Development and evaluation of a systems-based point-of-care ultrasound curriculum for a master of science in athletic training program.

BACKGROUND: As an imaging modality, point-of-care ultrasound (POCUS) holds substantial value to health professionals who are adequately trained in its use. The athletic training profession is an allied health profession with significant potential to adopt POCUS in training and practice. However, published research on POCUS curricula for graduate entry-level programs in athletic training is lacking. Furthermore, there are no published studies of learning outcomes from POCUS courses for graduate entry-level athletic training students. METHODS: We developed a POCUS curriculum tailored for a grad

A customized Raman system for point-of-care detection of arthropathic crystals in the synovial fluid.

Monosodium urate (MSU) and calcium pyrophosphate dihydrate (CPPD) are the most frequently observed crystals in joint space, leading to painful arthropathies. Correct diagnosis of the crystal identity is critical for the appropriate course of treatment. In this work, a custom Raman device in combination with a practical and efficient sample preparation method is used for chemically selective diagnosis of MSU and CPPD crystals in an automated fashion. The samples were prepared by brief enzymatic digestion treatment of synovial fluid followed by a customized filtration process which was able to c

Bilistick: a low-cost point-of-care system to measure total plasma bilirubin.

BACKGROUND: Severe neonatal hyperbilirubinemia, with consequent encephalopathy, remains a common cause of morbidity and death in many regions of the world. Poor access to clinical laboratory resources and screening programs to measure plasma bilirubin levels is a major contributor to delayed treatment in developing countries, and the cost of existing point-of-care screening instruments precludes their dissemination. OBJECTIVES: We are evaluating the accuracy of a low-cost, minimally invasive point-of-care system (Bilistick) requiring a 25-µl blood sample that could be used in low-resource env

Call to action for health systems integration of point-of-care testing to mitigate the transmission and burden of sexually transmitted infections.

OBJECTIVES: In 2016, WHO estimated 376 million new cases of the four main curable STIs: gonorrhoea, chlamydia, trichomoniasis and syphilis. Further, an estimated 290 million women are infected with human papillomavirus. STIs may lead to severe reproductive health sequelae. Low-income and middle-income countries carry the highest global burden of STIs. A large proportion of urogenital and the vast majority of extragenital non-viral STI cases are asymptomatic. Screening key populations and early and accurate diagnosis are important to provide correct treatment and to control the spread of STIs.

Leveraging Artificial Intelligence in the United States Health System: Point-of-Care Automation in Reducing Care Cost Burden.

INTRODUCTION: The United States healthcare system is at an inflection point, with the majority of hospitals now operating at suboptimal margins, ranging from -3.8% to 2.3%. Concurrently, perioperative inefficiencies and the escalating burden of administrative work have driven rising operating costs. METHODS: A scoping review was conducted to evaluate how artificial intelligence can improve surgical margins and reduce inefficiencies across medicine and surgery. The extracted data was then synthesized both qualitatively and quantitatively. RESULTS: Surgical care, which accounts for 70% of hosp

Feasibility of a portable PET system for interactive point-of-care (PoC) PET and ultrasound imaging.

BACKGROUND: Positron emission tomography (PET) is a molecular imaging technology that has been used for detection and staging of diseases, and patient stratification to identify candidates for molecularly targeted drugs. Clinical PET/CT scanners are optimized for whole-body diagnostic imaging instead of real-time guidance of interventional procedures or point-of-care (PoC) imaging applications. With rapid advances in novel radio-theranostics, there is growing interest in versatile, accessible molecular imaging tools. PURPOSE: This study evaluates the feasibility of a compact PET device integr

A Finger-Actuated Microfluidic System for Point-Of-Care Detection of SARS-CoV-2 and Influenza A.

The cocirculation and infection of SARS-CoV-2 and influenza A viruses significantly impact the transmission dynamics of infectious diseases and increase the public health burden on communities. Nucleic acid amplification testing (NAAT), the gold standard for pathogen detection, has been widely used in point-of-care testing (POCT). However, consumer-grade NAAT-based medical POCT products tailored to resource-limited settings remain scarce. In this study, we developed a finger-actuated visual microfluidic chip with a colorimetric detection system based on reverse transcriptase loop-mediated isot

Feasibility of photoacoustic/ultrasound imaging of synovitis in finger joints using a point-of-care system.

We evaluate a portable ultrasound and photoacoustic imaging (PAI) system for the feasibility of a point-of-care assessment of clinically evident synovitis. Inflamed and non-inflamed proximal interphalangeal joints of 10 patients were examined and compared with joints from 7 healthy volunteers. PAI scans, ultrasound power Doppler (US-PD), and clinical examination were performed. We quantified the amount of photoacoustic (PA) signal using a region of interest (ROI) drawn over the hypertrophic joint space. PAI response was increased 4 to 10 fold when comparing inflamed with contralateral non-infl

An assessment of a point-of-care information system for the anesthesia provider in simulated malignant hyperthermia crisis.

