Polysomnography in children scheduled for adenotonsillectomy.

OBJECTIVE: Several studies suggest that a reliable diagnosis of childhood sleep-disordered breathing (SDB) requires polysomnography (PSG). We compared clinical and PSG-based diagnoses in children scheduled for adenotonsillectomy (AT). Parent responses on a validated Pediatric Sleep Questionnaire were used to determine which symptoms could help identify children with clinical diagnoses of SDB but normal PSG. STUDY DESIGN AND SETTING: Thirty-four children aged 5.0 to 12.9 years and scheduled for AT to treat clinically diagnosed sleep-disordered breathing underwent laboratory-based PSG. Results

[Do we need polysomnography in insomnia?].

The elimination of insurance coverage for polysomnography (PSG) in insomnia raised the question about the diagnostic value of this sole objective assessment of sleep. The methodology of PSG is described including the significant criteria for evaluating the recuperative value of sleep. Comparing clinical symptomatology and PSG shows, that the dominating symptoms are neither consistent nor specific, that the subjective evaluation of sleep by the patient is uncertain, and that extreme deviations may occur, especially in sleep state misperception. In the field of differential diagnosis, overlappin

Polysomnography and neurocognition in children with Fontan circulation.

Children with single-ventricle post-Fontan palliation often struggle with chronic hypoxaemia and adverse neurodevelopmental outcomes. This brief report describes cardiorespiratory parameters obtained through polysomnography in 48 children and adolescents with Fontan circulation and investigates associations between polysomnography and neuropsychological testing in a subset of 26 patients. We found intermittent hypoxaemia was associated with parent-reported inattention symptoms, and severity of obstructive sleep apnoea with poorer visual-motor integration skills (all p < 0 .05). Utility of polysomnography in determination of laryngomalacia severity.

OBJECTIVE: To examine the efficacy of polysomnography in determining the severity of laryngomalacia in pediatric patients. METHODS: Prospective cohort study. Pediatric patients referred to our pediatric otolaryngology department with a polysomnogram already performed for a presumptive diagnosis of laryngomalacia were enrolled in the study. Patients with concurrent airway lesions or neuromuscular disorders were excluded. Patients underwent history, physical exam, and flexible fiberoptic laryngoscopy. These results were used to calculate a total laryngomalacia severity score. RESULTS: 25 pedia

Establishing a Role for Polysomnography in Hospitalized Children.

BACKGROUND: Children with medical complexity have a high prevalence of sleep disorders. However, outpatient polysomnography to evaluate for these conditions may be difficult to perform because of lack of skilled nursing care. The aim of this study was to explore polysomnography indications in hospitalized children and assess its impact on patient care. METHODS: Data from 85 inpatient polysomnographies of 70 children hospitalized between March and December 2014 were retrospectively collected. RESULTS: Sixty percent of patients were boys with ages 6.5 ± 6 years. Chronic respiratory failure was

Prolonged polysomnography in a case with recurrent hypersomnia.

An 18-year-old male patient with recurrent hypersomnia (RH) was evaluated using prolonged polysomnography (PSG). During symptomatic period (SMP), the patient showed both 'dissociated stage REM' (DREM), REM sleep without muscle atonia and 'dissociated stage 1' (DSt-1), and stage 1 sleep with rapid eye movement. These stages were observed in the morning or following daytime record. They decreased during asymptomatic period (ASMP). It has been said that RH is caused by dysfunction of the hypothalamus and midbrain limbic system. The present result suggests also that RH involves dysfunction of the

Utilization of polysomnography in the assessment of sleep disorders.

Polysomnography is an indispensable tool in dealing with sleep disorders, but because of its inherent logistical complexity and expense, it must be used with discretion. Many sleep disorders can be diagnosed clinically by the astute physician and treated empirically with good results. On the other hand, there are numerous circumstances in which the best clinical judgment indicates a polysomnographic evaluation. The ubiquity of sleep complaints in medical practice, and the knowledge that they may in some instances be associated with life-threatening medical complications, compel the modern clin

The clinical yield of repeated pediatric polysomnographies.

