Effect of clinical pharmacists on drug prescribing in a primary-care clinic.

The effect of clinical pharmacy services on prescribing patterns and drug costs for nonsteroidal anti-inflammatory drugs (NSAIDs) and salicylates in a primary-care clinic operated by a health-maintenance organization (HMO) was studied. Two pharmacists provided clinical services to a randomly selected cluster of family practice physicians in the HMO for six months. A second family practice cluster served as a control group. The pharmacists alerted prescribers to the availability of low-cost alternatives (such as ibuprofen and salicylates) and reviewed the medication profiles of patients receivi

Pharmaceutical practices before and throughout the opioid crisis: A scoping review.

METHODS: Opioid misuse has reached epidemic status in many countries. This crisis-recognized since 2014-questions the practices of prescribing and dispensing. Did this public health issue change pharmaceutical practices? This literature review presents pharmaceutical practices regarding treatment of noncancer pain. We will assess whether these practices changed after the declaration of the crisis. We will also present barriers and facilitators to their implementation in real life to understand the distance between them and current practices. A scoping review of the literature was conducted on

Development and implementation of a best practice alerting process for naloxone prescribing at rural community pharmacies in Wisconsin: A pilot study.

BACKGROUND: The opioid overdose epidemic continues to worsen in the United States, with opioid-related deaths increasing by 13 folds from 2000 to 2022 in Wisconsin. Naloxone, an opioid antagonist, can save lives by reversing opioid overdose in a matter of minutes. However, naloxone access and utilization remain suboptimal. OBJECTIVE: This study examined the development and implementation of best practice alerting (BPA) processes within community pharmacies. This study assessed to what extent the BPA processes (1a) prompted pharmacists to discuss naloxone with high-risk patients; (1b) increase

A comprehensive global review on pharmacist-led strategies to support medication therapy in hypertensive patients.

Hypertension, a global health burden affecting over a billion individuals, significantly contributes to cardiovascular diseases and mortality. Despite the availability of effective antihypertensive therapies, medication adherence remains a critical challenge, particularly in low-resource settings. Pharmacist-led interventions have emerged as an effective strategy to enhance adherence and optimize blood pressure (BP) control. Pharmacists' accessibility enables tailored interventions addressing multifaceted barriers to adherence, such as patient education, complex medication regimens, and commun

Association between state policies enabling pharmacist-led human immunodeficiency virus pre-exposure prophylaxis and need for human immunodeficiency virus prevention: An ecological analysis.

BACKGROUND: Models of pharmacist-led pre-exposure prophylaxis (PrEP) services have been shown to effectively reach populations disproportionately affected by human immunodeficiency virus (HIV). Pharmacist prescriptive authority for PrEP varies across states, limiting the potential scale up of these models. OBJECTIVE: We investigated whether states with the most restrictive policies were also the states that could benefit the most from expanded PrEP access. METHODS: We classified U.S. states and the District of Columbia into 5 distinct policy categories ranked from most restrictive to least r

Assessing pharmacy staff practices and knowledge of off-label drug use in Ministry of Health Hospitals, Saudi Arabia: A cross-sectional study.

Despite the growing prevalence of off-label prescribing, there is limited research exploring pharmacists' actual knowledge and practices, particularly in the Saudi context. Existing literature mainly focuses on physicians, leaving a gap in understanding pharmacists' readiness to handle such prescriptions. This study aimed to assess the knowledge and practices of pharmacy staff in Saudi Arabia regarding off-label drug use and examined the impact of education, age, gender, and experience on these outcomes. A cross-sectional survey was conducted over 6 months (February-July 2024) targeting regist

Clinical decision support tool evaluation: Correction of direct oral anticoagulant dosing errors by clinical pharmacists and physicians.

BACKGROUND: Inappropriate dosing occurs in up to 20% of direct oral anticoagulant (DOAC) prescriptions and is associated with bleeding, hospitalization, and death. Understanding why DOAC dosing errors arise after the initial prescribing period and how clinicians decide to adjust doses may help improve future antithrombotic stewardship efforts. OBJECTIVES: This study aimed to describe responses by clinicians to asynchronous electronic health record notifications regarding inappropriate DOAC prescriptions that develop after the initial prescribing period. METHODS: As part of a larger randomize

Prevalence of subjective symptoms among hospital pharmacists and association with drug compounding practices.

