Postmortem computed tomography (PMCT) of the brain has an important role in detection of subarachnoid hemorrhage (SAH), which has a high mortality rate. However, a phenomenon known as "pseudo-SAH," or high-attenuation areas along the cisterns mimicking SAH, may be seen on CT. The aim of this study was to evaluate the diagnostic accuracy of brain PMCT for SAH and to identify the characteristics of pseudo-SAH. Findings on PMCT (sulcal effacement, asymmetry, maximum thickness of SAH signs, presence of acute/subacute intraventricular/intraparenchymal hemorrhage) and clinical history (left ventricu

BACKGROUND: Postmortem magnetic resonance imaging (PMMRI) following fetal and perinatal demise is increasingly used in clinical practice as a non-invasive adjunct or alternative to autopsy. Imaging protocols vary widely between centers and there is currently no consensus.
OBJECTIVE: From expert consensus, we aimed to develop two PMMRI protocols for pragmatic clinical perinatal imaging: first, a standardized "minimal" PMMRI clinical protocol (with the minimum essential sequences needed per body part), and second, an "ideal" PMMRI clinical protocol with the ideal full protocol, time allowing.

The application of Artificial Intelligence (AI) in forensic science offers new opportunities for the automated detection of injuries in postmortem analysis. This study focuses on the semantic segmentation of two significant types of injuries-bruises and abrasions. A dataset of postmortem injury images was collected, followed by the development of appropriate data preprocessing and annotation techniques to train and evaluate AI models. Three deep learning architectures-U-Net, FPN, and LinkNe-were implemented, using EfficientNetB3 and ResNet50 as backbone networks. An optimisation strategy was e

We explored the possibility of using postmortem MRI for the diagnostics of lung hypoplasia associated with innate diaphragmatic hernia in neonates. The main experimental group consisted of 17 newborns with innate diaphragmatic hernia including 10 non-operated newborns and 7 newborns died after surgery for innate diaphragmatic hernia. It was demonstrated that postmortem MRI allows objective quantitative assessment of the absolute and relative dimensions of the lungs in the thoracic cavity and thereby reveals their hypoplasia, which contributes to the determination of tanatogenesis. In forensic investigations, it is important to detect traumatic axonal injuries (TAIs) to reveal head trauma that might otherwise remain occult. These lesions are subtle and frequently ambiguous on macroscopic evaluations. We present a case of TAI revealed by pre-autopsy postmortem magnetic resonance imaging (PMMR). A man in his sixties was rendered unconscious in a motor vehicle accident. CT scans revealed traumatic mild subarachnoid hemorrhage. Two weeks after the accident he regained consciousness, but displayed an altered mental state. Seven weeks after the accident, he suddenly died in ho

BACKGROUND: Postmortem imaging is becoming a more acceptable part of perinatal autopsy, particularly for early gestation or small fetuses in which conventional autopsies may be challenging and rejected by parents.
OBJECTIVE: This study aimed to establish the relative diagnostic yield of microfocus computed tomography as part of a less invasive autopsy in fetuses at <24 weeks of gestation.
STUDY DESIGN: This was a single-center, retrospective study of 7 years of 1007 consecutive unselected perinatal autopsies (2016-2023) of fetuses with a body weight of <300 g. Microfocus computed tomography

The use of postmortem computed tomography (CT) has been described in many articles concerning gunshot injuries. Postmortem magnetic resonance imaging (MRI) for better assessment of soft tissue injuries has also been mentioned in the literature, albeit much less often. The use of postmortem MRI for abdominal gunshot wounds has not been previously presented in the literature. The present case report describes the findings of an abdominal gunshot wound detected by postmortem CT and MRI, followed by an autopsy. The main imaging findings on CT were a hyperdense ring at the entrance wound, which ind

Postmortem imaging consists in the non-invasive examination of bodies using medical imaging techniques. However, gas volume quantification and the interpretation of the gas collection results from cadavers remain difficult. We used whole-body postmortem multi-detector computed tomography (MDCT) followed by a full autopsy or external examination to detect the gaseous volumes in bodies. Gases were sampled from cardiac cavities, and the sample compositions were analyzed by headspace gas chromatography-mass spectrometry/thermal conductivity detection (HS-GC-MS/TCD). Three categories were defined a

Blood aspiration is a significant forensic finding. In this study, we examined the value of postmortem computed tomography (CT) imaging in evaluating findings of blood aspiration. We selected 37 cases with autopsy evidence of blood in the lungs and/or in the airways previously submitted to total-body CT scanning. The CT-images were retrospectively analyzed. In one case with pulmonary blood aspiration, biopsy specimens were obtained under CT guide for histological examination. In six cases, CT detected pulmonary abnormalities suggestive of blood aspiration, not mentioned in the autopsy reports.