PURPOSE: The role of repeat polysomnography (PSG) in children is poorly defined, with limited evidence guiding indications, timing, and expected diagnostic yield. The aim of this study was to characterize real-world use and yield of repeat PSGs, with emphasis on adenotonsillectomy (AT) status and background diagnoses. METHODS: A retrospective observational study of children 0-18 years old who completed two or more PSGs at The Hadassah pediatric sleep laboratory between 2019 and 2023. [Application of Polysomnography in Common Neurodegenerative Diseases].

At present, the etiology and pathogenesis of most neurodegenerative diseases are still not fully understood, which poses challenges for the prevention, diagnosis, and treatment of these diseases. Sleep disorders are one of the common chief complaints of neurodegenerative diseases. When patients suffer from comorbid sleep disorder and neurodegenerative diseases, the severity of their condition increases, the quality of their life drops further, and the difficulty of treatment increases. A large number of studies have been conducted to monitor the sleep of patients with neurodegenerative disease

Polysomnography Applied to Patients with Tinnitus: A Review.

Introduction  Tinnitus has been defined as an "auditory phantom perception," meaning that tinnitus results from an abnormal activity within the nervous system, in the absence of any internal or external acoustic stimulation. About 10 to 15% of the adult population is affected by tinnitus, and a relevant percentage of tinnitus sufferers experience symptoms severe enough to significantly affect quality of life, including sleep disturbances, work impairment, and, in some cases, psychiatric distress. The self-rated complaints about tinnitus focus on emotional distress, auditory perceptual difficul

[Experience with polysomnography in the diagnosis of sleep apnea].

Whole-night recording of 9-30 parameters and calculation of various indices allow differentiation of various sleep related breathing disorders and estimation of their severity (Fig. 1, 2). Parallel monitoring of changes in ventilation, arterial oxygen saturation, ECG, EEG, EMG and others, give a possibility to appreciate the gradual development of cardiovascular, neuropsychic, endocrine-metabolic and other consequences of sleep related breathing disorders, their dependence on different phases and stages of sleep as well as their alterations by treatment.

[EEG and polysomnography findings in patients with dyskinesia].

It is known that EEG findings reveal various abnormalities in patients with involuntary movement. But these findings are not specific. It has been reported to be related to myoclonus and spike. The other involuntary movement is unknown to the relation to EEG findings. The involuntary movement usually disappears during sleep, but a certain involuntary movement appears only during sleep. In the patient with Huntington's chorea and dystonia musculorum deformans, PSG reveals an increase in interspersed wakefulness, decrease of deep sleep and prolongation of REM latency. Periodic limb movement and

Chronic fatigue, unrefreshing sleep and nocturnal polysomnography.

BACKGROUND AND PURPOSE: To investigate the complaint of unrefreshing sleep with study of sleep electroencephalogram (EEG) in patients with chronic fatigue. PATIENTS AND METHODS: Fourteen successively seen patients (mean age: 41.1 9.8) who complained of chronic fatigue but denied sleepiness and agreed to participate were compared to 14 controls (33.6+/-10.2 years) who were monitored during sleep recorded in parallel. After performing conventional sleep scoring we applied Fast Fourier Transformation (FFT) for the delta 1, delta 2, theta, alpha, sigma 1, sigma 2, beta EEG frequency bands. The pr

Diagnosing rhythmic movement disorder with video-polysomnography.

We evaluated the utility of accurate clinical and electrophysiologic characterization in the diagnosis of the rhythmic movement disorder. Seven children with an age range of 1-12 years, referred for evaluation of relatively violent nocturnal behaviors, were clinically assessed during split-screen, video-polysomnographic monitoring sessions, as they experienced unusual nocturnus movements. Differential diagnoses included self-injurious waking behaviors, seizures, and parasomnias such as somnambulism (sleepwalking), pavor nocturnus (night terrors), and the rhythmic movement disorder (headbanging

Polysomnography findings in spinocerebellar ataxia type 6.

Spinocerebellar ataxia type 6 (SCA6) is usually described as a pure ataxia syndrome. However, SCA6 patients may have sleep complaints. In this paper, sleep disorders were investigated in patients with SCA6. Twelve SCA6 patients and 12 subjects matched by gender, age and body mass index (control group) underwent polysomnography and clinical investigation for sleep disorders. SCA6 had a higher frequency of snoring (P = 0.01), a higher index of awakening due to respiratory events (P = 0.003) and central apnea events during sleep (P = 0.024), a longer sleep Stage N1 (P = 0.02) and a lower sleep St
