A study of patients with long-term bile flow after hepatic portoenterostomy for biliary atresia.

Thirty-five cases of biliary atresia that demonstrated the creation of internal fistula between intrahepatic bile ducts at the portahepatis and intestine and showed long-term bile flow after our hepatic portoenterostomy were investigated from various aspects. These 35 cases were divided into Group A cases, in which jaundice disappeared within three months after surgery, and Group B cases, in which persistent jaundice was seen more than three months after surgery. (1) Severe liver fibrosis and degeneration of intrahepatic bile ducts were more often seen in Group B cases than in Group A cases. (

Improved results with hepatic portoenterostomy: a reassessment of its value in the treatment of biliary atresia.

As reported in 1974, the initial experience at the Oklahoma Children's Memorial Hospital with the Kasai procedure for biliary atresia was unsatisfactory. A subsequent series of 20 patients, in which 50% of the patients are alive and improved and 25% jaundice-free, is described. Modifications of the initial operative technique have been utilized. Postoperative complications in the ten children with sustained bile flow included cholangitis in five, hyponatremia in four, esophageal variceal hemorrhage in two, stomal bleeding in two, and gallbladder conduit malfunction in two patients. The improve

Simultaneous Hepatic Artery and Portal Vein Thrombosis After Kasai Portoenterostomy: A Rare but Life-Threatening Complication.

Biliary atresia (BA) is a progressive neonatal bile duct disease treated with the Kasai procedure; however, complications may necessitate liver transplantation. We report a rare case of simultaneous hepatic artery and portal vein thrombosis in a term infant after Kasai surgery, leading to acute liver failure and urgent transplantation. Rapid deterioration with metabolic acidosis, hypovolemic shock, and coagulopathy suggested "liver infarction," confirmed by imaging, while thrombolysis failed to restore full viability. This case highlights the importance of recognizing vascular complications in

Early and Peri-operative Prognostic Indicators in Infants Undergoing Hepatic Portoenterostomy for Biliary Atresia: a Review.

PURPOSE OF REVIEW: Biliary atresia is the most common indication for liver transplantation among children. In recent years, prospective, multi-centre collaboration has been underway with the aim of providing high-quality data on the natural history of the condition, prior to and following hepatic portoenterostomy. RECENT FINDINGS: There is increasing evidence that specific histological findings, and age, at the time of portoenterostomy have relevance as prognostic indicators. Recent data suggest that the sub-type of biliary atresia, its co-existence with other anomalies and concurrent infecti

Hepatic phenotypes of HNF1B gene mutations: a case of neonatal cholestasis requiring portoenterostomy and literature review.

Hepatocyte nuclear factor 1-β (HNF1B) defects cause renal cysts and diabetes syndrome (RCAD), or HNF1B-maturity-onset diabetes of the young. However, the hepatic phenotype of HNF1B variants is not well studied. We present a female neonate born small for her gestational age [birth weight 2360 g; -2.02 standard deviations (SD) and birth length 45 cm; -2.40 SD at the 38(th) gestational week]. She developed neonatal cholestasis due to biliary atresia and required surgical intervention (portoenterostomy) when 32-d old. Following the operation, icterus resolved, but laboratory signs of liver dysfunc

Effects of post-hepatic portoenterostomy adjuvant therapy on liver transplantation in children with biliary atresia: A systematic review.

OBJECTIVES: Biliary atresia (BA) is a cholangiopathy characterized by obstruction of the intrahepatic and extrahepatic bile ducts. Hepatic portoenterostomy (HPE) is the primary palliative treatment and there is still an urgent need to improve post-HPE management. This study aims to identify post-HPE adjuvant therapies associated with reducing or delaying the need for liver transplantation in children with BA. METHODS: This systematic review was registered on the PROSPERO platform (ID: CRD42024553671). Studies were extracted from the following databases: PubMed, Web of Science, Lilacs, and Sci

Long-term prognosis in biliary atresia after hepatic portoenterostomy: analysis of 35 patients who survived beyond 5 years of age.

The long-term prognosis in 35 patients with biliary atresia, who had undergone successful hepatic portoenterostomy and survived beyond 5 years of age, was studied. Eighteen (51%) patients developed ascending cholangitis after surgery. Jaundice recurred or increased in half of the patients, mainly as a result of ascending cholangitis. Another serious problem was the development of portal hypertension. Esophageal varices developed in 16 (46%) patients, with rupture and massive hemorrhage in seven. Leukopenia resulting from hypersplenism was common. Eighteen patients had WBC counts less than 5000

APRi predicts native liver survival by reflecting portal fibrogenesis and hepatic neovascularization at the time of portoenterostomy in biliary atresia.