OBJECTIVES: Postmortem magnetic resonance imaging (MRI) may be an alternative to conventional autopsy. However, it is unclear how confident radiologists are in reporting such studies. We sought to determine the confidence with which radiologists report on various fetal organs by developing a scale to express their confidence of normality and abnormality, and to place this in the context of a pathological diagnosis of whether the organ was in fact normal or abnormal.
METHODS: Thirty fetuses, aged 16-39 gestational weeks and weighing 61-3270 g, underwent postmortem MRI prior to conventional aut

PURPOSE: To assess postmortem radiologic and pathologic findings by using modern imaging and autopsy techniques in two recent cases of judicial hangings.
MATERIALS AND METHODS: Cervical spine radiography; computed tomography (CT) of the head, neck, and chest; magnetic resonance (MR) imaging of the head and cervical spine; and vertebral angiography were followed by a complete autopsy, including head and neck dissection.
RESULTS: The first case (subaural knot) showed cervical spine ligamentous injury and partial disruption of the vertebral arteries without vertebral subluxation or injury to th

Skeletal remains often have missing mandibles owing to the environment in which the corpse was placed or damage caused by an animal. Loss of the mandible reduces the accuracy of skull identification. Although several studies have validated methods for estimating mandibular morphology from conventional anthropological measurements using skull specimens, there are no reports using three-dimensional computed tomography (3DCT) images. Here, we examined methods for establishing the mandibular morphology from the remaining skulls using postmortem computed tomography (PMCT) images. We used PMCT image

Postmortem computed tomography (PMCT) is becoming used more commonly in routine forensic investigation. CT is sensitive for detection of metal foreign bodies. Here we report a case of suicide due to self-ignition of kerosene that the victim had poured over herself. Prior to autopsy, PMCT detected tiny radiopaque particles arranged in a row in the surface of the back and either thigh, together with a series of similar particles under the skin lateral to the breasts or the bilateral inguinal region. At autopsy, external examination revealed third-degree burns involving charred tissues all over t

The locus coeruleus (LC), a tiny nucleus in the brainstem and the principal site of noradrenaline synthesis, has a major role in regulating autonomic function, arousal, attention, and neuroinflammation. LC dysfunction has been linked to a range of disorders; however particular interest is given to the role it plays in Alzheimer's disease (AD). The LC undergoes significant neuronal loss in AD, thought to occur early in the disease process. While neuronal loss in the LC has also been suggested to occur in aging, this relationship is less clear as the findings have been contradictory. LC density

Postmortem magnetic resonance imaging (MRI) is rarely used for the radiologic assessment of gunshot injuries, although it has clear advantages over postmortem computed tomography (CT) with regard to the imaging of soft tissue injuries. Another benefit in using MRI is that lodged projectiles composed of nonferromagnetic material such as lead present only marginal metal artifacts compared with severe artifacts on CT. This case report presents CT and MRI findings in a case with two gunshot wounds to the neck: a perforating wound and a nonperforating wound with a lead bullet lodged in the cervical

The rapid further development of computed tomography (CT) and magnetic resonance imaging (MRI) induced the idea to use these techniques for postmortem documentation of forensic findings. Until now, only a few institutes of forensic medicine have acquired experience in postmortem cross-sectional imaging. Protocols, image interpretation and visualization have to be adapted to the postmortem conditions. Especially, postmortem alterations, such as putrefaction and livores, different temperature of the corpse and the loss of the circulation are a challenge for the imaging process and interpretation

Postmortem imaging has become an integral component of modern forensic pathology, with postmortem computed tomography (PMCT) and magnetic resonance (PMMR) imaging now routinely supplementing or, in selected cases, partially replacing conventional autopsy. Despite these advances, postmortem imaging remains predominantly static, capturing anatomy and injuries at a single point in time after death. In contrast, clinical radiology has increasingly embraced four-dimensional (4D) imaging approaches that incorporate time as a diagnostic dimension, enabling the assessment of motion, functional change,

RATIONALE: Postmortem imaging (PMI), including computed tomography (PMCT), postmortem computed tomography angiography (PMCTA), and postmortem magnetic resonance imaging (PMMRI), is rapidly becoming effective and a practical method in forensic medicine. This study aimed to present a specific forensic case in which the PMI approach and its applications were used.
PATIENT CONCERNS: A 40-year-old male patient had moderate unilateral nose bleeding constantly 10 times after suffering from a head injury induced by a car accident. After a bilateral massive nose bleeding for the last time, he died fro

The value of iron-based MRI changes for the diagnosis and staging of Alzheimer's disease (AD) depends on an association between cortical iron accumulation and AD pathology. Therefore, this study determined the cortical distribution pattern of MRI contrast changes in cortical regions selected based on the known distribution pattern of tau pathology and investigated whether MRI contrast changes reflect the underlying AD pathology in the different lobes. T2*-weighted MRI was performed on postmortem cortical tissue of controls, late-onset AD (LOAD), and early-onset AD (EOAD) followed by histology