In this prospective, controlled study we compared the ability of anesthesia residents to diagnose and treat a simulated malignant hyperthermia (MH) scenario with and without the ability to use the On-Line Electronic Help (OLEH) information system or any other written guidelines. The OLEH is a point-of-care information system for the anesthesia provider in the operating room. The score for MH treatment after diagnosis based on clinical actions was significantly higher (P = 0.018) in the OLEH-user group (21.5 +/- 4.9) compared with a control group (15.5 +/- 7.6). This study demonstrates the poss

Artificial intelligence enhanced microfluidic system for multiplexed point-of-care-testing of biological thiols.

Cysteamine (CA) serves as a cystine-depleting agent employed in the management of cystinosis and a range of other medical conditions. Monitoring blood CA levels at the point of care is imperative due to the risk of toxicity associated with elevated CA dosages. An additional significant challenge is presented by the intricate composition of human plasma and the presence of various interfering biological thiols, which possess similar structures or properties. Here, this work proposes an AI-enhanced Lab-on-a-disc system, also termed AI-LOAD, for multiplexed point-of-care testing of cysteamine. Th

Could a handheld point-of-care ultrasound system alter the management of patients with pleural infection?

Lung ultrasonography is an essential tool in the management of pleural infection. We report the usage of lung point-of-care ultrasound (POCUS) using a handheld ultrasound system (HHUS), which changed the immediate management of a complex pleural effusion.A middle-aged diabetic patient presented with fever, dyspnoea and left-sided pleuritic chest pain, suggestive of left pleural infection. Lung POCUS using a HHUS showed a multiloculated pleural effusion with no connections between the locules, and this was confirmed later with CT, along with left empyema necessitans. An upfront video-assisted t

NIR-based Sensing System for Non-invasive Detection of Hemoglobin for Point-of-care Applications.

BACKGROUND: Hemoglobin is an essential biomolecule for the transportation of oxygen, therefore, its assessment is also important to be done frequently in numerous clinical practices. Traditional invasive techniques have concomitant shortcomings, such as time delay, the onset of infections, and discomfort, which necessitate a non-invasive hemoglobin estimation solution to get rid of these constraints in health informatics. Currently, various techniques are underway in the allied domain, and scanty products are also feasible in the market. However, due to the low satisfaction rate, invasive solu

Integrated, Automated, Fast PCR System for Point-Of-Care Molecular Diagnosis of Bacterial Infection.

We developed an integrated PCR system that performs automated sample preparation and fast polymerase chain reaction (PCR) for application in point-of care (POC) testing. This system is assembled from inexpensive 3D-printing parts, off-the-shelf electronics and motors. Molecular detection requires a series of procedures including sample preparation, amplification, and fluorescence intensity analysis. The system can perform automated DNA sample preparation (extraction, separation and purification) in ≤5 min. The variance of the automated sample preparation was clearly lower than that achieved us

Feasibility study of a point-of-care positron emission tomography system with interactive imaging capability.

PURPOSE: We investigated the feasibility of a novel positron emission tomography (PET) system that provides near real-time feedback to an operator who can interactively scan a patient to optimize image quality. The system should be compact and mobile to support point-of-care (POC) molecular imaging applications. In this study, we present the key technologies required and discuss the potential benefits of such new capability. METHODS: The core of this novel PET technology includes trackable PET detectors and a fully three-dimensional, fast image reconstruction engine implemented on multiple gr

First report of the point-of-care TEG: A technical validation study of the TEG-6S system.

Thrombelastography (TEG) measured by the TEG5000 Hemostasis Analyzer is an established but the labor-intensive method for assessing global hemostasis. The first true point-of-care TEG, the TEG6s system, uses resonance-frequency viscoelasticity measurements and a disposable multi-channel microfluidic cartridge to assess hemostasis and response to antiplatelet therapy. TEG assays (n = 5,100) were performed on the blood of healthy volunteers (n = 157) and patients undergoing coronary revascularization at three hospitals (n = 300). The results from the TEG6s were compared with the conventional TEG

Design and Testing of a Single-Element Ultrasound Viscoelastography System for Point-of-Care Edema Quantification.

Management of fluid overload in patients with end-stage renal disease represents a unique challenge to clinical practice because of the lack of accurate and objective measurement methods. Currently, peripheral edema is subjectively assessed by palpation of the patient's extremities, ostensibly a qualitative indication of tissue viscoelastic properties. New robust quantitative estimates of tissue fluid content would allow clinicians to better guide treatment, minimizing reactive treatment decision making. Ultrasound viscoelastography (UVE) can be used to estimate strain in viscoelastic tissue,

Innovative Digital Tools and Surveillance Systems for the Timely Detection of Adverse Events at the Point of Care: A Proof-of-Concept Study.

INTRODUCTION AND OBJECTIVE: Regulatory authorities often receive poorly structured safety reports requiring considerable effort to investigate potential adverse events post hoc. Automated question-and-answer systems may help to improve the overall quality of safety information transmitted to pharmacovigilance agencies. This paper explores the use of the VACC-Tool (ViVI Automated Case Classification Tool) 2.0, a mobile application enabling physicians to classify clinical cases according to 14 pre-defined case definitions for neuroinflammatory adverse events (NIAE) and in full compliance with da