BACKGROUND: Aspartate aminotransferase-to-platelet ratio index (APRi) may be useful noninvasive prognostic tool in biliary atresia (BA). We studied whether APRi predicts native liver survival and parallels biochemical and immunohistological signs of liver injury and fibrogenesis at the time of Kasai portoenterostomy (PE). METHODS: Serum and liver specimens were obtained at PE from 29 BA patients for liver biochemistry including APRi, histology and immunohistochemical analysis of collagen 1, α-SMA and CD34. APRi values were related to native liver survival and other clinical data as well as se

Indication for redo hepatic portoenterostomy for insufficient bile drainage in biliary atresia: re-evaluation in the era of liver transplantation.

To determine the role of redo hepatic portoenterostomy (HPE) in biliary atresia (BA) patients with insufficient bile excretion after the initial HPE, 25 patients (type I, correctable: 2; type III, uncorrectable: 23) undergoing the initial HPE at 25 to 119 days of age were studied. Four patients achieved disappearance of jaundice (total bilirubin [T.Bil] < 2 mg/dl) postoperatively. A redo HPE was performed at 2 to 8 months of age with sufficient and extensive removal of granulation and scar tissue at the hepatic hilum. Five patients became free of jaundice in 3 to 6 months (group 1), while the

Hepatic Hilar Lymph Node Reactivity at Kasai Portoenterostomy for Biliary Atresia: Correlations With Age, Outcome, and Histology of Proximal Biliary Remnant.

We hypothesized that if infection is the proximate cause of congenital biliary atresia, an appropriate response to antigen would occur in lymph nodes contiguous with the biliary remnant. We compared the number of follicular germinal centers (GC) in 79 surgically excised hilar lymph nodes (LN) and 27 incidentally discovered cystic duct LNs in 84 subjects at the time of hepatic portoenterostomy (HPE) for biliary atresia (BA) to autopsy controls from the pancreaticobiliary region of non-septic infants >3 months old at death. All 27 control LN lacked GC, a sign in infants of a primary response to

Robot-assisted Cyst Excision, Ductoplasty, and Embedding Portoenterostomy for Choledochal Cysts with a Small Caliber Hepatic Duct less than 6 mm.

Anastomotic stricture is a primary concern following the surgical treatment of choledochal cysts (CC), particularly in pediatric patients with narrow bile ducts (<6 mm), and remains the main indication for re-operation. Robotic-assisted surgery (RAS), with its superior 3D visualization and instrument dexterity, offers an ideal platform for the complex reconstruction required to prevent this complication. This study presents a robot-assisted modified technique designed to mitigate the risk of stricture, as demonstrated in a 34-month-old female with a Type Ia CC. The procedure involves complete

Preoperative porta hepatis-to-hepatic surface ratio on 24-h delayed hepatobiliary scintigraphy is associated with jaundice clearance after Kasai portoenterostomy.

PURPOSE: Biliary atresia (BA) often requires liver transplantation despite Kasai portoenterostomy (KPE). The prognostic role of hepatobiliary scintigraphy remains unclear. METHODS: Twenty-three infants undergoing KPE between 2017 and 2024 were retrospectively analyzed. Patients without 6-h bowel activity on technetium-99 m PMT hepatobiliary scintigraphy underwent 24-h delayed imaging. The primary endpoint was jaundice clearance (total bilirubin < 2 mg/dL within 3 months). Ancillary analyses included an internally validated receiver operating characteristic (ROC) cutoff and correlation of the

Hepatic blood flow velocity before and 3 months after Kasai portoenterostomy is a prognostic indicator for native liver survival in biliary atresia.

UNLABELLED: Biliary atresia (BA) is a progressive neonatal cholangiopathy causing fibrosis, cirrhosis, and portal hypertension. Kasai portoenterostomy (KPE) restores bile drainage and prolongs native liver survival (NLS), yet BA remains the leading cause of pediatric liver transplantation. Therefore, reliable predictors of KPE failure are needed. This study evaluated hepatic blood flow velocities (HBFV) via ultrasound as potential non-invasive predictors of NLS after KPE. We retrospectively analyzed 163 BA patients treated between 2008 and 2023 at two German tertiary centers. Standardized Dopp
