The efficacy of fractional CO2 laser with or without triamcinolone acetonide or 5-fluorouracil in the treatment of early postburn hypertrophic scars.

Hypertrophic scars (HTSs) are the result of an abnormal healing process resulting from burns and other severe traumas. The symptoms of that condition include skin irritation, discomfort, and itching. This study aimed to assess the efficacy of fractional carbon dioxide (CO2) laser therapy alone or with triamcinolone or 5-fluorouracil (FU) in the treatment of early post-burn hypertrophic scars (HTSs) that develop during the first 6 months after the injury. A prospective, randomized, single-blind comparative study was conducted on 30 patients aged 16-65 with hypertrophic scars (HTS) resulting fro

Stromal vascular fraction-enriched fat grafting as treatment of adherent scars: study design of a non-randomized early phase trial.

BACKGROUND: In the last decades, autologous fat grafting has been used to treat adherent dermal scars. The observed regenerative and scar-reducing properties have been mainly ascribed to the tissue-derived stromal vascular fraction (tSVF) in adipose tissue. Adipose tissue's components augment local angiogenesis and mitosis in resident tissue cells. Moreover, it promotes collagen remodeling. We hypothesize that tSVF potentiates fat grafting-based treatment of adherent scars. Therefore, this study aims to investigate the effect of tSVF-enriched fat grafting on scar pliability over a 12-month per

Effectiveness of Autologous Fat Grafting in Adherent Scars: Results Obtained by a Comprehensive Scar Evaluation Protocol.

BACKGROUND: Nowadays, patients normally survive severe traumas such as burn injuries and necrotizing fasciitis. Large skin defects can be closed but the scars remain. Scars may become adherent to underlying structures when the subcutical fat layer is damaged. Autologous fat grafting provides the possibility of reconstructing a functional sliding layer underneath the scar. Autologous fat grafting is becoming increasingly popular for scar treatment, although large studies using validated evaluation tools are lacking. The authors therefore objectified the effectiveness of single-treatment autolog

Progression of surgical scars in the hand and wrist over time: a peak in scar-related symptoms.

We have noted a peak in induration and tenderness around scars with accompanying symptoms 6-8 weeks after most hand operations. The purpose of this study was to characterise this phenomenon. All consecutive patients treated for fractures of the distal radius through a volar scar were prospectively enrolled. Exclusion criteria included: previous injuries of that wrist, no volar scar, open fractures with considerable soft tissue injury, or injuries to the nerve or vessel, or all three. Patients were evaluated every other week for three months after the operation for the following: tightness, the

Symptomatic Outcomes After Autologous Fat Grafting in Irradiated Postmastectomy Chest Wall.

BACKGROUND/OBJECTIVES: Radiotherapy of the chest wall after mastectomy frequently leads to fibrosis, reduced tissue elasticity, erythema, pain and chronic skin-related symptoms that complicate reconstructive strategies. Autologous fat grafting has been proposed as a regenerative option for radiation induced soft tissue damage, but clinical data focused on patient-reported symptoms remain limited. The objective of this study was to describe symptomatic and clinical changes after autologous fat grafting in irradiated postmastectomy chest wall tissue. METHODS: This pilot observational study incl

[Interhemisphere asymmetry of the CSF dynamics and biomechanical properties of the skull].

This investigation shows that both intracranial liquor circulation and skull biomechanical properties evaluated by its pliability (compliance) to intracranial pressure are characterised by marked interhemisphere asymmetry. The interhemisphere differences of cerebrospinal fluid mobility were evaluated by means of asymmetry coefficient (right/left ratio of liquor mobility) which was found to be 1.25-1.45 in healthy middle-age persons. For the skull pliability (compliance) the coefficient of hemispheric asymmetry was 0.75-0.95. These two hemisphere asymmetry coefficients are characterized by reci

The efficacy, satisfaction, and safety of carbon dioxide (CO2) fractional laser in combination with pulsed dye laser (PDL) versus each one alone in the treatment of hypertrophic burn scars: a single-blinded randomized controlled trial.

The tendency to use less-invasive laser-based methods with far more obvious effectiveness has been taken into consideration today for improvement of burning scars. The present study thus aimed to assess the efficacy of two laser-based techniques including pulsed dye laser (PDL) and ablative fractional CO2 laser (AFCL) and its combination on improving different aspects of burning scars regardless of the types of scar as hypertrophic or keloid scars. This randomized single-blinded clinical trial was performed on patients suffering hypertrophic or keloid burning scars. The patients were randomly

Radiofrequency therapy attenuates hypertrophic scar formation in rabbit ears by modulating the TGF-β1/Smad3 signaling pathway.

OBJECTIVE: To investigate the impact of radiofrequency (RF) therapy on the formation of hypertrophic scars in rabbit ears and its regulatory effect on the TGF-β1/Smad3 signaling pathway. METHODS: Six male New Zealand white rabbits aged 3-5 months were selected. Six full-thickness skin defect wounds were created on the ventral side of each ear (72 total). Wounds were observed on days 0, 7, 14, and 21, and the scar formation rate was calculated on day 21. Right ear scars (n = 36) served as controls; left ear scars (n = 36) received RF therapy. After 14 days, scar pigmentation and pliability wer

Long-term Performance of Fresh Autologous Pericardium for Mitral Valve Leaflet Repair.

BACKGROUND: Glutaraldehyde-fixed autologous or bovine pericardial patches used for mitral valve leaflet reconstruction have been associated with late calcification. Fresh autologous pericardium (FAP) may be a durable alternative. METHODS: Transthoracic echocardiography was used to assess valve function (regurgitation, mean pressure gradient, patch pliability, and calcification) in patients undergoing FAP mitral leaflet repairs. Pliability was scored between 1 (similar to native leaflets) and 4 (rigid). Calcification was scored between 1 (echobrightness similar to native leaflets) and 4 (very

Application of Cultured Epidermal Homograft (Kaloderm) for Wide Scar Treatment.

BACKGROUND: For the treatment of wide scars, laser resurfacing procedures are generally used. However, sometimes their results are not satisfactory. Many clinical studies have reported that cultured epidermal allogenic sheets promote rapid and good quality wound healing. Therefore, the authors applied a cultured epidermal homograft (CEH) for scar management and investigated its outcomes. METHODS: Thirty-two patients who received a CEH (Kaloderm) after laser resurfacing (n = 14, under general anesthesia; n = 18, under local anesthesia) between February 2016 and June 2017 were enrolled. Endoscopic endonasal recanalization of the obliterated eustachian tube via ureteral stent.

Eustachian tube recanalization is a feasible procedure but additional studies are needed to determine its safety. Eustachian tube closure can result from different etiologies and can cause severe symptoms. Ureteral stents have appropriate shape and pliability for placement and long-term healing. Multidisciplinary team approach allows for simultaneous endonasal and otologic approaches.

Using Shockwave Lithotripsy to Treat Severe Calcified Mitral Stenosis in a Pregnant Woman.

Rheumatic mitral stenosis (MS) with significant calcification presents challenges for both surgical and transcatheter interventions. Percutaneous balloon mitral valvuloplasty (PBMV) is often limited in these cases due to valve rigidity, increasing the risk of mitral regurgitation (MR). Transcatheter mitral valve lithotripsy (TMVL) is an emerging technique that uses sonic waves to disrupt calcification, enhancing valve pliability and PBMV outcomes. A 39-year-old woman, 18 weeks pregnant, with a history of rheumatic heart disease and prior surgical mitral commissurotomy, presented with palpitati
