Brazilian journal of microbiology : [publication of the Brazilian Society for Microbiology]2016eng
During the last 30 years there has been a dissemination of plasmid-mediated β-lactamases in Enterobacteriaceae in Brazil. Extended spectrum β-lactamases (ESBL) are widely disseminated in the hospital setting and are detected in a lower frequency in the community setting. Cefotaximases are the most frequently detected ESBL type and Klebsiella pneumoniae is the predominant species among ESBL producers. Klebsiella pneumoniae carbapenemase-producing Enterobacteriaceae became widely disseminated in Brazil during the last decade and KPC production is currently the most frequent resistance mechanism

Aim: Polymyxin B (PMB) is one of the few therapeutic options for treating infections caused by carbapenem-resistant Gram-negative bacteria (CR-GNB). However, the emergence of PMB-resistant CR-GNB strains has prompted the exploration of antibiotic adjuvants as potential therapeutic avenues. Thus, this study evaluates the potential of 3,5-dinitrobenzoic acid derivatives (DNH01, DNH11, DNH13 and DNH20) and isoniazid-N-acylhydrazones (INZ1-7, INZ9 and INZ11) as adjuvants to enhance PMB efficacy against CR-GNB.Materials & methods: MIC, MBC and drug combination assays were conducted using multidrug-

Polymyxin-induced nephrotoxicity is the major dose-limiting factor and can occur in up to 60% of patients after intravenous administration. This chapter reviews the latest literature on the mechanisms of polymyxin-induced nephrotoxicity and its amelioration. After filtration by glomeruli, polymyxins substantially accumulate in renal proximal tubules via receptor-mediated endocytosis mainly by megalin and PEPT2. It is believed that subsequently, a cascade of interconnected events occur, including the activation of death receptor and mitochondrial apoptotic pathways, mitochondrial damage, endopl

Background/Objectives: Polymyxin B (PMB) was incorporated into a sodium deoxycholate sulfate (SDCS) micelle formulation to mitigate polymyxin-induced nephrotoxicity. This study examined the effect of the formulation on nephrotoxicity and biodistribution in a rat model. Methods: Four groups of rats were subcutaneously administered one of the following: normal saline, SDCS, PMB, or a PMB-SDCS formulation. After treatment, the weight changes were recorded, and the rats were euthanized to collect blood for serum biochemistry measurements. The histopathological damage to organs was examined. Two ad

The global spread of multi-drug-resistant bacteria, including strains resistant to both polymyxin and carbapenems, poses a significant threat to public health, highlighting the need to develop new antimicrobial agents. This study assessed the antimicrobial potential of cinnamaldehyde, alone and in combination with antibiotics, against carbapenem-polymyxin-resistant K. pneumoniae strains (CPR-Kp). The antimicrobial activity was assessed through minimum inhibitory concentration (MIC), checkerboard assay, and survival curve analysis. Cinnamaldehyde showed inhibitory effects (MIC 281 µg/mL), and w

This study provides insights into the mechanisms of polymyxin resistance in K. pneumoniae clinical isolates and demonstrates potential strategies of polymyxin and vancomycin combinations for combating this resistance. We also identified possible mechanisms that might be associated with the treatment of these combinations against carbapenem- and polymyxin-resistant K. pneumoniae clinical isolates. The findings have significant implications for the development of alternative therapies and the effective management of infections caused by these pathogens.

BACKGROUND: Pentacyclic triterpenoids are diverse class of plant-derived bioactive scaffolds, but their potential to resensitize resistant Gram-negative pathogens to antibiotics remains underexplored. Polymyxins are last-line drugs for multidrug-resistant (MDR) Gram-negative infections, yet the global dissemination of mcr genes and chromosomal mutations in pmrAB/phoPQ compromise their efficacy.
PURPOSE: This study aimed to evaluate the antibacterial potential of the natural triterpenoid celastrol (CEL, derived from Tripterygium wilfordii). We specifically investigated its ability to function

The increasing incidence and emergence of polymyxin-resistant Acinetobacter baumannii pose a significant threat to global public health. The presence of sub-minimal inhibitory concentration (sub-MIC) antibiotics during clinical treatment is inevitable and may drive the development of antibiotic resistance. Therefore, understanding the fitness costs and underlying mechanisms of resistance evolution under sub-MIC polymyxin B (PMB) is crucial for controlling the spread of PMB-resistant strains. In this study, PMB resistance evolved rapidly within 1-2 days in different A. baumannii strains and acq