In Japan, the principal role of hospital pharmacists has changed from that of dispensing medicines for outpatients to provision of clinical pharmacy services for inpatients. A self-administered questionnaire about subjective symptoms, working patterns, work environments and job satisfaction was administered to 495 hospital pharmacists and 84 prefectural office-based pharmacists (control group). The response rates were 63.4% and 90.5%, respectively. Hospital pharmacists showed a higher prevalence of nasal symptoms than that shown by the control office-based pharmacist group. The prevalence rate

Effect of Pharmacist Email Alerts on Concurrent Prescribing of Opioids and Benzodiazepines by Prescribers and Primary Care Managers: A Randomized Clinical Trial.

IMPORTANCE: Policy makers have sought to discourage concurrent prescribing of opioids and benzodiazepines (coprescribing) because it is associated with overdose. Email alerts sent by pharmacists may reduce coprescribing, but this intervention lacks randomized evidence. OBJECTIVE: To investigate whether pharmacist emails to practitioners caring for patients who recently received opioids and benzodiazepines reduce coprescribing of these medications. DESIGN, SETTING, AND PARTICIPANTS: Randomized clinical trial (intention to treat) conducted in 2019-2021 of patients and their practitioners (pres

Evaluation of professional pharmacy activities under advanced practice agreements for inpatients at the McGill University Health Centre.

BACKGROUND: A limited number of jurisdictions worldwide authorize independent pharmacist prescribing. Legislative reforms in Quebec (Canada) now empower pharmacists to independently perform certain professional activities. Since 2021, Advanced Practice Agreements at the McGill University Health Centre have enhanced pharmacists' clinical autonomy, though the scope of each agreement varies across units. Limited evidence exists on how these agreements influence prescribing practices. This audit aimed to describe pharmacist-written prescriptions at the McGill University Health Centre, and examine

Pharmacist-Led Antimicrobial Stewardship Optimizes Antibiotic Prescribing for Pediatric Community-Acquired Pneumonia in Vietnam: A Quasi-Experimental Before-and-After Study.

BACKGROUND: Suboptimal antibiotic prescribing for pediatric community-acquired pneumonia (CAP) contributes to avoidable harm and antimicrobial resistance, yet evidence on pharmacist-led stewardship in low- and middle-income countries remains limited. The objective of this study is to evaluate the impact of a pharmacist-led antimicrobial stewardship intervention on antibiotic prescribing appropriateness for pediatric CAP in Vietnam. METHODS: We conducted a quasi-experimental before-and-after study at a provincial maternal and pediatric referral hospital. Using monthly stratified systematic sam

Clinical rule-guided pharmacists' intervention in hospitalized patients with hypokalaemia: A time series analysis.

WHAT IS KNOWN AND OBJECTIVE: Physicians' response to moderate and severe hypokalaemia in hospitalized patients is frequently suboptimal, leading to increased risk of cardiac arrhythmias and sudden death. While actively alerting physicians on all critical care values using telephone or electronic pop-ups can improve response, it can also lead to alert fatigue and frustration due to non-specific and overdue alerts. Therefore, a new method was tested. A clinical rule built into a clinical decision support system (CDSS) generated alerts for patients with a serum potassium level (SPL) <2.9 mmol/L w

Insights Into Pharmacist-Led Deprescribing of Benzodiazepines and Related Drugs: A Qualitative Study With Physicians and Pharmacists.

While deprescribing benzodiazepines and related drugs (BZRDs) is crucial for preventing prolonged use and their associated adverse effects, it presents challenges from a healthcare perspective, because of limited resources and time. Recently, a pharmacist-led deprescribing of BZRDs in the treatment of insomnia was introduced in Helsinki's primary care health centres. To explore pharmacists' and physicians' insights, qualitative semi-structured interviews were conducted via Microsoft Teams with physicians and pharmacists involved in a pharmacist-led deprescribing. The interviews were recorded,
