BACKGROUND/OBJECTIVES: Assisted living (AL) is an important provider of long-term residential care to people with dementia, but little research has used clinician's records-arguably the most reliable and valid source of medically related information. This article uses clinician records to examine the prevalence of dementia, treatment with dementia-specific medications and antipsychotic medications, and how prescribing varies by assisted living residence (ALR) and resident characteristics.
DESIGN: Analysis of medical records from a long-term care medical practice.
SETTING: Ninety ALRs that ha

Portal hypertension is a hemodynamic consequence of altered mesenteric circulation. The chief drivers of portal hypertension are rising hepatic resistance and venous remodeling. A major challenge in treating patients with portal hypertension is early diagnosis because most patients progress through disease asymptomatically. Noninvasive tests such as serologic markers and elastography are the basis of determining if patients have clinically significant portal hypertension. If identified early, patients placed on carvedilol and who undergo etiologic cure may be able to delay or avert decompensat

More than 3 million Australians are estimated to have migraine disorders, and over a quarter of a million Australians are estimated to have medication overuse headache (MOH). The personal, societal and economic burden of MOH is high. MOH impacts an individual's ability to work or study, care for family or themselves, culminating in poor quality of life. Accurate and timely diagnosis and treatment of MOH are imperative. Withdrawal failures and relapse rates are high in MOH. Treatment of MOH is aimed at ceasing medication overuse and reducing monthly migraine days with the aim of achieving a pat

BACKGROUND: Dengue has emerged as a major public health problem in Bhutan, with increasing incidence and widening geographic spread over recent years. This study aimed to investigate the knowledge and clinical management of dengue among medical practitioners in Bhutan.
METHODS: We administered a survey questionnaire to all practitioners currently registered under the Bhutan Medical and Health Council. The questionnaire contained items on four domains including transmission, clinical course and presentation, diagnosis and management, and surveillance and prevention of dengue. Participants were

There is a specific requirement for undergraduate dental students to be trained in the management of medical emergencies that may arise in dental practice. This paper describes a practical skills course that has been developed specifically to fulfil this requirement. The rationale, course structure, methodology of delivery and assessment methods are discussed in detail.

Using a nationally representative sample of office-based physicians, the management practices of pediatricians, psychiatrists, and family practice physicians were investigated. The major aims were to determine (1) what types of services these physicians were providing to children who received stimulants, (2) what factors predicted receipt of stimulants, and (3) whether these practices were concordant or discordant with professional consensus on diagnosis and treatment of attention-deficit hyperactivity disorders (ADHD). Prescribing and management practice data from the 1995 National Ambulatory

BACKGROUND: Paediatric pain is frequently under-managed in prehospital care despite the known consequences. Practitioner-related factors, during transition from student to independent practice, influence pain management decisions.
AIM: This study aimed to explore and describe the elements influencing senior undergraduate emergency medical care students' self-reported attitudes, perceived readiness to practise and confidence in managing paediatric pain in South Africa and to identify self-reported barriers and enablers shaping their approach to paediatric pain management.
SETTING: The study w

Over the past two decades, medical methods of abortion have been developed throughout the world and are now used in the United States. Medical abortion, which involves the use of medications to induce an abortion rather than a surgical abortion, is an option for women who wish to terminate a pregnancy up to 63 days of gestation (calculated from the first day of the last menstrual period). Medical abortions currently account for 6% of all abortions in the United States (1). The purpose of this document is to present evidence of the effectiveness, benefits, and risks of medical abortion and prov

PURPOSE: Type 2 diabetes mellitus (T2DM) is a complex metabolic disorder that affects almost 24 million Americans. Healthcare providers often do not initiate and/or intensify therapy appropriately during patient visits, which may be due, in part, to a lack of understanding of the new diabetes medications. This review focuses on means by which primary care nurse practitioners (NPs) might evaluate the utility of pharmacologic agents based upon their relation to the pathogenesis of T2DM.
DATA SOURCES: The evidence used in developing this review included evidence-based reviews, clinical trials, c

Patient blood management (PBM) is a patient-centered, systematic, evidence-based approach to improve patient outcomes by managing and preserving a patient's own blood, while promoting patient safety and empowerment. Globally, PBM guidelines have been developed locally in several countries and societies based on the expertise and resources of each. In Lebanon, the presence of prevalent iron deficiency anemia and hemoglobinopathies, together with scarce resources and economic constraints, necessitate generating local PBM guidelines to improve outcomes, cost-effectiveness, and safety. INTRODUCTION: Non-medical use of prescription drugs (NMUPD) has reached epidemic proportions in the U.S. With approximately one in five high school students engaging in NMUPD, it is important to understand behavioral correlates.
METHODS: Data were combined from the 2011 and 2013 cycles of the Youth Risk Behavior Survey, a nationally representative, cross-sectional survey. After restricting the analytic sample to students who reported a weight loss goal of either staying the same weight or losing weight, logistic regression models were used to estimate adjusted prevalence ratios and 95% CIs fo

Inpatient hyperglycemia is common and is associated with an increased risk of hospital complications, higher healthcare resource utilization, and higher in-hospital mortality rates. Appropriate glycemic control strategies can reduce these risks, although hypoglycemia is a concern. In critically ill patients, intravenous (IV) insulin is most appropriate, with a starting threshold no higher than 180 mg/dL. Once IV insulin is started, the glucose level should be maintained between 140 and 180 mg/dL. In noncritically ill patients, basal-bolus regimens with basal, prandial, and correction component

Exercise-associated hyponatremia (EAH) is defined by a serum or plasma sodium concentration below the normal reference range of 135 mmol·L-1 that occurs during or up to 24 h after prolonged physical activity. It is reported to occur in individual physical activities or during organized endurance events conducted in environments in which medical care is limited and often not available, and patient evacuation to definitive care is often greatly delayed. Rapid recognition and appropriate treatment are essential in the severe form to increase the likelihood of a positive outcome. To mitigate the r

BACKGROUND: Fistulas occur in about 25% of patients with Crohn's disease (CD) and can be difficult to treat. The aim of this consensus was to provide guidance for the management of patients with perianal fistulizing CD.
METHODS: A systematic literature search identified studies on the management of fistulizing CD. The quality of evidence and strength of recommendations were rated according to the Grading of Recommendation Assessment, Development and Evaluation (GRADE) approach. Statements were developed through an iterative online platform using a modified Delphi process, then finalized, and

This is a retrospective analysis of 82 patients undergoing thyroid operations in a general medical practice hospital in sub-Saharan Africa. All patients complained of neck swelling, and in 75% of patients this was the only symptom. Indications for operation included cosmesis in 74%, suspected malignancy in 13%, hyperthyroidism in 7% and pressure symptoms in 5%. Operative complications included recurrent laryngeal nerve injury in two (2.4%), wound haematoma in two (2.4%), wound infection in three (3.6%) and hypoparathyroidism in one (1.2%). There were no deaths and no instances of thyroid storm

Considerable progress has been made regarding pathophysiology, diagnosis, and medical prevention of recurrent renal stone formation. The medical approach is not applied widely because of the availability of extracorporeal shockwave lithotripsy and the complexity of medical diagnostic and treatment modalities. In this review, a simplified program for the medical management of stones is described. From analysis of stone risk factors in 24-hour urine specimens, uncomplicated calcium stone disease is separated from other stone diseases. The uncomplicated calcium stone disease, comprising the illne