The commercial Rapid Polymyxin NP test was evaluated to detect colistin-resistant Enterobacteriaceae. A total of 223 enterobacterial isolates corresponding to 136 resistant (including 38 MCR-like producers), 19 heteroresistant, and 78 colistin-susceptible isolates were tested. The test was performed according to the manufacturer's instruction, and the color of the wells was read after 2 and 3 hours of incubation. The results were compared with those of the homemade Rapid Polymyxin NP test, and manual broth microdilution according to EUCAST guidelines was used as the reference method to determi

The emergence of resistance to polymyxins in KPC-producing Klebsiella pneumoniae isolates has been a major clinical problem. This study evaluated the molecular mechanisms associated with polymyxin B (PMB) resistance that emerged in a previously PMB-susceptible KPC-2-producing K. pneumoniae during PMB therapy for a bloodstream infection in a neutropenic patient. The first isolate (PMB-susceptible) was obtained while the patient was receiving meropenem and other isolates were recovered from 2 sets of blood cultures in different dates while the patient was receiving PMB therapy (4 of 6 blood cult

INTRODUCTION: The incidence of healthcare-associated multidrug resistant bacterial infections, particularly due to carbapenem resistant organisms, has been on the rise globally. Among these are the carbapenem resistant Acinetobacter baumannii and Enterobacteriaceae, which have been responsible for numerous outbreaks in neonatal units. The polymyxins (colistin and polymyxin B) are considered to be the last resort antibiotics for treating such infections. However, pharmacokinetic and pharmacodynamic data on the use of polymyxins in neonates and children are very limited, and there are safety con

INTRODUCTION: Polymyxins are considered last-resort antibiotics for treating Gram-negative bacterial infections. The alarming global rise in polymyxin-resistant bacteria coupled with the slowness of conventional antimicrobial susceptibility testing methods leads to an unmet need for rapid susceptibility testing to facilitate timely and targeted therapeutic interventions.
OBJECTIVES: In this study, we aimed to develop novel polymyxin-derived fluorescent probes and improve the identification of polymyxin resistance by using these pathway-specific tools.
METHODS: We designed, synthesized, and p

OBJECTIVES: Extremely multiresistant strains of Enterobacteriaceae, such as those of Escherichia coli and Klebsiella pneumoniae, are emerging and spreading at a worrisome speed. Polymyxins (polymyxin B, colistin) are used as last-line therapy against such strains, in spite of their notable nephrotoxicity that may even require discontinuation of the therapy. We have previously developed polymyxin derivatives NAB739 and NAB815 that are better tolerated in cynomolgus monkeys than polymyxin B and are, in contrast to polymyxin B, excreted in the cynomolgus urine to a very significant degree. The rising incidence of multidrug-resistant Gram-negative infections has reaffirmed the importance of polymyxins (polymyxin B (PMB), colistin methanesulfonate (CMS), and colistin sulfate (CS)) as essential last-line treatments. This review assesses the pharmacokinetic (PK) and population pharmacokinetic (PopPK) data for polymyxins in special populations, aiming to establish agent-specific PK profiles and optimize dosing strategies. A total of 72 articles published between 2005 and 2025 were reviewed, with 36 focusing on the PK of PMB, 30 on CMS, and 6 on CS. PopPK analyses constituted the majo

Polymyxin B (PMB) is a potent antibiotic targeting gram-negative bacteria and is associated with serious side effects including nephrotoxicity, neurotoxicity, and hypersensitivity reactions. PMB is a therapeutic option for the management of infections caused by multi-drug-resistant (MDR) bacteria and used in combination with other antibiotics when options are limited. We describe the case of a 30-year-old female patient with a complex medical history who underwent a multi-visceral transplantation complicated by intra-abdominal infections. Subsequently, patient developed diffuse skin darkening

In this chapter, we systematically reviewed studies that assessed polymyxin's effectiveness and summarized results through meta-analysis. The outcomes addressed were all-cause mortality, assuming that for patients with severe multidrug-resistant infections survival is the most important outcome, and resistance development, important for future patients. Most clinical data on polymyxins in the literature are from retrospective, observational studies at high risk of bias. The majority of clinical studies were unpowered to examine mortality controlling for other risk factors. The studies had no c
