, and marked stenosis of the right foramen transversarium of the ax

For a long time the terms basilar invagination and platybasia were used interchangeably. Basilar invagination has been defined as a prolapse of the vertebral column into the spinal cord. Platybasia is defined as an abnormal obtuse angle between the anterior skull base and the clivus. The authors review the existing literature and summarize the historical and modern perspectives in the management of basilar invagination. From radiological curiosities, the subject of basilar invagination is now viewed as eminently treatable. The more pronounced understanding of the subject has taken place in the

This brief review focuses on the imaging of the craniocervical junction with particular attention to linear and angular craniometry. It attempts to answer the question of why there are so many measurements and how they are clinically useful. Congenital anomalies of the craniocervical junction will be discussed with an overview of craniocervical junction embryology and development. Entities affecting the craniocervical junction which will be discussed include platybasia, basilar invagination, atlanto-occipital assimilation, and assorted congenital anomalies.

A previously undescribed form of short-limbed dwarfism is reported in an 86-year-old man. All the long bones were shorter than normal, with extreme shortening of the humeri. Other features included dysplastic humeral heads, coxa vara with dysplasia of the femoral heads and acetabula, platybasia, and small vertebral bodies. In all other respects, including intelligence, the patient was normal for his age.

A case of dicephalus, Arnold-Chiari malformation (ACM) and spina bifida was reported in a Japanese Black calf. Both brains and crania showed the same abnormalities as those of the monocephalic cases with ACM and were mirror-images of each other. The common findings were elongation of the brain, caudal displacement and hypoplasia of the cerebellum, dorso-ventral compression of the skull, platybasia, and enlargement of the foramen magnum. The craniolacunia was also found in our case.

A case of Leopard syndrome with full clinical expression is reported. In addition to the cardinal signs of the syndrome this patient presented some abnormalities which have not been previously described such as macroglossia, multiple dental anomalies, basilar impression and platybasia, megacolon, hypertrophy of clitoris and anal ectopy. The presence of dental anomalies and megacolon may represent involvement of the dental papillae and myenteric plexus favoring the view that the syndrome results from a derangement of the neural crest elements.

An 18-year-old, previously healthy male presented with bilateral pneumonia and acute respiratory failure with severe carbon dioxide retention. The presence of mild brainstem signs and hypoventilation led to the discovery of a platybasia, basilar invagination, and kinking of the medulla oblongata with early syrinx. He was operated upon but postoperatively was noted to have a mixed type of sleep apnea. This case illustrates the diagnostic challenge in acute respiratory failure in a previously healthy young person and the possible pathogenic mechanisms underlying it.

Hajdu-Cheney syndrome (HCS) is a rare genetic disorder with autosomal dominant inheritance, although most cases result from de novo mutations. Progressive platybasia and basilar impression (BI) can potentiate obstructive hydrocephalus due to aqueductal stenosis. Limited literature exists on the surgical intervention for hydrocephalus in patients with this condition. The authors present (to their knowledge) the first case of obstructive hydrocephalus due to aqueductal stenosis from BI treated with an endoscopic third ventriculostomy in a patient with the complex anatomy of HCS.

Odontoidectomy is the treatment of choice for irreducible ventral cervical-medullary compression. The endonasal endoscopic approach is an innovative approach for odontoidectomy. The aim of this article is to identify in which conditions this approach is indicated, discussing variants of the technique for selected cases of craniovertebral malformation with platybasia. We believe that the technical difficulties of this approach are balanced by the advantages for patients. Some conditions related to the patient and to the anatomy of the craniovertebral junction may favor adoption of the endoscopi

OBJECT: Osteogenesis imperfecta (OI), which usually results from mutations in type I collagen genes, causes bone fragility and deformities. The head is often abnormally shaped, and changes in skull base anatomy in the form of basilar impression and basilar invagination have been reported. The authors analyzed the skull base anatomy on standardized lateral cephalograms from 54 patients with OI (Types I, III, and IV) and 108 control volunteers. They were surprised to find that the previously used diagnostic measures for basilar abnormality in patients with OI were exceeded in 6.5 to 7.4% of the
