Agenesis of the posterior cruciate ligament (PCL) is a rare disorder often found in conjunction with various other abnormalities within the knee. A 15-year-old adolescent boy presented with an absent PCL, leading to intermittent symptoms. At the age of 20 years, the patient underwent arthroscopically assisted PCL reconstruction. The patient was permitted to weightbear immediately after the procedure and started physical therapy 4 weeks after the procedure. No complications were encountered at the 2-year follow-up.

The optimal treatment of posterior cruciate ligament ruptures remains controversial despite numerous recent basic science advances on the topic. The current literature on the anatomy, biomechanics, and clinical outcomes of posterior cruciate ligament reconstruction is reviewed. Recent studies have quantified the anatomic location and biomechanical contribution of each of the 2 posterior cruciate ligament bundles on tunnel placement and knee kinematics during reconstruction. Additional laboratory and cadaveric studies have suggested double-bundle reconstructions of the posterior cruciate ligame…

Injuries to the posterior cruciate ligament (PCL) and methods of treatment of the injured PCL have historically been surrounded by controversy in the orthopedic community. The infrequent occurrence of PCL injuries and PCL-based, multiple ligament knee injuries has led to limitations in clinical studies and a subsequent lag in basic science and clinical research compared with that for other ligamentous injuries. In recent years, new studies have elucidated the biomechanical function and complex anatomy of the PCL leading to an increased interest in research, understanding, and treatment of thes…

Successful posterior cruciate ligament (PCL) reconstruction is challenging because of the complex structures and difficult reconstruction techniques that are required. The reported results have been inconsistent. Variables that affect the results of surgery to restore PCL function include combined associated ligaments injury, difficulty to duplicate PCL anatomy, wide variation in broad femoral insertion footprint, difficulty in accurate placement of the transtibial tunnel, tunnel erosion, high internal graft stresses and graft elongation. The outcome of conservative treatment of isolated PCL i…

Multiligamentous knee injuries are challenging to treat and diagnose. Posterior cruciate ligament (PCL) injuries are commonly found in the constellation of injuries included in a multiligamentous knee injury and are caused by a posteriorly directed force on the proximal tibia with relation to the femoral condyles. A thorough history and physical examination should be performed to evaluate for associated neurovascular injuries and associated ligamentous, chondral, or bony injuries. Nonsurgical management is reserved for patients who are critically ill or have very low activity demands. Surgical…

INTRODUCTION: Posterior cruciate ligament (PCL) reconstruction using the remnant preserving technique may contribute to improved postoperative posterior stability, graft healing, and proprioception recovery. Although there have been several reports on remnant preserving PCL reconstruction, no study has yet evaluated the proprioceptive functions before and after PCL reconstruction with remnant preservation. The purpose of this study is to retrospectively evaluate the clinical outcomes and proprioceptive function after isolated single-bundle PCL reconstruction with remnant preservation for chron…

Knee dislocation is a rare but severe injury that involves damage to the anterior cruciate ligament (ACL), posterior cruciate ligament (PCL), lateral or medial ligamentous structures, and other soft tissues or bony structures surrounding the knee joint. No consensus exists regarding the best treatment method. This work presents a method of 1-stage ACL and PCL reconstruction in which a contralateral quadriceps tendon-bone autograft with tibial inlay technique is used for the PCL and contralateral hamstring tendon autograft with suspension fixation is used for the ACL. After harvesting grafts, t…

ACL-PCL-posterolateral corner injuries most frequently are seen in multiple trauma patients but do occur in the athletic injury population. Acute three-ligament-injured knees may have been tibiofemoral dislocations with spontaneous reduction in the field. Careful documentation of the neurovascular status is essential in these cases to avoid the complications associated with limb ischemia. Systematic evaluation of these patients with history, physical examination, imaging studies, examination under anesthesia, and diagnostic arthroscopy will aid in the correct diagnosis and treatment plan formu

PURPOSE: Ligaments are fibrous connective tissues that transmit force from bone to bone. One of the most important ligaments in the human body is the knee-joint ligament, which is exposed to large and dynamic forces during daily activities. Owing to their frequent injuries, cruciate ligaments are among the most studied in the current literature. Thus far, research has focused mainly on the study of individual ligament structures and less frequently on analysing entire bone-ligament systems. Therefore, this study examines the mechanical properties of the entire cruciate ligament system.

Knee dislocations are rare but devastating injuries. The ACL-PCL-lateral side injury combination is representative of the challenges these injuries present. Early management is focused on vascular integrity. When possible, acute repair and reconstruction within 3 weeks from injury is preferred. Chronically deficient knees generally will require lateral side reconstruction rather than repair and may require limb realignment. Addressing all injured structures is imperative to afford the best chance at a reasonable outcome. Good results with surgery are possible, but some degree of persistent dis

Injuries involving the anterior cruciate, posterior cruciate, and medial collateral ligaments represent one combination of injuries representing knee dislocation. Prompt reduction and neurovascular evaluation are necessary when treating these injuries. Operative management involving reconstruction and repair of the injured structures produces optimal results. This article outlines the principles involved in evaluating and managing these injuries, combined with specifics of the authors' treatment approach, and review of the literature.

Simultaneous ACL and PCL ruptures are rare but serious injuries resulting in distinct instability of the knee joint followed by an early degenerative arthritis. This combined trauma, which is often accompanied by additional ligament lesions, originates from a knee dislocation. While the conservative treatment of this complex instability is abandoned, the operative procedures are not yet standardised. The timing of the cruciate ligament reconstruction depends on the additional injuries, but generally the postprimary treatment is performed. Autografts and allografts, which can be also combined,

Isolated posterior cruciate ligament injuries are rare and their management is controversial. Bony avulsion fractures have commonly been repaired with open reduction and internal fixation using the posterior approach described by Trickey. However, this approach does not lend itself to detection and repair of associated injuries. We report the case of a posterior cruciate ligament avulsion that was arthroscopically reduced and stabilized with cannulated screws. This technique provided rigid fixation for early rehabilitation and allowed concomitant arthroscopic examination of the knee for any as…

Congenital absence of the cruciate ligaments is a very rare condition, with most reported cases being related to the anterior cruciate ligament (ACL). Posterior cruciate ligament (PCL) congenital abnormalities have been described as almost always being associated with other anomalies. We present a case of a patient with lateral knee pain and subjective instability sensation after a low-energy trauma secondary to a lateral meniscus tear, with absence of the PCL on MRI examination and thickened meniscofemoral ligaments, with anterior and superior insertion at the femur, in the footprint of PCL. …

PURPOSE OF THE STUDY: Injury to the posterior cruciate ligament (PCL) is relatively rare and, if combined with avulsion fracture of the PCL insertion site, it accounts for only a small number of knee injuries. This is why PCL avulsion fracture is an easily missed diagnosis resulting in knee instability and arthritis development. The aim of this study was to show the necessity of surgical treatment of these injuries.
MATERIAL: Between January 2004 and September 2005, four patients with PCL injury underwent surgery. Three had avulsion fracture at the PCL insertion site, and in one the avulsion …

The treatment of posterior cruciate ligament injuries remains controversial. There is no consensus on the ideal technique for reconstruction or the optimal graft design. Specifically, there is intense debate about which technique is superior: the open tibial inlay technique or the transtibial arthroscopic technique. We present a novel all-arthroscopic tibial inlay technique using a bifid bone-patellar tendon-bone allograft. This technique offers the advantages of the open tibial inlay technique, including avoidance of the killer turn and restoration of biomechanical stability. Furthermore, thi…
